Compared to the remarkable progress in risk analysis of normal accidents, the risk analysis of major accidents has not been so well‐established, partly due to the complexity of such accidents and partly due to low probabilities involved. The issue of low probabilities normally arises from the scarcity of major accidents’ relevant data since such accidents are few and far between. In this work, knowing that major accidents are frequently preceded by accident precursors, a novel precursor‐based methodology has been developed for likelihood modeling of major accidents in critical infrastructures based on a unique combination of accident precursor data, information theory, and approximate reasoning. For this purpose, we have introduced an innovative application of information analysis to identify the most informative near accident of a major accident. The observed data of the near accident were then used to establish predictive scenarios to foresee the occurrence of the major accident. We verified the methodology using offshore blowouts in the Gulf of Mexico, and then demonstrated its application to dam breaches in the United Sates.  相似文献

Accidents with automatic production systems are reported to be on the order of one in a hundred or thousand robot-years, while fatal accidents are found to occur one or two orders of magnitude less frequently. Traditions in occupational safety tend to seek for safety targets in terms of zero severe accidents for automatic systems. Decision-making requires a risk assessment balancing potential risk reduction measures and costs within the cultural environment of a production company. This paper presents a simplified procedure which acts as a decision tool. The procedure is based on a risk concept approaching prevention both in a deterministic and in a probabilistic manner. Eight accident scenarios are shown to represent the potential accident processes involving robot interactions with people. Seven prevention policies are shown to cover the accident scenarios in principle. An additional probabilistic approach may indicate which extra safety measures can be taken against what risk reduction and additional costs. The risk evaluation process aims at achieving a quantitative acceptable risk level. For that purpose, three risk evaluation methods are discussed with respect to reaching broad consensus on the safety targets.  相似文献

Risk-Based Ranking of Dominant Contributors to Maritime Pollution Events   总被引:2,自引:0,他引:2  
This report describes a conceptual approach for identifying dominant contributors to risk from maritime shipping of hazardous materials. Maritime transportation accidents are relatively common occurrences compared to more frequently analyzed contributors to public risk. Yet research on maritime safety and pollution incidents has not been guided by a systematic, risk-based approach. Maritime shipping accidents can be analyzed using event trees to group the accidents into "bins," or groups, of similar characteristics such as type of cargo, location of accident (e.g., harbor, inland waterway), type of accident (e.g., fire, collision, grounding), and size of release. The importance of specific types of events to each accident bin can be quantified. Then the overall importance of accident events to risk can be estimated by weighting the events' individual bin importance measures by the risk associated with each accident bin.  相似文献

The purpose of this study is to compare the value of statistical life (VSL) estimates for traffic, drowning, and fire accidents. Using a choice experiment in a mail survey of 5,000 Swedish respondents we estimated the willingness to pay for risk reductions in the three accidents. In the experiment respondents were asked a series of questions, whether they would choose risk reducing investments where type of accident, cost of the investment, the risk reduction acquired, and the baseline risk varied between questions. The VSLs for fire and drowning accidents were found to be about 1/3 lower than that for traffic accidents. Although respondents worry more about traffic accidents, this alone cannot explain the difference in VSL estimates. The difference between fire and drowning accidents was not found to be statistically significant.  相似文献

Over the past decade, terrorism risk has become a prominent consideration in protecting the well‐being of individuals and organizations. More recently, there has been interest in not only quantifying terrorism risk, but also placing it in the context of an all‐hazards environment in which consideration is given to accidents and natural hazards, as well as intentional acts. This article discusses the development of a regional terrorism risk assessment model designed for this purpose. The approach taken is to model terrorism risk as a dependent variable, expressed in expected annual monetary terms, as a function of attributes of population concentration and critical infrastructure. This allows for an assessment of regional terrorism risk in and of itself, as well as in relation to man‐made accident and natural hazard risks, so that mitigation resources can be allocated in an effective manner. The adopted methodology incorporates elements of two terrorism risk modeling approaches (event‐based models and risk indicators), producing results that can be utilized at various jurisdictional levels. The validity, strengths, and limitations of the model are discussed in the context of a case study application within the United States.  相似文献

The recent occurrence of severe major accidents has brought to light flaws and limitations of hazard identification (HAZID) processes performed for safety reports, as in the accidents at Toulouse (France) and Buncefield (UK), where the accident scenarios that occurred were not captured by HAZID techniques. This study focuses on this type of atypical accident scenario deviating from normal expectations. The main purpose is to analyze the examples of atypical accidents mentioned and to attempt to identify them through the application of a well-known methodology such as the bow-tie analysis. To these aims, the concept of atypical event is accurately defined. Early warnings, causes, consequences, and occurrence mechanisms of the specific events are widely studied and general failures of risk assessment, management, and governance isolated. These activities contribute to outline a set of targeted recommendations, addressing transversal common deficiencies and also demonstrating how a better management of knowledge from the study of past events can support future risk assessment processes in the identification of atypical accident scenarios. Thus, a new methodology is not suggested; rather, a specific approach coordinating a more effective use of experience and available information is described, to suggest that lessons to be learned from past accidents can be effectively translated into actions of prevention.  相似文献

The frequency of occupational accidents in fishery is high in most fishery nations. Implementation of safety measures has, however, been limited. The purpose of the present cross-sectional study was to explore Swedish fishermen's attitudes towards occupational risks and accident control. Information was acquired through a questionnaire distributed to a convenience sample of 92 fishermen. Indices were built, containing variables relevant to perceived risk levels in connection with work, perceived manageability of risks, activity in safety work, technical knowledge and individual attitudes towards risks. The results did not support earlier findings of low risk awareness and risk acceptance among fishermen. Perceived manageability of risks but not perceived risk level, accident experience or risk acceptance, were significantly associated with activity in safety work. The results suggest that safety work might benefit from efforts to develop fishermen's understanding of how the development of hazardous situations can be prevented or managed. The results do not support strategies for accident control in fishery based solely on increasing risk awareness.  相似文献

Occupational risk rates per hour of exposure have been quantified for 63 occupational accident types for the Dutch working population. Data were obtained from the analysis of more than 9,000 accidents that occurred over a period of six years in the Netherlands and resulted in three types of reportable consequences under Dutch law: (a) fatal injury, (b) permanent injury, and (c) serious recoverable injury requiring at least one day of hospitalization. A Bayesian uncertainty assessment on the value of the risk rates has been performed. Annual risks for each of the 63 occupational accident types have been calculated, including the variability in the annual exposure of the working population to the corresponding hazards. The suitability of three risk measures—individual risk rates, individual annual risk, and number of accidents—is examined and discussed.  相似文献

文章旨在洞悉制度因素对企业风险行为影响的内在机理。通过我国国有和民营上市企业风险倾向实证对比, 结果发现:(1)金融危机爆发前, 在整体层面和行业层面上, 国有企业较民营企业都更倾向于冒险, 且二者间的差距不断缩减;(2)危机爆发后, 不同目标水平情况下, 国有和民营企业风险倾向间的对比较此前出现了一定变化;(3)在高于目标水平情况下, 国有和民营企业的风险倾向均与前景理论的预期相悖。这表明制度因素在很大程度上决定了我国企业风险行为, 同时, 企业自身改革、发展水平, 以及所处的经济大环境也会对其风险倾向产生一定的影响。因此, 我国企业风险管理机制构建应立足改革与发展, 努力实现政府、企业和市场多方联动, 内外兼修。  相似文献

This article deals with the question of how societal impacts of fatal accidents can be integrated into the management of natural or man‐made hazards. Today, many governmental agencies give additional weight to the number of potential fatalities in their risk assessments to reflect society's aversion to large accidents. Although mortality risk aversion has been proposed in numerous risk management guidelines, there has been no evidence that lay people want public decisionmakers to overweight infrequent accidents of large societal consequences against more frequent ones of smaller societal consequences. Furthermore, it is not known whether public decisionmakers actually do such overweighting when they decide upon the mitigation of natural or technical hazards. In this article, we report on two experimental tasks that required participants to evaluate negative prospects involving 1–100 potential fatalities. Our results show that neither lay people nor hazard experts exhibit risk‐averse behavior in decisions on mortality risks.  相似文献

The Fukushima Daiichi accident raises a fundamental question: Can science and technology prevent the inevitability of serious accidents, especially those with low probabilities and high consequences? This question reminds us of a longstanding challenge with the trans‐sciences, originally addressed by Alvin Weinberg well before the Three Mile Island and Chernobyl accidents. This article, revisiting Weinberg's issue, aims at gaining insights from the accident with a special emphasis on the sociotechnical or human behavioral aspects lying behind the accident's causes. In particular, an innovative method for managing the challenge is explored referring to behavioral science approaches to a decision‐making process on risk management; such as managing human behavioral risks with information asymmetry, seeking a rational consensus with communicative action, and pursuing procedural rationality through interactions with the outer environment. In short, this article describes the emerging need for Japan to transform its national safety management institutions so that these might be based on interactive communication with parties inside and outside Japan.  相似文献

This article presents results from the Commission of the European Communities (CEC) project 'Safety of Shipping in Coastal Waters' (SAFECO). The project was performed by ten European partners during the period 1995-1998. The principal aim of the SAFECO project was to determine the influences that could increase the safety of shipping in coastal waters by analyzing the underlying factors contributing to the marine accident risk level. The work reported here focuses on the Marine Accident Risk Calculation System (MARCS) that was further developed during the SAFECO project. This paper presents the methods used by MARCS, as well as data and results from a 'demonstration of concept' case study covering the North Sea area. The estimated accident frequencies (number of accidents per year) were compared with historical accident data, to demonstrate the validity of the modeling approach. Reasonable (within a factor of 5) to good (within a factor of 2) agreement between calculated accident frequencies and observed accident statistics was generally obtained. However, significant discrepancies were identified for some ship types and accident categories. The risk model has particular problems with estimating the accident frequency for drift grounding in general and powered grounding for ferries. It was concluded that these discrepancies are related to uncertainties in several areas, specifically in the risk model algorithms, the traffic data, the error and failure probability data, and the historical accident statistics.  相似文献

The purpose of this article is to present a quantitative analysis of the human failure contribution in the collision and/or grounding of oil tankers, considering the recommendation of the “Guidelines for Formal Safety Assessment” of the International Maritime Organization. Initially, the employed methodology is presented, emphasizing the use of the technique for human error prediction to reach the desired objective. Later, this methodology is applied to a ship operating on the Brazilian coast and, thereafter, the procedure to isolate the human actions with the greatest potential to reduce the risk of an accident is described. Finally, the management and organizational factors presented in the “International Safety Management Code” are associated with these selected actions. Therefore, an operator will be able to decide where to work in order to obtain an effective reduction in the probability of accidents. Even though this study does not present a new methodology, it can be considered as a reference in the human reliability analysis for the maritime industry, which, in spite of having some guides for risk analysis, has few studies related to human reliability effectively applied to the sector.  相似文献

This study presents probabilistic analysis of dam accidents worldwide in the period 1911–2016. The accidents are classified by the dam purpose and by the country cluster, where they occurred, distinguishing between the countries of the Organization for Economic Cooperation and Development (OECD) and nonmember countries (non-OECD without China). A Bayesian hierarchical approach is used to model distributions of frequency and severity for accidents. This approach treats accident data as a multilevel system with subsets sharing specific characteristics. To model accident probabilities for a particular dam characteristic, this approach samples data from the entire data set, borrowing the strength across data set and enabling to model distributions even for subsets with scarce data. The modelled frequencies and severities are combined in frequency-consequence curves, showing that accidents for all dam purposes are more frequent in non-OECD (without China) and their maximum consequences are larger than in OECD countries. Multipurpose dams also have higher frequencies and maximum consequences than single-purpose dams. In addition, the developed methodology explicitly models time dependence to identify trends in accident frequencies over the analyzed period. Downward trends are found for almost all dam purposes confirming that technological development and implementation of safety measures are likely to have a positive impact on dam safety. The results of the analysis provide insights for dam risk management and decision-making processes by identifying key risk factors related to country groups and dam purposes as well as changes over time.  相似文献

The purpose of this paper is to provide the main results of a study concerning the risk of chlorine transport by train in France. The specific problem of chlorine transport is presented in the framework of a general model for assessing the risk in the transport of dangerous materials. The probability of accidents followed with a chlorine release involving fatalities are put in perspective with other risks having potential health effects on the public. Two types of application of the model are envisaged in relation to the management of risk: the selection of protective measures through a cost-effectiveness approach and the use of the model for a better planning of decisions in an accident situation.  相似文献

Human factors are widely regarded to be highly contributing factors to maritime accident prevention system failures. The conventional methods for human factor assessment, especially quantitative techniques, such as fault trees and bow-ties, are static and cannot deal with models with uncertainty, which limits their application to human factors risk analysis. To alleviate these drawbacks, in the present study, a new human factor analysis framework called multidimensional analysis model of accident causes (MAMAC) is introduced. MAMAC combines the human factors analysis and classification system and business process management. In addition, intuitionistic fuzzy set theory and Bayesian Network are integrated into MAMAC to form a comprehensive dynamic human factors analysis model characterized by flexibility and uncertainty handling. The proposed model is tested on maritime accident scenarios from a sand carrier accident database in China to investigate the human factors involved, and the top 10 most highly contributing primary events associated with the human factors leading to sand carrier accidents are identified. According to the results of this study, direct human factors, classified as unsafe acts, are not a focus for maritime investigators and scholars. Meanwhile, unsafe preconditions and unsafe supervision are listed as the top two considerations for human factors analysis, especially for supervision failures of shipping companies and ship owners. Moreover, potential safety countermeasures for the most highly contributing human factors are proposed in this article. Finally, an application of the proposed model verifies its advantages in calculating the failure probability of accidents induced by human factors.  相似文献

Risk Homeostasis Theory and Traffic Accident Data   总被引:3,自引:0,他引:3  
The risk homeostasis theory posits, in essence, that a control mechanism analogous to the thermal homeostatic system in warm-blooded animals tends to keep risk per unit time constant, and, as a consequence, the number of traffic accidents per unit time of driving also tends to remain constant, essentially independent of changes in the traffic safety system. It is the purpose of the present research to examine the validity of this claim using a wide variety of traffic accident data. All the data examined are found to be incompatible with the risk homeostasis theory. The only specific field accident data offered in the literature to support the risk homeostasis theory are found to, in fact, refute the theory. The accident data provide evidence that a rich variety of user responses occur. While it is possible for users to collectively respond in such a way that safety benefits are completely cancelled, such a response is not particularly common; it is certainly not universally occurring, as suggested by the risk homeostasis theory. It is concluded that the risk homeostasis theory should be rejected because there is no convincing evidence supporting it and much evidence refuting it.  相似文献

The risk of catastrophic failures, for example in the aviation and aerospace industries, can be approached from different angles (e.g., statistics when they exist, or a detailed probabilistic analysis of the system). Each new accident carries information that has already been included in the experience base or constitutes new evidence that can be used to update a previous assessment of the risk. In this paper, we take a different approach and consider the risk and the updating from the investor's point of view. Based on the market response to past airplane accidents, we examine which ones have created a surprise response and which ones are considered part of the risk of the airline business as previously assessed. To do so, we quantify the magnitude and the timing of the observed market response to catastrophic accidents, and we compare it to an estimate of the response that would be expected based on the true actual cost of the accident including direct and indirect costs (full-cost information response). First, we develop a method based on stock market data to measure the actual market response to an accident and we construct an estimate of the full-cost information response to such an event. We then compare the two figures for the immediate and the long-term response of the market for the affected firm, as well as for the whole industry group to which the firm belongs. As an illustration, we analyze a sample of ten fatal accidents experienced by major US domestic airlines during the last seven years. In four cases, we observed an abnormal market response. In these instances, it seems that the shareholders may have updated their estimates of the probability of a future accident in the affected airlines or more generally of the firm's future business prospects. This market reaction is not always easy to explain much less to anticipate, a fact which management should bear in mind when planning a firm's response to such an event.  相似文献
