Peer recovery support services (PRSS) in child welfare are being provided by peer mentors in sustained recovery from substance use disorders (SUD) to parents with acute SUD. Previous retrospective interviews demonstrate that peer mentors engage parents in family-centered systems of care through relationships and empowerment. However, the specific profile of services provided is unknown. Personnel challenges and opportunities for persons in recovery serving as peer mentors are described in the literature without understanding the frequency of both. As enthusiasm for hiring peer mentors grows, it is important to understand the specific services provided, the risks and opportunities associated with hiring individuals in recovery, and the impact of mentor services on outcomes. This knowledge can assist in developing training, implementation guides, policies, job expectations, and program evaluation strategies. This is a prospective study of 28 family mentors providing PRSS services to 783 families with child maltreatment and parental SUD over 8 years in a family-centered integrated program with SUD treatment providers. We describe mentor services overall, during the early engagement period, in rural and urban settings, and test the association between services and child/parent unification status at case closure; we identify the proportion of peer mentors that experienced employment challenges and career advancement opportunities. Results demonstrate the complexity of service provision overall and in differing contexts. Face to face visits with children were associated with greater likelihood of parent/child unification at case closure and 64.3% of peer mentors experienced career advancement opportunities. The implications of these findings are discussed.  相似文献

This research involved a small qualitative study focused on family experiences of child protection practice, commissioned as part of a Best Value review of child protection services in a large rural Midlands local authority. Qualitative interviews were undertaken with 18 families who had received child protection services. Findings were mixed in relation to perceived helpfulness of the processes of child protection interventions, with 50% of families reporting some positive bene?t and 22% reporting that such interventions had caused them harm. From the families' perspectives, factors that are associated with positive and negative outcomes are outlined. These include the availability of preventive services, crisis support, respite care, actual provision of services speci?ed in protection plans and an engaging style of practitioners. Conclusions emphasize the value of research focused on family perceptions of services as an important contribution to quality control and service development in child protection service provision. This research provides a three‐way perspective on the experiences and needs of children who are living with and caring for parents with severe and enduring mental illness. The views of children, parents and key workers were sought in order to provide deeper insight into the needs of families and the nature of interfamilial relationships, as well as the relationships between service users and providers. Child protection and medical research has long proposed a link between parental mental illness and the risk to children of abuse, neglect and developmental delay. The inevitability of risk associations is challenged by the research described here and outcomes for children of caring for parents with mental illness are discussed not simply in terms of risk to children but more broadly in respect of, for example, positive parent–child relationships. This paper reports a study that sought to understand what facilitates engagement between parents and child protection workers and to ascertain the relationship between such engagement and intervention outcome. Quantitative and qualitative data were gathered through personal interviews with 131 worker-parent dyads (workers and corresponding parents receiving service) from 11 child protection agencies in Ontario, Canada. Measures included scales for engagement, parental well-being (depression and stress), and worker well-being (burnout, job satisfaction, stress). Outcome measures included perception of child safety, changed parenting practices and satisfaction. A relationship was found between workers' perception of parent engagement and parents' perception of their own engagement, as well as between the perceptions that workers and parents had around their own respective engagement. Workers who were satisfied with service outcomes were significantly more engaged than those who were unsatisfied. Parents thinking that their children were safer as a result of intervention were significantly more engaged than parents who thought that their children were less safe. The strongest reason given by parents for positive change was being able to trust their worker (p < .001) and believing that their worker was knowledgeable about parenting (p < .01). Qualitative data suggested that parents valued experienced workers because they thought such workers could better understand their problems and how to deal with them. Correspondingly, workers felt that experience enabled them to better understand clients' problems and provide more effective support. No relationship was found between parent engagement and parental depression or stress, but the mean depression score for parents placed them at risk of clinical depression. There was a negative correlation between worker stress and engagement. The study demonstrates that engagement between clients and workers is related to positive outcomes (as reported by workers and parents) and supports the contention that promoting engagement is integral to a successful child protection intervention.  相似文献

Child welfare concerns have drifted to an inappropriate focus on crisis intervention and a punitive approach to child protection intervention at the expense of community-based preventive child welfare programs. Recent attempts to divert cases from the child protection system through differentiated response mechanisms have been criticised for failing to provide access to relevant services or preventing vulnerable families from re-entering the child protection process. A tension inherent in providing both child protection and family support within the one agency is also identified as a barrier to effective service delivery. This paper discusses the value of the UK Children in Need approach as a model for enhancing support to children and families outside the statutory child protection system. Information from an evaluation of a trial implementation of the UK Children in Need approach in Victoria is used to discuss the implications for policy and practice of placing responsibility for coordinating a response based on the needs of children and their families within family support services.  相似文献

Abstract

Parents with a history of serious mental illness are vulnerable in many ways and are therefore likely to be accessing services from a range of government and community agencies. The use of multiple services, sometimes with conflicting practice frameworks, can result in sub-optimal management of these families. This study surveyed 77 service providers from a range of government and non-government agencies targeting their views regarding parents with a serious mental illness (specified in this study as schizophrenia, bipolar disorder and psychotic depression). They were asked what they perceived were difficulties for this population, interventions required to meet the needs of this group and finally, barriers to effective service delivery. We found that service providers reported that current services were inadequate for these parents. Parent-based interventions were seen as being more beneficial than those targeting children. Lack of liaison between agencies and lack of coordinated service provision were seen as the greatest barrier to effective service delivery. The results highlight the need for policy planners and service providers to develop strategies to ensure effective coordination between services that work with this population.  相似文献

Successfully engaging clients in the helping process is a critical task for child welfare practitioners. Drop-out and noncompliance rates in child welfare services are high and lead to high rates of removal of children from their families and to eventual termination of parental rights. Although no known interventions guarantee treatment compliance, this review of the empirical literature delineates critical components of engagement in child welfare services. Effective engagement strategies, including service components and caseworker qualities and behaviors, are identified as contributing to the positive case outcomes of treatment compliance, family preservation, and placement prevention. The unique needs of neglectful parents are also examined, with recommendations for practice.  相似文献

There is a growing understanding that preventative child welfare efforts can be both more cost effective and are more desirable philosophically than strictly remedial programs. The Family Preservation Act provides funds to assist communities in strengthening and preserving at-risk families. In order to implement this program effectively, local communities must design new programs and procedures which will be both attractive to consumers and effective in meeting their needs. This study it identifies problems with contemporary child and family services and suggests solutions to those problems. This study uses a focus group format and presents ideas generated by those individuals who will be most directly affected by this legislation; front line service providers and service consumers. Several areas of weakness in the current service delivery system are identified, including bureaucratic barriers, limited resources, problematic consumer/provider relations and a range of collaborative issues, both within and between agencies and the communities they serve.  相似文献

Autism spectrum disorder (ASD) diagnoses are increasing, leading to growing demands on the service delivery system to meet the needs of children and families. Further understanding individual enabling components such as perceptions of parents/caregivers and personnel, as well as community-level enabling components such as the availability of and quality of services, may help inform system improvements. In this qualitative study, 19 parents/caregivers of children with ASD in one state in the United States were interviewed regarding their experiences with the ASD service delivery system, as well as their individual responses in relation to these experiences. Emergent themes point to the importance of individual coping strategies such as social support, collaboration with service providers, advocacy and empowerment, determination, and luck. Characteristics of the service delivery system, positive and negative, also were described. Findings can inform the design of future programs, services, and policies to support parent/caregiver coping and improved service delivery.  相似文献

The number of Latino families involved with the public child welfare system has increased exponentially in the last decade. This qualitative study examined one of this population's inherent resources – their social network – and the role it plays when involved with the public child welfare system. Findings reveal that Latino families rely heavily on their network for emotional support, advice/information, and advocacy. Often parents received incorrect or conflicting information or advice grounded on experiences that were very different from the participant's case. In addition, their networks are homophilous; that is, limited to people who are in their same circumstances which limits their ability to access formal sources of advocacy and support. Many parents were also genuinely interested in helping other parents with open cases. Practice recommendations are made as it relates to actively engaging Latino families in their case process and supporting Latino families by incorporating their networks and genuine interest in helping others into child welfare service delivery.  相似文献

Divorced mothers and their school-aged children in 50 single-mother families and 37 stepfather families reported on mothers' ex-partner relationships, children's relationships with both parents, and children's well-being. A 2 (family structure) × 2 (gender) multivariate analysis of variance revealed a main effect of gender: Mothers with sons report higher levels of ex-partner relationship satisfaction. An interaction effect was also found: Boys in single-mother families report more acceptance and fewer conflicts than boys in stepfather families. Multiple regression analysis revealed that mother–ex-partner relationships are associated with children's problem behavior, whereas child–nonresidential father relationships are associated with positive aspects of children's well-being. The results indicate that family structure itself is not associated with child adjustment. What matters most is the quality of family relationships.  相似文献

This study uses data on the experiences of families involved with child welfare services to examine the nature of housing problems and needs among these families and whether housing status affects case outcomes. First, the article describes the housing difficulties faced by two distinct child welfare service populations: families receiving voluntary in-home services and families with children in court-ordered out-of-home care. Second, the study demonstrates the relationship between housing problems and the likelihood of family reunification for children in out-of-home care. The findings have implications for the delivery of child welfare services and the provision of housing assistance to low-income families with children.  相似文献

The recent emphasis on permanency planning in child welfare has strengthened the resolve of policy makers and service providers to serve the families of dependent children better. Permanency planning asks child welfare services either to reunite children in care with their families of origin or to find permanent substitute families for children, but the goal of a permanent family for every child has been much easier to articulate than it has been to achieve.  相似文献

Child protection work is complex and demanding and presents both parents and workers with a number of emotional and relationship demands. Although it is recognised that a good working alliance between child protection workers and parents is associated with therapeutic success and service user satisfaction, some current developments, including the growth of an audit culture with its attendant emphasis on targets and performance, appear to undermine the worker's ability to establish good relationships and working alliances. To the extent that poor‐quality working relationships and increased emotional distance reduce the worker's capacity to be empathic, levels of stress for both parents and workers are likely to remain high. Heightened stress reduces the capacity of parents and workers to keep at‐risk children in mind and in focus. This reduces their safety. The paper revisits the value of containment as a way of processing and managing difficult feelings which, along with more recent therapeutic concepts such as mentalisation, encourages workers to keep the parents' feelings in mind as well as hold the child in mind for the parent as a psychological, meaningful and mentalising being. Helping parents think about their feelings and understand, indeed celebrate, their children as burgeoning, independent psychological entities increases understanding, reduces stress and plays a part in helping keep children safe. This article discusses the National Center on Addiction and Substance Abuse's CASA Safe Haven, an evidence-based, community-driven intervention program for children and families in child welfare whose lives have been adversely affected by substance abuse, and for staff in the agencies that work with them. CASA Safe Haven builds collaborative relationships that feature a blend of multidisciplinary teams that share responsibility for helping families; family group conferencing, in which families are equal and welcome participants in designing and driving a service plan; and the influence of family court to hold families and service providers accountable for progress. SUMMARY. In a service such as day care, different beliefs about appropriate child care and family patterns make quality a relative concept. However, service providers should work towards overall aims and specific goals established on values and beliefs about day care. It is argued that the day care service needs to tackle the definition of quality, for the job satisfaction of the service providers, for the well being of the children involved and for the satisfaction and peace of mind of the parents. In this study, we examined the extent and type of contact with birth family in adoptive families headed by sexual minority and heterosexual parents prior to or at the time of placement, following placement, and currently. Data were drawn from the Modern Adoptive Families project, a nationwide, non-random survey of adoptive parents' beliefs and experiences that was conducted from 2012 to 2013. The current sample consisted of 671 families headed by heterosexual parents, 111 families headed by lesbian parents, and 98 families headed by gay male parents whose oldest adopted child was less than 18 years old and who was placed domestically either from the public child welfare system or from a private agency or independent adoption facilitator. For child welfare adoptions, sexual minority parents reported higher levels of contact and tended to have more positive relationships with birth family compared to heterosexual parents. Fewer differences by family type were found for private agency adoptions. Higher rates of contact and more positive relationships with birth family were found for private domestic placements compared to those from foster care. Secondary analyses suggest that family demographic and adoption placement differences between sexual minority- and heterosexual-parent families do not account for family type differences in contact with birth family. Child welfare professionals increasingly recognize the importance of the client's perspective in evaluating service outcomes, but there is a dearth of well-developed instruments that meet the needs of service providers and planners. This article describes the implementation of a measure of primary caregivers' satisfaction with mandated child welfare services. This measure is significant for its flexibility regarding the nature of the child welfare population to be surveyed and for its attention to cultural aspects of the clients' experience. The instrument is an adaptation of Harris, Poertner, and Joe's (2000) client satisfaction measure and includes selected items from related instruments in the field. The measure was developed with the help of an Aboriginal child welfare agency and piloted with that agency's clients. The adapted instrument also includes items requested by the pilot testing agency to reflect high quality child welfare practice including specific attention to the clients' racial, ethnic, and cultural identity. This project furthers Harris et al.'s (2000) prior work by using the instrument in another country (Canada) and with diverse populations, thereby adding to the body of evidence supporting the development of this measure. Future research with larger samples could further enhance the validation of the measure and its usefulness to policy makers and practitioners.  相似文献

This study aims to investigate differences in perception among Kuwaiti parents, teachers, and employees regarding the services that are needed in different special education settings. The participants (n = 135) included 45 parents of children with intellectual developmental disabilities (IDD), 63 teachers in special education schools, and 27 employees working in the Higher Council of the Disabled. A survey was conducted using Hassan’s (The needs of parents of children with disabilities in relationship with child’s age, gender, and type of disability, 1992) Arabic version of the Family Need Inventory. The findings showed differences in perception among parents, teachers, and employees regarding what they consider to be the most important services for families of children with IDD. Another result showed significant differences in perception among the three groups regarding services (match or/and mismatch of responses), except for 3 out of 35 types of services. Suggestions and implications are discussed to provide information to social service providers to help them evaluate the laws that pertain to disability in Kuwait. The findings of this study will help social service providers determine and implement laws that meet the needs of parents of children with IDD and give them the right to be full partners in their children’s education.  相似文献
