The role of supervisors to aid injured workers, access health care, and provide reasonable accommodation may prevent prolonged disability among workers reporting musculoskeletal pain. Although supervisor training has been a common element of broad-based ergonomic interventions to prevent injuries, the impact of supervisor training alone to improve injury response has not been studied. In a controlled design, 11 supervisors in an intervention group and 12 supervisors in a delayed intervention control group from the same plant were provided a 4-hour training workshop. The workshop emphasized communication skills and ergonomic accommodation for workers reporting injuries or health concerns. Workers' compensation claims data in the 7 months before and after the workshop showed a 47% reduction in new claims and an 18% reduction in active lost-time claims versus 27% and 7%, respectively, in the control group. Improving the response of frontline supervisors to employees' work-related health and safety concerns may produce sustainable reductions in injury claims and disability costs.  相似文献

OBJECTIVE: To evaluate the effect of a community-based injury prevention program on work-related injuries. STUDY DESIGN: A population-based quasi-experimental design was used with pre- and post-implementation measurements in an intervention and a control area. The program was based on cross-sectoral participation in detecting and taking action against work-related injuries. Change in the relative risk of injury was estimated by the odds ratio. RESULTS: The relative risk of work-related injury decreased following program exposure, whereas a tendence toward increase was observed in the control area. Middle-aged men accounted for the largest decrease in injury risk. Among the women, a decrease was observed only in the upper middle-aged group. No female-dominated occupational area showed a decrease in injury risk following exposure to the program. CONCLUSION: The study showed that the community-based approach had a general effect on the incidence of work-related injuries.  相似文献

The purpose of this study was to investigate and evaluate the current workplace management of rotator cuff injuries in a manufacturing plant. The secondary aims were to examine the impact of the company's return-to-work processes, compare outcomes to current industry standards for work (re)entry and to identify the components that characterized this workplace-based return-to-work (RTW) program. This investigation involved a case study approach comprised of an examination of the program context using interviews, onsite visits, a document review and a retrospective analysis of the RTW experiences of 184 workers with shoulder injuries. Findings revealed that the workplace-based RTW program was consistent with and shaped by the organizational culture of problem solving, knowledge exchange and equitable participation of workers, supervisors and health professionals. These components contributed to the program in achieving the following outcomes for workers with shoulder injuries. One-third of workers were placed on modified duties within three days, 56% of workers who engaged in an early RTW program returned to work within one month. Overall, 87.8% of workers with rotator cuff injuries successfully returned to pre-injury work. The implications of developing capacity for workplace-based programs to manage injuries at work are discussed.  相似文献

Using data from the 4-year American Dream Demonstration, this study compares saving performance and program participation of banked (n = 1,538) and unbanked participants (n = 466) enrolled in 14 IDA programs. The study shows banked participants had $2.74 higher average monthly net deposit (p < 0.05); 5% higher deposit frequency (p < 0.001); and 42% less odds of drop out than unbanked participants (p < 0.001). Moreover, program characteristics such as financial education, monthly saving targets, peer group meetings, and direct deposit are important predictors of program performances. Individual characteristics such as race/ethnicity, home ownership, and income are significantly associated with program performance.  相似文献

This 9 month prospective study, conducted at the US Army Sergeants Major Academy (USASGMA), examined the association of selected psychological variables (e.g., measures of tension/anxiety, sleep disturbance, Type A behavior pattern) with injury occurrence and physical performance in 126 soldiers. ANOVA and logistic regression analyses revealed significant relationships between: 1) Traumatic injury occurrence and mean tension/anxiety scores, 2) Mean self-reported sleep disturbance scores and traumatic injury occurrence, 3) The Type A behavior pattern (abbreviated Jenkins Activity Survey) and number of sit-ups repetitions completed in 2 minutes, one component of the Army Physical Fitness Test (APFT), 4) The Type A behavior pattern and total score APFT. No significant associations were found for mean tension/anxiety scores and overuse injuries, or Type A behavior pattern and two mile run time or number of push-up repetitions completed in 2 minutes. These data suggest traumatic injury occurrence is influenced by tension/anxiety and disturbances in sleep habits. Additionally, individuals with higher Jenkins Activity scores (characteristic of the Type A behavior pattern) perform better physically.  相似文献

Many intervention programs, including physical activity programs, have been developed to deal with youth involvement in delinquency. The current study explored whether youth participation in sport and physical activity programs reduces their involvement in delinquent behaviors. It examined the interaction effects of the features of the sports program with participation in the sports program. The sample consisted of 126 Israeli adolescents aged 13–18 (M?=?15.68, SD?=?1.32) who completed questionnaires about involvement in delinquency at the beginning of their sports program and again 6 months later. We found significant reductions in adolescents’ involvement in all the delinquent acts explored: crimes against a person; crimes against property, and public disorder crimes. However, no interaction effects were found between program features (sport type; program intensity; training and supervision in the program; and interaction with community services) and participation in the sports program. The findings highlight the importance of including sports programs in the interventions provided for at-risk youth and call for further investigation of the factors that may increase the benefits provided by participation in physical activity programs.  相似文献

This research explores the effects of participating in the Rebuilding Seminar on postdivorce adjustment and the general health of divorced women in Arak, Iran. For this quasi-experimental study, 15 participants were recruited through convenience sampling. Prior to the participation in group intervention, participants were asked to complete a demographic questionnaire, the Fisher Divorce Adjustment Scale (FDAS), and the General Health Questionnaire (GHQ–28). Participants then completed the 10 weekly, 2-hour group intervention program. After the completion of the group intervention, postintervention questionnaires were administered. Results indicate that this particular educational program affected adjustment in participants on all subscales of the FDAS, except for social self-worth. The results also suggest that the intervention program was effective on all subscales of the GHQ–28, except for somatic symptoms. Due to numerous changes in the life of divorced women and the stressful nature of divorce, designing and implementing effective intervention plans seems necessary and constructive.  相似文献

The present study was designed to develop an innovative motivational intervention (based on Self-Determination Theory and Social Cognitive Theory) to increase physical activity (PA) in underserved adolescents. Sixty-four adolescents (35 females, 29 males; 50% minority; 65% on reduced lunch program; ages 11-13 yr) participated in either an 8-week motivational intervention after-school (n = 32) or a typical after-school program (n = 32). The conceptual framework for the intervention targeted the social environment (perceived autonomy, perceived social support, participation, fun), cognitive mediators (perceived choice, self-efficacy, and relatedness/belongingness), and motivational orientation (intrinsic motivation, commitment, positive self-concept). Formative evaluation data was collected by staff through daily forms throughout the 8-week program and through observational data completed by independent objective observers during 2 weeks of the program. The major themes that were identified addressed theoretical concepts regarding the intervention and logistical issues in delivering the intervention. The data revealed information regarding the importance of the cognitive appropriateness of the PA and motivational activities, the environmental climate for promoting nurturing relationships, developing specific strategies for increasing intrinsic rather than extrinsic reinforcement, and developing methods for preventing social "cliques" and gender conflicts to maintain an appropriate level of support in the social climate. Themes for training staff included focusing on team building, leadership, and nurturing. This formative evaluation is being used to formalize a randomized trial to test the effects of a student-centered motivational intervention on increasing PA in underserved 6th graders.  相似文献

Previous research indicates that students who drop out of high school are at risk for a number of negative outcomes. Both ecological and individual risk factors contribute to students' propensity for dropout. This study examined the effectiveness of a targeted dropout prevention program at improving academic achievement and investigated whether improved social support and emotional competencies partially mediated the association between intervention participation and student outcomes. A sample of 110 middle and high school students (55 intervention participants and 55 students in a matched control group) completed surveys investigating their social-emotional assets and schools provided academic data for participants. Structural equation modeling revealed that students who participated in the program had higher academic achievement compared to the control group. Prior levels of academic achievement moderated the effectiveness of the program with students with low initial levels of academic achievement benefiting more. Emotional competencies and social support did not mediate the relation between participation and achievement. The intervention was related to improved academic achievement among participants; however, the means by which it was effective was unclear. Given the evidence that the program was more effective for some students than others, targeting interventions to meet specific needs of students may be advantageous.  相似文献

A post-offer screening program jointly developed by the Occupational and Physical Therapy Departments (University of Illinois at Chicago) was first implemented in March 1998. A total of 712 screens were completed from 3-01-98 through 2-28-01. A quasi-experimental design was utilized to analyze changes in Workers' Compensation costs between the three-year period prior to, and the three-year period following program inception. Additionally, injury rates and mean cost per injury were compared between injuries sustained between 3-98 and 3-01 which were incurred by workers who had passed screening and those that had not been screened. Finally, the tabulated cost per screen and reduction in Workers' Compensation costs allowed derivation of a dollar spent/ dollar saved ratio. Dramatic declines in number of injuries, total costs and mean cost per case occurred in the three-year period following program inception. Mean cost per case also dramatically declined comparing injured workers who had passed screening versus those who had never been screened. Cost savings were over US dollars 18 per dollar spent on the program. While a number of mitigating variables may have accounted for some of the profound effects noted, decrease in average cost per case (indicating possible reduction in severity of injury) was consistent with existing literature.  相似文献

The following case study describes an ergonomics intervention program, designed and implemented by an occupational therapist (OT), which included principles of participatory ergonomics, had full support of management and administration, and addressed the work, worker, and environment. The ergonomics program took place in the Sterile Processing Center (SPC) of a 398 bed, mid-western hospital. The SPC was responsible for the collection, sterilization, and reassembling of hospital surgical equipment. The 30 employees of the SPC were vulnerable to injury, having experienced 19 in only two years (of the 19 injuries, 9 were WMSD or ergonomics-related injuries). Over a two and a half year period, the OT and SPC manager conducted ergonomic evaluations, implemented employee training, designed a new SPC department, and modified work processes and equipment. In the year following the training and changes, the SPC staff experienced no injuries. Because of it's success, the program is continuing on a quarterly basis to ensure new problems are addressed quickly, new processes or equipment are evaluated, and new staff are properly trained.  相似文献

Employee health and productivity losses as a result of work-related injury are estimated to be US dollars 1.2 trillion annually to US companies. This is approximately 14.3% of the gross domestic product [6,8,11,35]. Workers' compensation, medical care, and short and long-term disability are a part of these costs. Controlling or eliminating these costs is a problem for US employers [3,6,14,21,29]. The study discussed in this article examined the perceptions of manufacturing employees in identifying factors that influence a return to work after a work-related musculoskeletal injury. The classification of employees who participated in this study were safety professionals, supervisors and workers from the manufacturing industry in central Kentucky. The worker group consisted of material handlers, assembly line workers and quality control inspectors. The participants completed a developed survey instrument, the Return to Work Perception Survey. This survey instrument examined the perception of the participants on factors related to return to work: company policies and procedures, job satisfaction, worker relationships and work environment. The results indicated safety professionals and supervisors perceptions differ from workers on the variables of job satisfaction, worker relationships and work environment. Their perceptions did not differ on the variable relating to company policies and procedures. In addition, the safety professional and supervisor groups rated the items addressing job satisfaction higher than did the worker group. The worker group did not differ from one another on any of the factors. Implications of this study for manufacturing companies suggest (a). identifying those issues contributing to employee job satisfaction, (b). developing a plan for achieving increased job satisfaction and employee recognition at the workplace among all workers, and (c). consider allowing employees to develop new capacities and new learning, thus fostering motivation and job satisfaction [18,20].  相似文献

The purpose of this project was to evaluate the effectiveness of an initial screening and referral process in reducing the impact of musculosketetal injuries among soldiers attending Health Care Specialist training. Musculoskeletal injury among Army Health Care Specialist students have been reported to be approximately 24% for men and 24-30% for women. The highest student visit rate to the troop medical clinic for musculoskeletal injuries, for men and women, occurs during the first week of training. Anecdotal reports indicate that many students arrive for training with existing injuries or symptoms. This project was designed to assess whether formalized early screening, referral, and intervention could 1) identify arriving students who need musculoskeletal injury-related medical attention, 2) reduce the number of students receiving limited duty status during their 10-week training, 3) decrease the total number of limited duty days for students, and 4) decrease the number of students who cannot graduate due to musculoskeletal injury. Students (N=291) from one company were divided into three groups of 97 students. Three methods were used to screen and refer students for medical intervention: 1) traditional (T), 2) by health care providers (HCP), or 3) by Drill Sergeants (DS). Screening by HCP and DSs involved using a new screening tool to identify and consequently refer students with symptoms to a troop medical clinic (TMC) for early evaluation and intervention. Using the screening tool, HCPs identified 92% of students with injuries, while DSs accurately identified 80%. The screening did not reduce the number of students receiving limited duty status, total limited duty days, or the number of students that could not graduate due to musculoskeletal injury ("holdovers") (p>0.05). The screening tool demonstrated good sensitivity and specificity whether conducted by HCPs or DSs. It failed to demonstrate efficacy in reducing the impact of musculoskeletal injuries among Combat Medic soldiers, as measured by limited duty days and holdovers.  相似文献

This article discusses the results of a 1993 national survey of undergraduate social work program directors on the role of alumni in their programs. Respondents indicated that they involved alumni, primarily as field supervisors, campus speakers, and members of curriculum advisory boards. Only 20% of respondents reported social work-specific alumni organizations; most often these were institutions with public affiliations, joint MSW-BSW program structures, and enrollments exceeding 10,000. Having a social work alumni organization was associated with higher rates of alumni participation in field supervision, mentoring, linkage and networking activities, recruitment, and curriculum advisory boards. A profile of the typical social work alumni organization is presented, and the need for more programs to initiate such organizations is examined.  相似文献

Survey data from a national sample of homeless family shelters (N=59)were analyzed to describe the family support programming available to residents. Data were reported on facility and resident characteristics and family support programming, as well as on facility admission criteria and program participation requirements. Four independent variables (length of stay, shelter capacity, sponsorship, and program philosophy) were examined for their relationship to the family-oriented services offered. Facilities sheltered an average of 17 families, with a range of 2–200; the average length of stay for families was slightly over 5 months. Regarding family programming, services of an educational nature were more popular than those focused on providing support or therapy/intervention; services to parents to further adult development were more often offered than those either directed to parents in their parenting roles or to the family as a unit. Virtually all shelters (98%) applied eligibility criteria to families seeking admission. The most common reasons for exclusion were active drug (86%) and alcohol (83%) abuse by a family member; 40% also refused acceptance to families with adolescent males. Seventy-eight percent of facilities mandated participation in some support services. Smaller shelters operated with greater numbers of exclusions (¯x=5.98773; p<.05);larger shelters had higher percentages of mandatory family programming (¯x= 9.21823; p<.06).These findings shed light on shelter directors' beliefs about the etiology of family homelessness and proper steps to solve the problem.  相似文献

The purpose of this study was to examine outcomes for students enrolled in two teen pregnancy prevention programs: the Youth Asset Development Program (YADP); and the Postponing Sexual Involvement Program (PSIP) and identify possible areas for program refinement. The study design consisted of two sets of comparisons: (a) sixth-grade students who completed the PSIP versus the control school; and (b) the YMCA Success Centers where the YADP was introduced versus the control school. Two hundred thirty-seven students were selected to participate in the study. Students who participated in the prevention programs had significantly (p < .05) higher academic achievement, participation in school related activities, and enjoyed school much more than students from the control school. However, no statistically significant differences were found between program participants and the control group when they were compared on personal orientation and judgments about teenage parenthood. Implications for program modification and program effectiveness are discussed.  相似文献

The Adoption and Safe Families Act of 1997 has made child safety an explicit focus in child welfare. The authors describe an automated critical incident reporting program designed for use in foster care and family-support programs. The program, which is based in Lotus Notes and uses e-mail to route incident reports from direct service staff to supervisors and administrators, facilitates timely clinical oversight and risk management and ensures the security of clients' protected health information. The authors present data collected using the program to illustrate how it can be used to monitor abuse and neglect allegations in a foster care program. A survey of users found that the program saved time, was easy to use, and helped manage critical incident reports.  相似文献

Rates of return to active substance use after addiction treatment tend to be high; participation in 12-step fellowships (e.g., Alcoholics Anonymous) reduces relapse rates but many clients do not attend or attend for a short period only. This quasi-experimental study uses repeated measurement to explore the role of presence/absence of on-site 12-step meetings during treatment on post-treatment outcomes. Polysubstance-dependent clients (N = 219) recruited at a program with and one without 12-step on-site, were followed for one year post-treatment. On-site 12-step enhanced 12-step attendance, especially during treatment, and predicted continuous abstinence for the post-treatment year. Holding 12-step meetings on-site is a low-cost strategy that programs should consider to foster post-treatment remission maintenance.  相似文献
