We use a unique dataset from Italy to investigate the impact of socioeconomic characteristics and social capital on family wellbeing and satisfaction. We assess wellbeing using four dimensions of satisfaction with family life: satisfaction with decision making processes, with relationships with partner and children, and with time spent with children. Social capital is measured through information about membership in organizations, trust, and interactions with others. We find that while socioeconomic characteristics in general do not have strong effects on family wellbeing, social capital matters for family life satisfaction.

BackgroundMidwife-led continuity of care models benefit women and the midwives who work in them. Australian graduate midwives are familiar with, and educated to provide, continuity of care to women although the opportunity to work exclusively in positions providing continuity of care on graduation is uncommon.AimTo explore the immediate and aspirational employment plans and workforce choices, reasons for staying in midwifery and perceptions around factors likely to influence job satisfaction of midwives about to graduate from one Australian university during the years 2012–2016.MethodsThis longitudinal study draws on survey responses from five cohorts of midwifery students in their final year of study.FindingsNinety five out of 137 midwifery students responded to the survey. Almost nine out of ten respondents either aspired to work in a continuity of care model or recognised that they would gain job satisfaction by providing continuity of care to women. Factors leading to job satisfaction identified included making a difference to the women for whom they care, working in models of care which enabled them to provide women with ‘the care I want to give’, and having the ability to make autonomous midwifery decisions.ConclusionAligning early graduate work experiences with continuity of care models may have a positive impact on the confidence and professional development of graduate midwives, which in turn may lead to greater satisfaction and retention among a workforce already committed to supporting the maternity healthcare reform agenda.  相似文献

BackgroundThere has been a rise in induction of labour over recent decades. There is some tension in the literature in relation to when induction is warranted and when not, with variability between guidelines and practice. Given these tensions, the importance of shared decision-making between clinicians and women is increasingly highlighted as paramount, but it remains unclear to what extent this occurs in routine care.MethodUsing a scoping review methodology, quantitative and qualitative evidence were considered to answer the research question “What are the views, preferences and experiences of women and clinicians in relation to induction of labour more broadly, and practices of decision-making specifically?” To identify studies, the databases PubMed, Maternity and Infant Care, CINAHL and EMBASE were searched from 2008 to 2018, and reference lists of included studies were examined.Findings20 papers met inclusion criteria, in relation to (a) women’s preferences, experiences and satisfaction with IOL; (b) women’s experience of shared-decision making in relation to induction; (c) interventions that improve shared decision-making and (d) factors that influence decision-making from the perspective of clinicians. Synthesis of the included studies indicates that decision-making in relation to induction of labour is largely informed by medical considerations. Women are not routinely engaged in the decision making process with expectations and preferences largely unmet.ConclusionThere is a need to develop strategies such as decision aids, the redesign of antenatal classes, and clinician communication training to improve the quality of information available to women and their capacity for informed decision-making.  相似文献

This paper is a comparative analysis of the gender gaps in the non-paid domestic and care work (NPDCW) undertaken in homes in Argentina, Chile, Spain and Uruguay. The explanatory factors of this gap in two-income households and their magnitude and impact on the distribution of NPDCW are analyzed using data from national time use surveys. The weakness of micro-sociological approaches and the variables related to relative resources and time availability is demonstrated using the estimation of a regression model, while the importance of approximations of gender roles and analyses that incorporate macro-sociological factors is shown. Furthermore, the findings show that NPDCW is done by women in 70% of cases with women’s incomes and time availability among the individual variables that drive change within the couple. The results show that the equalizing effects of time availability and gender ideology are stronger for women in more egalitarian countries; women in less egalitarian countries benefit less from their individual-level assets. Additional comparative analysis shows that other macro-level factors (economic development, female labor-force participation, gender norms and welfare systems) may also influence the division of this work. The results suggest that changes in individual-level factors alone may not be enough to achieve an equal division of labor in the household without a parallel reduction in macro-level gender inequality.

BackgroundMaternal satisfaction with maternity care is an important indicator of quality maternity services. Continuity of midwifery models of care are increasing in Australia and while several instruments have been developed to measure satisfaction with maternity care most of these have not been validated and there are none that are appropriate to continuity of midwifery maternity care models.AimTo develop a questionnaire to measure women’s satisfaction with maternity services provided in a continuity of midwifery care service model.MethodsA modified Delphi technique was used. A heterogenous panel of eight experts provided feedback over four rounds. The starting point for the questionnaire was informed by two systematic literature reviews focusing on available instruments for measuring maternal satisfaction with maternity care and what women value continuity of midwifery models of care.FindingsThe Continuity of Midwifery Care Satisfaction Survey (COMcareSS) was developed after four rounds of feedback with the expert panel. The survey comprises nine domains and fifty-nine questions. The domains include demographics, maternity care outcomes, facilities, the midwife/woman relationship, building capacity-empowerment, decision making and involvement, personalised care, advice care and support and general.ConclusionsConsumer satisfaction is an important indicator of quality care. This is the first instrument to be developed that is appropriate to continuity of midwifery models of care. The important next step is to pilot test the instrument to establish its validity and reliability.  相似文献

Background

Opportunities for women and providers to use decision aids and share decisions about birth after caesarean in practice are currently limited in Japan. This is despite known benefits of decision aids to support value-sensitive healthcare decisions.

Aim

To explore Japanese women’s decision making experiences using a decision aid program for birth choices after caesarean.

Methods

A mixed methods study was conducted among 33 consenting pregnant women with previous caesarean in five obstetrics institutions located in the western part of Japan. Outcome measures included change in level of decisional conflict, change in knowledge, and preference for birth method. Semi-structured interviews examined women’s decision making experiences, and qualitative data were analyzed using thematic analysis.

Findings

The participants in the program experienced a statistically significant improvement in knowledge and reduction in decisional conflict about birth after caesarean. Four themes were identified in the qualitative data related to decision making: change in women’s knowledge about birth choices, clarifying women’s birth preference, feelings about shared decision making, and contrasting feelings after receiving information.

Discussion

This study confirmed potential benefits of using the decision aid program. However, uncertainty about mode of birth continued for some women immediately prior to the birth. This finding emphasized the need to identify additional ways to support women emotionally throughout the process of decision making about birth after caesarean.

Conclusions

It was feasible to adapt the decision aid for use in clinical practice. Future research is necessary to examine its effectiveness when implemented in Japanese clinical settings.  相似文献

Active participation of the elderly is a recognized response to address the societal and individual challenges of rising life expectancy such as releasing the pressure of age-related public spending, reducing social isolation and improving well-being. How much time older people devote to active participation and whether their time allocation is associated with well-being remains under-investigated. Using time-use data from Belgium (n?=?1384) and the USA (n?=?2133), we investigate the time older people (65–80 years) spent on active participation and examine how this relates to their life satisfaction as an indicator of well-being. The countries vary in the amount of time spent on paid employment and volunteering, but not on informal help. Belgian older people spend much less time on paid employment than their American counterparts. This implies more are available to volunteer and provide informal help. Yet participation rates in these activities are higher in the USA. Multivariate analyses show that associations between active participation and life satisfaction vary between both countries and within both countries by gender and age. Overall, positive associations between paid work and volunteering and life satisfaction suggest that governments would do well to mobilize elderly into active participation, especially in Belgium. Negative associations between informal help and life satisfaction suggest governments should provide greater support for informal carers.

ABSTRACT

Inviting husbands to come to terms with women’s adaptation to the phenomenon of menopause is beneficial. The aim of this research was to uncover the effect of couples education on marital satisfaction. This study was performed on 80 menopausal women accompanied by their husbands. The intervention group received four sessions of couples education. The results showed that the mean increase in the rate of marital satisfaction after couples education in the intervention group was significantly higher than the control. Short-term couples education has a significant positive effect on marital satisfaction. Therefore, this counseling strategy can be integrated into health-care programs.  相似文献

The stratification system in India has resulted in the socioeconomic inequality in society and defines women domestic workers as one of the lowest segments of society. This qualitative and quantitative study aims at describing the problems of female domestic workers, the relationship of their employers with them, and exploring the impact of socioeconomic status mainly, occupation, education, and income on life satisfaction. We used ethnographic observation and in-depth interview over a 4 year period in Mysore (a city in the south of India). Furthermore, our quantitative research was guided by convenience sampling technique with selecting 125 (65 domestic workers, 60 employers). We prepared a structured questionnaire for gathering demographic information, i.e. age, education, marital status, occupation, and religion. To measure the degree of life satisfaction, we administered Satisfaction with Life Scale (Diener et al. J Person Assess 49(1):71–75, 1985). The data were collected during September and October 2011. The hypotheses were tested by applying Pearson correlation, regression, and t test (SPSS, version 18). The results of our observation and interview indicate that domestic workers suffer from various problems inside and outside their homes. The relationship between domestic workers and employers is a master–servant relationship. Domestic workers remain as an unorganized job group though in 2004 the Karnataka Minimum Wage Act was passed. Our quantitative findings reveal that there is a positive correlation between socioeconomic status and life satisfaction. This type of study displays that enacting law is not sufficient; it necessitates implementing and monitoring properly. This study suggests measures for supporting female domestic workers.  相似文献

The changes which have taken place in 15 countries of the Western world during the first half of this century with regard to the proportion of economically active to total females, have been studied ; some countries record a rise, others a fall. Further analysis shows that the participation of women in non-domestic work for pay has risen with only a few exceptions. The proportion of married women who are at work has also risen substantially in most of the English-speaking countries, as well as in Sweden.

Changes in population structure cannot explain these developments. The influence of economic factors is not simple to assess, but the main explanation for the rise in women's work participation appears to be full employment and the development of new industries, increasing the demand for labour, and at the same time helping to raise women's earnings relatively to those of men and thus encouraging women to take up paid work.

t is anticipated that female work participation will increase in the underdeveloped countries, where it is generally at a low level, and will remain high in the advanced countries, involving the continued employment of a substantial minority of married women. This calls for a reconsideration of social policy, since a single-earner grant to families where the woman's full-time attendance at home is required for the care of small children, might become more important than child endowment.  相似文献

BackgroundImmunisation is universally accepted as one of the most significant health initiatives in recent times. However, vaccine hesitancy is increasing in Australia and other high- and middle-income countries. There is evidence to suggest that many parents, even those who elect to immunise, may have a degree of vaccine hesitancy. The recommendation of a healthcare professional is a predictor for vaccine uptake.AimThe purpose of this study was to explore the values, beliefs and choices made by vaccine hesitant parents and pregnant women, regarding their decision not to vaccinate their child or children. The aim being to determine the factors that influence this decision making and to give a voice to vaccine hesitant parents.MethodsA qualitative exploratory online survey of 106 vaccine hesitant parents and pregnant women was conducted in 2021. The survey utilised closed and open-ended questions.FindingsPregnant women and parents obtained most of their immunisation education from nurses, midwives, and general practitioners. Vaccine decision-making was however, influenced by multiple factors including vaccine safety concerns, the sources of information accessed, and a previous negative immunisation experience. Other influential factors included the use of alternative therapies, diet, and lifestyle factors.DiscussionAlong with general practitioners, nurses and midwives are a popular, respected and a vital source in the provision of accurate and timely immunisation education. However, further education is required at an undergraduate level to adequately prepare them for their role of listening to and educating vaccine hesitant pregnant women and parents.  相似文献

Background

Decision-making in midwifery, including a claim for shared decision-making between midwives and women, is of major significance for the health of mother and child. Midwives have little information about how to share decision-making responsibilities with women, especially when complications arise during birth.

Aim

To increase understanding of decision-making in complex home-like birth settings by exploring midwives’ and women’s perspectives and to develop a dynamic model integrating participatory processes for making shared decisions.

Methods

The study, based on grounded theory methodology, analysed 20 interviews of midwives and 20 women who had experienced complications in home-like births.

Findings

The central phenomenon that arose from the data was “defining/redefining decision as a joint commitment to healthy childbirth”. The sub-indicators that make up this phenomenon were safety, responsibility, mutual and personal commitments. These sub-indicators were also identified to influence temporal conditions of decision-making and to apply different strategies for shared decision-making. Women adopted strategies such as delegating a decision, making the midwife’s decision her own, challenging a decision or taking a decision driven by the dynamics of childbirth. Midwives employed strategies such as remaining indecisive, approving a woman’s decision, making an informed decision or taking the necessary decision.

Discussion and conclusion

To respond to recommendations for shared responsibility for care, midwives need to strengthen their shared decision-making skills. The visual model of decision-making in childbirth derived from the data provides a framework for transferring clinical reasoning into practice.  相似文献

ABSTRACT

Based on data obtained from in-depth, qualitative interviews conducted with 30 older Canadian women, this article examines older women’s experiences with, and views of, physicians. The research participants report a very high level of satisfaction with the patient–physician relationship; however, they also report physician shortcomings. Findings indicate that participants’ satisfaction with the patient–physician relationship was influenced by “stories about other doctors,” rationalizations and justifications, and a high level of patient assertiveness.  相似文献

The fertility rate in Hong Kong has been very low for decades. Because work–family conflict is one of the major barriers for married couples in actualizing their fertility ideals, domestic outsourcing that relieves women from the burden of domestic labor may help reduce the gap between ideal and actual fertility. Hiring live-in domestic helpers, who co-reside with the hiring families and work on a full-time basis, is gaining popularity in Hong Kong. However, past studies neither inside nor outside of East Asia have examined how employing live-in helpers affects fertility. This study investigates the relationship between live-in helpers and fertility by analyzing retrospective event-history data we collected from a representative survey of married couples in Hong Kong (n?=?1697). Our results show that married couples employing live-in helpers tend to have more children than couples not employing live-in helpers. Specifically, the practice is associated with higher odds of first childbirth and of second childbirth, with no evidence of a positive effect beyond bearing a second child. The findings have implications for other East Asian societies, which share similar backgrounds of ultra-low fertility rates, rising female labor force participation rates, rigid gender inequalities in domestic labor, and demanding work cultures.

The main aim of this paper is to evaluate the life satisfaction among women living in the Northern Cyprus by focusing on socio-economic and gender related factors. Within this context, we aim to analyze the influence of socio-personal characteristics of Northern Cypriot women in regard to life satisfaction. To fulfill these objectives, a questionnaire survey has been applied to 510 women living in the major cities of Northern Cyprus. Applying an ordered logit model, this paper examines the role of several factors such as normal and social reference income, employment, and savings, health, and education, participation in the household expenditures, being a full-time housewife, domestic violence, political preferences, and perception of government performance on life satisfaction. The results demonstrate that good health, high education, high income, saving, high household income has positive effects on life satisfaction whereas domestic violence, being a full-time housewife, unemployment and social reference income has negative effects on life satisfaction.  相似文献

Previous research has shown little difference in the average leisure time of men and women. This finding is a challenge to the second shift argument, which suggests that increases in female labor market hours have not been compensated by equal decreases in household labor. This paper presents time-use and leisure satisfaction data for a variety of western European countries, and shows that accounting for socio-economic factors that differ between men and women is vital for understanding gender differences. We find that working mothers have leisure levels that are much lower than those of working fathers and singles. Working mothers are also most likely to report the least satisfaction with free time. Finding that time stress and leisure time are positively correlated within socio-demographic groups suggests that the second shift argument is still valid, and that feelings of time stress are indeed associated with the lack of leisure time.  相似文献

This paper explores various dynamics in the relationship between life satisfaction and education in South Africa using the 2008 National Income Dynamics Survey. The results indicate a strong positive association between educational attainment and individual satisfaction with life, which is true in the overall sample and for men and women. This positive relationship also holds for Black and Coloured individuals, but is insignificant in the Asian and White samples. Evidence indicates that education is a positional good, in that people who have attained more than the mean level of education in their relevant cluster are significantly more satisfied with life compared to those possessing less than the mean education.

ABSTRACT

This article highlights a gendered and forced mobility which has been under-recognised in the literature on mobility. It explores the hidden relocations of women (often with children) due to intimate partner abuse; presenting findings from mixed methods research on women’s journeys to escape domestic violence, including analysis of over 80 journey segments made by 20 women within England and Wales, and from abroad. Focusing on means of transport, the research found that under a third of journey segments were made by public transport, and these tended to be longer distances; that journeys by disabled women were more likely to be by private transport, and that journeys from rural areas were more likely to be with the assistance of others. Thematic analysis of interviews at different stages of women’s journeys is used to explore their experiences of different means of transport in terms of degrees of control and agency, in terms of losing or retaining personal possessions on the move; and in highlighting the role of others’ assistance in compounding or counteracting the implications of abuse. Women’s domestic violence journeys are thereby contextualised within wider mobilities research, uncovering the inequalities and implications of this hidden internal displacement in the UK.  相似文献

This study explores the effect of providing regular help with personal care to a resident or non-resident parent or parent-in-law on different aspects of psychological well-being. We use cross-sectional data from the Norwegian Life Course, Ageing and Generation (LOGG) study (N ~ 15,000, age 18–79) and two-wave panel data from the Norwegian study on Life course, Ageing and Generation (NorLAG) (N ~ 3,000, age 40–79). We separate outcomes into cognitive well-being (life satisfaction, partnership satisfaction, self-esteem), affective well-being (happiness, positive and negative affect, depression, loneliness) and sense of mastery. Caregiver status is largely unrelated to these aspects of well-being, both in cross-section and longitudinally. One notable exception is that caring for a resident (but not a non-resident) parent relates to lower affective well-being among women, also longitudinally. This effect is more marked among unpartnered and lower educated women. In addition, caring for a non-resident parent is associated with a positive change in sense of mastery among women. The results reviewed and presented indicate that caregiving has less detrimental effects in the Nordic countries than in other countries, highlighting the role of social policies and care systems in shaping the impact of caregiving on well-being.  相似文献
