A new public health frontier challenging maternity care is addressing the sub-optimal breastfeeding rates of women who are obese. Despite the World Health Organisation's recommendation that breastfeeding is initiated within the first hour of birth and continued exclusively for six months, less than half of infants and young children globally are optimally breastfed. While initiation rates of exclusive breastfeeding immediately after birth are as high as 90 percent in Australia, this rate dramatically declines in the first few weeks postpartum, with only approximately 15 percent of infants exclusively breastfed to five months of age (less than 6 months). Analyzing nationally representative survey data collected in the United States in 2014 (n?=?1,932), we formulate and test a novel explanation for the educational gradient in depressive symptoms. We theorize that status as cultural capital drives this gradient in addition to well-established economic and social factors, via the feelings of cultural entitlement it inspires. Therefore, we use structural equation modeling to decompose the education effect. We demonstrate that in addition to economic (job security and income) and social factors (embeddedness in the neighborhood), cultural capital indeed accounts for the educational gradient in depressive symptoms via cultural entitlement. We conclude that for understanding social gradients in mental health it is vital to be sensitive for the cultural mechanisms that status as cultural capital can inspire. Research on the illusory truth effect has found that repeated presentation of uncertain statements increases validity judgments of those statements. Three experiments explored the shape of the repetition–validity–judgment relationship over multiple repetitions, the mediating role of processing fluency, and the moderating role of dispositional skepticism. Participants read narratives in which different rumors were repeated 0–6 or 0–9 times; validity estimates, processing fluency, and dispositional skepticism were also measured. Validity judgments were logarithmically related to repetitions; this effect was mediated by processing fluency, and moderated slightly by skepticism. Results explore the boundaries of the processing fluency contrast account of the illusory truth effect, suggest a minor role for skepticism, and inform research on belief in rumor (uncertain statements in circulation). 相似文献

Few women patients in this study were found to be active partners in the medical management of systemic lupus erythematosus (SLE). Despite the growing emphasis on doctors and patients sharing knowledge and decision-making power, most of the women were struggling to have their symptoms and needs taken seriously, and appeared relatively powerless in relation to the medical profession. Acknowledging patient expertise requires that doctors share some of their power, and we suggest this may be especially problematic when medical resources and power are primarily located in the hands of white men, and when the patients are typically women and often from the black community. Implications for action that are considered include maximising the influence of women with SLE on research, training and practice. 相似文献

Summary Despite a debate lasting some decades, there has been littleor no clear resolution about the way knowledge may be relatedto practice. To a considerable degree this is because of anassumption that knowledge is knowledge as product:given knowledge, researched and applied to practice. In recentyears, the concept of knowledge as process hasemerged, focusing on knowledge of rigorous ways to think aboutpractice situations. This has generated a focus on hypothesisgeneration, testing and falsification, as well as lessons tobe learned from rigorous methods of qualitative analysis insocial research. Rigorous social workers, it has been suggested,would behave like practical qualitative researchers ofthe individual instant. However, there is little empiricalresearch on the matter. This paper evaluates process knowledgeforms used by social workers. It presents findings that socialworkers vary considerably in the extent to which they develophypotheses about the whole case, or aspects of it, in the depthwith which they analyse cases, and the systematic way they pursuean information gathering strategy. The paper concludes thatthere is clear evidence of social workers behaving as practicalqualitative researchers, but the rigour with which they do sovaries, with fundamental implications for the training process,and the extent to which rigorous strategies, along the linesof research methodology, should be taught. 相似文献

This article explores how the police and municipal authorities of Le Havre responded to the colonial others who passed through, or resided in, the Seine-Inférieure port between the outbreak of the First World War and the defeat of France in 1940. Interrogating how the police and urban authorities monitored migrants to the port, it reveals how Le Havre’s imperial and transnational space was distinctive in terms of the peoples who established themselves in the port, the ways in which they forged links with other peripheral locations throughout the French empire, and how the local authorities attempted to control migrants and incomers from the French overseas empire. It highlights particularities of Le Havre’s urban space – notably its lack of a university and prestigious lycées, its pre-1914 history of militant strike action, its role as France’s main transatlantic port, and the presence of a small colonial population with a narrow social-economic profile – and shows how these particularities resulted in the enactment and sometimes neglect of national policies and agendas according to specific local priorities. 相似文献

This paper examines market issues in the provision of children's services in the light of the changing role and practice of local authorities contracting for welfare services. In adult services, where there has been a legal requirement to reorganize on market lines, the services have had to modify some of their earlier contracting practices to take account of the complex requirements of health and welfare services and it is argued that relational contracting offers a more appropriate paradigm for these. Following the implementation of the Children Act 1989, although not a requirement of the Act, many of the organizational systems in relation to markets, originally developed for adult services, have been adopted for children's services. Two areas of services for children, day care and fostering services, are discussed in order to demonstrate that these contracting systems are inappropriate and often dysfunctional for the children concerned. It is argued that there is a widening gap between contracting systems in adult and children's services which needs to be addressed. In general, it is concluded that the philosophies of the market place are flawed when applied to children's services. 相似文献
