The prospects for higher domestic savings rates in Latin America

This paper analyzes the prospects for raising domestic saving rates to permit increased investment and/or reduced dependence on foreign-capital inflow in Latin America. We analyze the question of rising saving rates by applying a random-coefficients approach: treating the parameters estimated in time-series analysis for individual countries as observations drawn from an international cross-section of savings behavior. Correlation analysis is then applied to clarify the conditions associated with international differences in savings parameters across a sample of 21 Latin American countries.     

A Successful Debrief Program for House Staff

Challenging end-of-life encounters can be stressful and may lead to burnout. Monthly debriefing sessions are led by a physician and clinical social worker (LCSW). Sessions focus on experiences and emotional reactions rather than case details. Themes identified included; feeling unsupported after difficult deaths, identifying with patients emotionally, and conceptualizing a “good” versus “bad” death. Debriefing provides opportunities for residency physicians to reflect on the affective side of their work and helps promote resiliency.     

Productivity drift in extended learning curves

The pattern of changes in the Extended Learning Curves representing the performance of 114 assembly workers in an electronics firm is examined in terms of changes in productivity drift over long periods of time. It is shown that 2 types of discontinuities can occur. The first consists of a temporary levelling off in productivity followed by a recovery to new higher levels of productivity; the second has a sustained period of negative productivity drift. A number of work facets are statistically tested as possible causes of these patterns of productivity drift, but, the only significant relationship (at P ≤ 0.05 level) is with length of service in job. Some evidence is presented to show that conventional work measurement methods are not effective in terms of assessing the true work output of experienced workers since such workers are able to beat the system by claiming those allowances which are most difficult to cross-check.     

Media and agenda setting: effects on the public, interest group leaders, policy makers, and policy

Using an experimental design built around a single media event, the authors explored the impact of the media upon the general public, policy makers, interest group leaders, and public policy. The results suggested that the media influenced views about issue importance among the general public and government policy makers. The study suggests, however, that it was not this change in public opinion which led to subsequent policy changes. Instead, policy change resulted from collaboration between journalists and government staff members.     

HIV rapid testing in substance abuse treatment: implementation following a clinical trial

The Substance Abuse Mental Health Services Administration has promoted HIV testing and counseling as an evidence-based practice. Nevertheless, adoption of HIV testing in substance abuse treatment programs has been slow. This article describes the experience of a substance abuse treatment agency where, following participation in a clinical trial, the agency implemented an HIV testing and counseling program. During the trial, a post-trial pilot, and early implementation the agency identified challenges and developed strategies to overcome barriers to adoption of the intervention. Their experience may be instructive for other treatment providers seeking to implement an HIV testing program. Lessons learned encompassed the observed acceptability of testing and counseling to clients, the importance of a "champion" and staff buy-in, the necessity of multiple levels of community and agency support and collaboration, the ability to streamline staff training, the need for a clear chain of command, the need to develop program specific strategies, and the requirement for sufficient funding. An examination of costs indicated that some staff time may not be adequately reimbursed by funding sources for activities such as adapting the intervention, start-up training, ongoing supervision and quality assurance, and overhead costs.
