A model of social work classification in health care

Abstract

The introduction of casemix funding for Australian acute health care services has challenged Social Work to demonstrate clear reporting mechanisms, demonstrate effective practice and to justify interventions provided. The term ‘casemix’ is used to describe the mix and type of patients treated by a hospital or other health care services. There is wide acknowledgement that the procedure-based system of Diagnosis Related Groupings (DRGs) is grounded in a medical/illness perspective and is unsatisfactory in describing and predicting the activity of Social Work and other allied health professions in health care service delivery. The National Allied Health Casemix Committee was established in 1991 as the peak body to represent allied health professions in matters related to casemix classification. This Committee has pioneered a nationally consistent, patient-centred information system for allied health. This paper describes the classification systems and codes developed for Social Work, which includes a minimum data set, a classification hierarchy, the set of activity (input) codes and ‘indicator for intervention’ codes. The advantages and limitations of the system are also discussed.

Responding to student mental health concerns in social work education: reflective questions for social work educators

In this paper, we explore ways in which social work educators might respond to students who report that mental health issues underlie their difficulty in meeting core competencies, or otherwise use the language of mental health to describe their struggles to succeed in social work programs. We discuss various trends in policy responses in Canada, the US, the UK, and Ireland. While there are general policy trends, it is clear that responding to these kinds of issues requires the development of highly flexible and situated policy processes that can respond to student realities, concern for students’ rights and privacy, and an awareness of potential discrimination against students. These processes also need to meet the specificities of practicums, particular institutional policies, the mandates of relevant professional bodies, and the precise local legislative framework that shapes these situations. Given these varying contexts, in this conceptual paper, we used a framework on disability that is informed by critical theory to engage existing school policies and propose a set of reflective questions that can guide schools of social work to create an overall responsive environment. These reflective questions are designed to help social work educators balance the rights and needs of students with the professional and institutional demands that students meet core competencies in their education.     

Parental substance misuse and child care social work: research in a city social work department in England

This research set out to establish evidence about the scale and impact of and response to parental substance misuse in child care social work teams in a city social services department in England. The article draws on some aspects of the data collected in the research, which includes: a snapshot survey of all child care social work caseloads in the city; group interviews with practitioners and parents in recovery; individual interviews with parents using a pilot project that focused on parental substance misuse. The research provides evidence of parental substance misuse as a key factor that needs greater consideration within child care social work assessments and as an issue to target in developing preventative responses to child welfare concerns. Child care social workers are shown to need specialist support in undertaking this task to best effect. Copyright © 2004 John Wiley & Sons, Ltd.     

Navigating stormy waters: Challenges and opportunities for social work in mental health

The reforms driven by the National Mental Health Strategy have created both opportunities and challenges for social work as a profession. This paper examines the rapidly changing context for practice in mental health, including policy change, and developments in the education and training of the mental health workforce. Key practice issues identified for social work include: (i) the need to establish a viable paradigm for practice; (ii) a more positive response to the challenge of evidence-based practice models; (iii) a national agenda for education and training; and (iv) the importance of working collaboratively with consumers and families in a way that values their human rights and the lived experience of mental illness.     

Fair play: creating a better learning climate for social work students in social care settings

This paper describes a pilot study and subsequent evaluation undertaken by the author which arose from his experiences as a practice teacher working with a student in a day care facility. The catalyst for the study arose from one student's comments about the confusion he and others on the placement site experienced regarding his role as a social work student in a setting where there was no defined social work role.

It led to a study of students' views in a range of settings across South & East Belfast Trust, some of which may be deemed to be ‘social work settings’, i.e. with a defined social work role, while others may be considered ‘social care settings’, i.e. with no defined social work role. The aim of the study and subsequent evaluation was to consider problematic issues which arose for students in social care settings and how the quality of learning opportunities for them could be improved.     

Why social work should care: Television violence and children

There is an abundance of research on the influence of television violence on children. The convergence of research demonstrates an association between heavy viewing of violence and aggressive behavior. Despite thirty years of research, regulatory policy has failed to decrease violence on children's television. Children exposed to high rates of violence on television may be at greater risk of developing school and social problems. This paper highlights both research and theory which explains the relationship of television violence to children. It further makes the case that television violence should be an important and legitimate concern for social work.Ms. Lazar is an Associate Professor of Social Work at the University of Southern Maine and a Doctoral student at Simmons School of Social Work.     

Applying health determinants and dimensions in social work practice

Dimensions and determinants related to physical health, mental health and well-being can be used as tools in social work practice. These frameworks are useful for assessment, planning and intervention activities in complex client situations, alerting social workers to consider diverse features and processes that influence well-being in people's lives. They can be applied in social work with individuals, families, groups, and communities. In our view health and well-being need to be viewed holistically, broadly and in each unique situation, changing over time over the life course. Literature based on key word searches of concepts we use in our teaching and research and that has informed our conceptualisation of dimensions and determinants in physical health, mental health and well-being, is reviewed. A case illustration is presented to illustrate how the dimensions and determinants are used to inform practice and compare and apply alternative frameworks. Finally, the implications of these and other frameworks for social work, not only in health care settings, but also in other fields of social work practice, are discussed.     

Sexuality,child care and social work education

Whilst it is important to acknowledge the holistic nature of sexuality and its relevance in all areas of social welfare and service provision, this paper is specifically concerned with lesbian, gay and bisexuality and child care. The issues discussed will be common to all service user groups but the insidious nature of the 'contamination or corruption' theories of homosexuality are particularly inflammatory for those working in the child care field and the young people with whom they work. This paper argues that the needs of lesbians and gay men, particularly young people, will continue to go unmet unless lesbian and gay issues are addressed explicitly in social work education and in a way which does not oversimplify what is an extremely complex task. Anti-discriminatory and anti-oppressive practice are central features of the academic and practice curriculum; however, it is not simply a matter of adding lesbian and gay issues to the ever-increasing list of oppressions. The ideology of heterosexism and a legal framework which endorses the discrimination of lesbians and gay men create additional challenges to working with this group of people. As social work education becomes increasingly government and agency led, with a competency model of assessment ruling the day, the realities of what social workers can achieve need to be acknowledged if we are not to be accused of perpetuating meaningless rhetoric; playing into the hands of those who refer to political correctness.     

Conceptualizing anxiety among social work students: implications for social work education

Student anxiety is frequently discussed within social work education. Due to its potentially devastating impacts, social work educators need to have a full understanding of anxiety in this context, in order to determine the best way to support students. Research on student anxiety from a broad body of literature provides implications for social work classrooms and field education. Key underlying concepts from previous social work and related research on student anxiety inform a conceptual model to help educators develop a more complex understanding of student anxiety. Implications for further research and important next steps in the development of support strategies for social work students are identified.     

Paying for the health and social care of the elderly

The aging population of Japan is causing serious concern among social policymakers. The most urgent issue is to find a way to pay for the health and social care of the frail elderly. After universal coverage of pension and health insurance was achieved, but just before the economic growth rate was considerably slowed, in part, because of the oil crisis, the Japanese government more than doubled pension benefits and made medical care for the elderly free. Since the early 1980s, the government has tried hard to cut and control these benefits, only with moderate success. With a consumption tax rate of only 5%, rather than the proposed 7%, the government is now considering establishing a new health and social care insurance scheme for the elderly to finance the increasing cost of their care.     

Legal and social work approaches to sibling separation in foster care

As an important subsystem in the families of child welfare clients, siblings deserve greater attention. This article identifies gaps in our knowledge about siblings, notes social trends that call for increased responsiveness to their needs, and reviews social work and legal opinions concerning their separation in foster care. The perspectives of the two professions are compared, and implications for child welfare are considered.     

Policy-practice in social work and social work education in Israel

Policy-practice is a form of social work intervention that is intended to influence social policy. It is linked to an understanding of the role of social workers which places the struggle for social justice at the forefront of social work activity. However, this form of social work intervention has remained on the sidelines of social work practice and education in most welfare states. This paper seeks to understand the role that policy-practice and social policy play, and have played, in social work and social work education in Israel. The findings indicate that, despite a growth in interest in the political role of social workers in Israel during the 1970s, policy-practice has remained a mode of practice adopted by a minority of members of the profession in Israel. An empirical study of the curriculums of the schools of social work in Israel indicates that this is the case for the study program in most of the schools. The reasons for this can be linked primarily to the overwhelming influence of American social work upon the development of the Israeli profession and to the process of liberalization and privatization of the Israeli welfare state in the last two decades.
