In the current restructuring of agro-food systems quality is seen as increasingly important and in the United Kingdom this is evidenced by the growth in quality assurance schemes (QAS). The aim of this paper is to critically examine the process of introducing quality through QAS in the UK. This is done by identifying and analysing the key discourses in the farming and food industries surrounding this process. Drawing on Farmers Weekly as a data source, four main discourses are identified and analysed. These are discourses of organisational change within the agro-food chain; discourses surrounding the definition of quality; discourses of farmer acceptance of, and resistance to, QAS; and discourses which construct a particular representation of consumers. The implications of each of these discourses for the development of QAS are also discussed and key issues for further research identified.

Abstract

This paper explores the rising cost of care services in affluent countries, with a particular focus on nursing. It suggests that competitive pressures lead to forms of ‘over-commodification’ that can both lower the quality of care and lead to deterioration of the work environment. While immigration and automation can help reduce costs, they can also pose threats to care quality. Care providers and care recipients need to collaborate in efforts to set and enforce high quality standards.

This essay examines a subset of Reconstruction-era cases decided in Southern appellate courts that dealt with the legacies of slavery. Drawing on suits that either described emancipation as legal death or considered black civil rights, it argues that judges placed freedpeople into a distinct legal category – the former slave – in order to preserve the antebellum legal status quo. Opinions differentiated free-born citizens from those who acquired rights after emancipation, and marked freedpeople with stains of their previous servitude, which limited their potential for the equal enjoyment of newly acquired civil rights.

Pornography consumption is consistently associated with lower marital quality. Scholars have theorized that embeddedness within a religious community may exacerbate the negative association between pornography use and marital quality because of greater social or psychic costs to porn viewing. As a test and extension of this theory, I examine how being married to a religiously devout spouse potentially moderates the link between respondents' reported pornography consumption and their marital satisfaction. Data are taken from the 2006 Portraits of American Life Study. In the main effects, porn consumption is negatively related to marital satisfaction, while spousal religiosity is positively related to marital satisfaction. Interaction effects reveal, however, that spousal religiosity intensifies the negative effect of porn viewing on marital satisfaction. These effects are robust whether marital satisfaction is operationalized as a scale or with individual measures and whether spousal religiosity is measured with respondents' evaluations their spouses' religiosity or spouses' self‐reported religiosity measures. The effects are also similar for both husbands and wives. I argue that for married Americans, having a religiously committed spouse increases the social and psychic costs of porn consumption such that marital satisfaction decreases more drastically as a result.

Findings, issues, and lessons learned about program evaluation are examined from a national qualitative study of 15 faith-based human service programs targeting those in need in urban areas. Using a grounded theory design, five properties emerge as part of the evaluation network: (1) philosophy of accountability, (2) legitimacy, (3) evaluation design, (4) feedback loop, and (5) barriers to evaluation. While funders expect measurable outcomes to evaluate service effectiveness, respondents acknowledge other competing expectations of multiple constituents in religious and secular communities. What emerges is an excellent example of managing multiple program evaluation demands in programs that are particularly facile at process evaluation in the interest of quality service and relationship building. The article concludes with important lessons learned about the process of program evaluation.

Testosterone therapy has been used for more than 60 years in the treatment of male hypogonadism. The classical forms of hypogonadism are comprised of primary testicular failure or insufficient testicular stimulation due to the lack of pituitary gonadotropins. Typical causes of primary hypogonadism are Klinefelter's syndrome, anorchia or acquired disturbances of testicular function. Secondary hypogonadism is characterized by insufficient production of pituitary gonadotropins, due either to pituitary failure or defects at the hypothalamic level. It is unequivocally accepted in clinical practice that any male with inadequately low testosterone production for his age will require androgen therapy. In addition to the classical forms of hypogonadism, the past decade of research has clearly demonstrated that, with increasing age, many men will suffer from decreasing testosterone production. About 15-25% of men over the age of 50 years will experience serum testosterone levels well below the threshold considered normal for men between 20 and 40 years of age. Studies substituting testosterone in elderly men with low serum testosterone have shown that men with clinical symptoms identical to the symptomatology of classical hypogonadism will benefit most from such therapy. Therefore, it is the general consensus to treat men with age-related hypogonadism only when clinical symptoms are present that can be potentially corrected by testosterone administration. Until recently, intramuscular injections of esters, such as testosterone enanthate, have been the mainstay of testosterone therapy. The introduction of testosterone patches has not challenged this approach, since many users of patches suffer from moderate to severe skin reactions. Some oral testosterone formulations have proven to be problematic, as absorption can be variable, bioavailability is frequently poor, due to the first-pass effect of the liver, and frequent administration is often required<citeref rid="b1"><emph>1</emph></citeref>. Oral testosterone undecanoate avoids, at least partially, the first-pass effect of the liver. However, plasma testosterone levels generally undergo large fluctuations<citeref rid="b2"><emph>2</emph></citeref>. The large fluctuations in serum testosterone levels caused by conventional intramuscular injections result in unsatisfactory shifts in mood and sexual function in some men, which, combined with the frequency of injections, make the intramuscular mode of delivery far from ideal. Recently, a hydroalcoholic gel containing 1% testosterone has proven to be as efficient as a testosterone patch, but with fewer side-effects and a higher grade of patient satisfaction<citeref rid="b3"><emph>3</emph></citeref>-<citeref rid="b4"></citeref><citeref rid="b5"><emph>5</emph></citeref>. Doses of 50-100 mg gel applied once daily on the skin deliver sufficient amounts of testosterone to restore normal hormonal values and correct the signs and symptoms of hypogonadism. The gel has been shown to be effective and successful in patients in the United States, who have benefited from its availability for almost 3 years. In the near future, intramuscular injections of testosterone undecanoate will become commercially available. Such injections have a very favorable pharmacokinetic profile, with one injection every 3 months maintaining serum testosterone well within the normal range. In phase III studies, intramuscular testosterone undecanoate proved to be as efficient as testosterone enanthate, with only one-quarter of the number of injections required and more stable serum testosterone levels. Thus, the new application modes - hydroalcoholic gel (for example, Testogel®, Schering AG, Germany) and intramuscular testosterone undecanoate (Nebido®, Schering AG, Germany) - appear to be the methods of choice in the near future, one being very suitable for hormone therapy in elderly men, the other for long-term substitution in classical forms of hypogonadism.

Abstract

The broad spectrum of health services and wellness surrounding lesbian, gay, bisexual, transgender, and queer (LGBTQ+) communities operates within multiple layers of social inequities. Research using intersectionality features numerous research methods purposed specifically for clarity toward increasing health equity. Generated in the scope of various research methods (e.g., quantitative, qualitative, mixed-methods), utilizing intersectionality illustrates potential to explore complex phenomena and outcomes within LGBTQ+?communities. As intersectionality thrives on interdisciplinarity, this article focuses on three overarching purposes: (a) accentuate health equity and social justice across an interdisciplinary scope for LGBTQ+?communities, (b) highlight formative principles grounded in intersectionality, and (c) supplement recommendations for research targeting health equity.

The wine industry is becoming increasingly globalized as consumer demand, capital investment, and industry restructuring lead to higher volumes of trade, greater levels of multinational ownership, and the evolution of new networks of production and consumption that link the four corners of the world economy. While there are some tendencies towards increasingly homogenized and low-cost production, wine is an industry that exemplifies the complex and contradictory elements of globalization. This article outlines some key parameters of globalization drawing on empirical evidence from key cases. In particular, it focuses on the role of geography and how place and scale matter in production, marketing, and consumption. As globalization unfolds, restructuring in the wine industry is leading to the increased economic and social differentiation of rural space. The resultant geographies of place are influenced by a complex combination of local development history, national policy context, and the nature of the insertion of the given locality into global value chains for wine. As such, globalization has and will continue to produce increasingly complex and intricate geographies.

Conclusion In conclusion, I would like to consider some of the common themes in the writings of Freud, Jung, and Lévi-Strauss, and to offer some observations on their historical significance. Firstly, all three theorists were historical pessimists. While it may be true that their historical pessimism reflected their class position as bourgeois social theorists in the age of mass society, I think it is equally important to recognize that utilization of the theory of the unconscious itself creates a paradigm with strongly conservative and anti-utopian implications. Their dependence on the philosophy of Arthur Schopenhauer is significant. His work, early in the nineteenth century, lay the foundations both for the theory of the unconscious and for the historical pessimism that went with it. His metaphysical pessimism lies behind not only Freud's Libidolehre and Jung's psychic energy, but also behind the somber prophetic, cataclysmic imagery employed by Claude Lévi-Strauss from Tristes Tropiques to L'Homme Nu.The following passage, which draws to a finale the pessimism of Tristes Tropiques, a very Schopenhauerian book, is typical: The world began without the human race and it will end without it. The institutions, manners, and customs which I shall have spent my life cataloguing and trying to understand are an ephemeral efflorescence of a creative process in relation to which they are meaningless, unless it be that they allow humanity to play its destined role. That role does not, however assign to our race a position of independence. Nor, even if man himself is condemned, are his vain efforts directed towards the arresting of a universal process of decline. Far from it: his role is itself a machine, brought perhaps to a greater point of perfection than any other, whose activity hastens the disintegration of an initial order and precipitates a powerfully organized Matter towards a condition of inertia which grows even greater and will one day prove definitive. From the day when he first learned how to breath and how to keep himself alive through the discovery of fire and right up to the invention of the atomic and thermonuclear devices of the present day, man has never-save only when he reproduces himself-done other than cheerfully dismantle million upon million of structures and reduce their elements to a state in which they can no longer be reintegrated. No doubt he has built cities and brought the soil to fruition; but if we examine these activities closely we shall find that they also are inertia-producing machines, whose scale and speed of action are infinitely greater than the amount of organization implied in them. As for the creations of the human mind, they are meaningful only in relation to that mind, and will disappear into nothingness as soon as it ceases to exist. Taken as a whole, therefore, civilization can be described as a prodigiously complicated mechanism: tempting as it would be to regard it as our universe's best hope of survival, its true function is to produce what physicists call entropy: inertia, that is to say. Every scrap of conversation, every line set up in type, establishes a communication between two interlocutors, levelling what had previously existed on two different planes and had had for that reason, a greater degree of organization. Entropology, not anthropology, should be the word for the discipline that devotes itself to the study of this process of disintegration in its most highly evolved forms. And yet I exist. Not in any way, admittedly, as an individual: for what am I, in that respect, but a constantly renewed stake in the struggle between the society, formed by the several million nerve-cells which take shelter in the anthill of the brain, and my body, which serves that society as a robot?I have quoted this passage at length because it gives a vivid feeling of the profound metaphysical despair that lies at the roots of Lévi-Strauss' work. In his recent writings and interviews his pessimism has become even more pronounced; he seems convinced that the entire civilized world is moving rapidly and inexorably towards its ecological self-destruction.A second theme that runs through the writings of Freud, Jung, and Lévi-Strauss is the concern with polarities and their dialectical reconciliation or transcendence. Freud's theory was shot through with polarities-one thinks of the dualism of instincts, and the polarities of pleasure/unpleasure, active/passive, subject/object, etc. In the eternal struggle between these immortal adversaries, Life and Death, Super-Ego and Id, Mind and Body, Freud placed the Ego as an integrating and synthesizing principle. Freud's proclivity for dualistic ideas was shared by Jung. The interests of Jung and Lévi-Strauss in the dialectical reconciliation of the opposites was already discussed above. Once opposites are seen to be in relationship, as parts of a system, they cease to be opposites and become polarities.Schopenhauer provides a link to another common theme shared by these writers, the belief that everything is inter-related and mutually attuned. Schopenhauer believed that physical causality was only one of the rulers of the world; at a deeper level there was a kind of universal consciousness, compared to which individual consciousness was rather like a dream compared to wakefulness. For all of these thinkers individual consciousness was based on a larger system of intercommunications, but whereas this theme was not stressed by Freud, it became central in the works of Jung and Lévi-Strauss.As we have seen throughout this essay, Freud, Jung, and Lévi-Strauss were committed to the notion that there is a hidden order in the mental and cultural life of mankind, and they were convinced that this hidden order can be discovered by human reason. Behind the diversity of human cultures they believed that they saw an underlying unity, and they explained this unity in terms of what they believed to be a universality of unconscious processes of the human mind. Freud and Jung tried to explain their notions of the unconscious in terms of energy, drawing their models from physics. Freud's libido theory was more physical, Jung's more psychical, but they both remained tied to an energy model. I believe that one of Claude Lévi-Strauss' most important contributions to the social sciences was to liberate the notion of the unconscious from this energy theory. Instead, he spoke of it as being like a language, employing the ideas of system and structure and particularly the concept of the symbolic function drawn from structural linguistics and information theory. In Lévi-Strauss' Structural Anthropology System and Structure are treated as belonging to the realm of Information/Communication rather than as belonging to the realm of matter/energy. Structure is the ensemble of laws which govern the behavior of the system, and the components in the system are largely interchangeable. They do not necessarily derive from the same level of organization as the system which controls their various combinations, permutations and structural transformations. As we saw above, for Lévi-Strauss the unconscious is empty. It is simply a universe of rules similar to the phonological laws that govern languages. In this usage, the unconscious is a term designating a process of the human mind, a process which operates in all human cultures according to the same laws. In fact, the unconscious is nothing but the totality of these laws and relationships. Even the world of symbolism—though it exhibits an infinite variety of contents-is always bound and limited by these structural laws, because all human beings are bound by the same mental constraints. In the Kantian tradition, Lévi-Strauss sees his task as analyzing the operations of the human mind (l'esprit humain) within these contraints, and I think it is fair to say that his work is a kind of critique of sociological and anthropological reason in the same sense that Wilhelm Dilthey's was a critique of historical reason. In this paper I have attempted to show that there was a progressive development in the theory of the unconscious from Freud, through Jung, to Lévi-Strauss. Jung, working in German Switzerland, was more sympathetic to German idealism and historicism than was Freud. In his work Jung blended this German philosophical tradition with French sociological theory. This unique amalgam could have led him to elaborate a depth sociology correlative with his depth psychology, but his search for his own historical predecessors led him to investigate the psycho-historical significance of mysticism, spiritualism and alchemy instead. He was always convinced of the power, importance and significance of the collective representations that guide and shape our perception and experience. Jung wrote: We should never forget that in any psychological discussion, we are not saying anything about the psyche, but that the psyche is always speaking about itself. Modern civilized man's belief in the sole reality of the individual, along with his belief that he is born a tabula rasa, was simply an illusion, a modern myth. At our deepest core level, each of us is united to all mankind and to the history of the human race. Whether we call this deepest level society with Durkheim and George Herbert Mead, the will and its representations with Schopenhauer, the wider self through which saving experiences come with William James, the Unconscious with Freud, or the Psyche with Jung, or the structures of the human mind with Lévi-Strauss, depends on our primary assumptions, metaphysics and temperament. They are but different terms pointing towards the one common transpersonal background structure that makes possible both human experience and communication of that experience.Lévi-Strauss: Tristes Tropiques, p. 397. Since the early 1980s, international migration has moved beyond humanitarian, economic development, labour market and societal integration concerns, raising complex interactive security implications for governments of migrant sending, receiving and transit countries, as well as for multilateral bodies. This article examines the effects of international migration on varied understandings and perceptions of international security. It discusses why international migration has come to be perceived as a security issue, both in industrialized and developing countries. Questions are raised on the migration-security nexus and the way in which the concepts 'security' and 'migration' are used. The real and perceived impacts of international migration upon national and regional security, both in industrialized and developing countries, are analysed. The policies developed by governments and multilateral agencies since the mid-1980s to mitigate the destabilizing effects of certain kinds of international population movement and human displacement are examined. The conclusions stress the need for the establishment of a comprehensive framework of international cooperation among origin and receiving countries and international organizations to address the destabilizing implications of international migration.

Given the turbulent conditions of the early 21st century and the release of data from the 2020 Census, it is an appropriate time to examine contemporary population redistribution trends in nonmetropolitan America. Analysis centers on the major demographic components of population change: migration; and natural increase. The analysis demonstrates that the turbulent economic, social, and now epidemiological conditions of recent years altered traditional demographic trends in nonmetropolitan America. For the first time in history, nonmetropolitan America lost population between 2010 and 2020 because of shifts in migration trends and diminishing natural increase. In contrast, post-censal population estimates suggest that nonmetropolitan population gains exceeded those in metropolitan areas for the first time in 50 years between 2020 and 2021. The recent widespread nonmetropolitan population increases are the result of substantial net migration gains that offset the growing natural decrease fostered by COVID-19. Sustained net migration gains in nonmetro areas provides a demographic lifeline to many counties that would otherwise face depopulation because of accelerating natural decrease. Whether these migration patterns can be sustained remains to be seen.

This qualitative study investigates the perspectives of two cohorts of elementary preservice teachers on citizenship education in today's culturally and globally diverse classrooms. Both cohorts were enrolled in the same university; however, one group participated in an urban-based teacher education program designed around the needs of urban school children. While participants reported varying views of citizenship and offered diverse strategies for incorporating citizenship in the classroom, findings suggested that those participating in the urban-based cohort viewed citizenship more in terms of culture and valued promoting cultural learning and sharing in the classroom. Implications for teachers wanting to teach for cultural citizenship are discussed.

Habermas's idea of a conflict shift from ‘old’ to ‘new’ social movements has been a point of ongoing contention in the field of sociology. The criticisms aimed at his idea of ‘newness’ are numerous and persuasive. Rather than reiterating these debates, this paper argues that the next logical step is to apply Habermas to an analysis of the contemporary labour movement. In order to do this, I develop the notion of the ‘colonization of the lifeworld at work’, and explore the relationship between colonization and union activism in two cases drawn from the current UK context: the Fire Brigades Union (FBU) and the National Union of Teachers (NUT). In exploring colonization empirically, I suggest that it is met by a range of responses, from activism to acquiescence.
