Direct care staff struggle with the decision of when to physically intervene with patients. There is a widely held belief that they are expected to place themselves in danger of harm to prevent patients from hurting themselves or others. In this acute mental health care setting, an educational program was developed, using principles of adult and transformational learning, to dispel the idea that getting hurt is part of the job. The Staying Safe program strongly discourages staff from physically intervening alone and promotes staff getting enough help and having a plan. Staff are encouraged to interact with patients in helpful ways and to respond to patient requests in ways that do not increase frustration or evoke anger. There has been a positive response to the training program, evidenced by changes in the way staff think about their jobs and a reduction in the number of assaults and injuries to staff.  相似文献

This article considers the impact of new managerial reform on the recruitment and retention of women into senior management posts across the Irish education sector. In Ireland as elsewhere, the rhetoric of gender equality permeates new managerial reforms. Yet our data suggest that an emphasis on performativity and an intense commitment to paid work consolidates masculinist management cultures disguised through the ideology of choice. This works to the detriment of women, especially those with caring responsibilities. Drawing on studies of 23 top‐level educational appointments in primary, secondary and higher education, we show how the relentless crafting of an elastic self is required for both those who remain in management positions in education and those who seek them.  相似文献

There is considerable debate regarding the extent to which punitive parenting adversely impacts youth well‐being. Using an ecological–transactional model of human development, we examined reciprocity and contextual variability in associations between maternal punitive discipline and adolescent adjustment among 1,147 low‐income, urban youth followed through adolescence. Longitudinal structural equation modeling results indicated that delinquency and depressive symptoms during pre‐ and early adolescence (Time 1) were associated with increased punitive discipline about a year later (Time 2). When mothers reported less Time 2 neighborhood disorder, punitive discipline at Time 2 was associated with increased delinquency (for boys) and depressive symptoms (for girls) during mid‐ to late adolescence (Time 3). The costs of punitive discipline for adolescent adjustment are best understood considering the dynamic, transactional, and contextual nature of development.  相似文献

Despite the importance of context in studies of language use, sociolinguists have ignored the impact of noise on conversational interaction. This inattention is of particular concern in classrooms where language is a learning tool. Our research on interaction in noisy settings took place in English language elementary school classrooms with students in grades 3, 5, and 7, whose first language was English. Students were observed during regular classroom activities. Employing a novel method, in which students wore ear‐level microphones, we obtained stereophonic recordings of the noise and conversation that reached each listener's ears. A dosimeter measured the noise levels in each classroom. Analyses of students’ patterns of conversation suggest that noise levels impeded the intended development of complex conversational interaction and collaborative learning. This study also questions the place of acoustics in understanding context, and the significance of the hearer's perspective in sociolinguistic studies of conversational interactions.  相似文献

Following perinatal loss, a type of ambiguous loss, bereaved couples struggle with and experience distress due to various forms of ambiguity. Moreover, the juxtaposition of their grief with society's minimization often disenfranchises them from traditional grieving processes. The purpose of this study was to explore sources of ambiguity and disenfranchised grief related to perinatal loss. Audio-taped interviews with 13 bereaved couples at 2, 6, and 13 months following the death of their fetus or infant were analyzed. Several categories of ambiguity and disenfranchised grief emerged, pertaining to: (a) the viability of the pregnancy; (b) the physical process of pregnancy loss; (c) making arrangements for the remains; and (d) sharing the news. This study uncovers the many sources of ambiguity and disenfranchised grief that bereaved couples face in interactions with family, friends, society, and healthcare professionals. These insights may inform healthcare professionals in their attempts to ease distress related to perinatal loss.  相似文献

This paper evaluates the association between welfare policies implemented by states in the early to mid-1990s and the rate at which female household heads with children exit AFDC for work or for non-work reasons. The results show that waiver policies requiring work, such as the work requirement and the elimination of exemptions for mothers of very young children, are associated with more rapid exits through employment. Incentive policies that make work more attractive by allowing mothers to remain eligible as earnings rise extend the period of welfare receipt. Neither time limits, the family cap, nor sanctions were related to welfare exits during this early period of welfare reform. Results suggest that other policies implemented in the 1993-1996 period, such as the EITC, may have also influenced welfare exits. In addition, the economy played an important supportive role in facilitating recipient exits from public assistance. While relatively high unemployment rates did not deter recipients from working, state prosperity increased the positive impact of work requirements on work exits.  相似文献

Traditional approaches to themeasurement of leisure's relationship toquality of life have emphasised place-centredindicators (e.g., the frequency of leisurefacility usage) and tended to ignoreperson-centred criteria (e.g., satisfactionwith leisure experiences). Moreover, theunderlying assumption in subsequent policyoutcomes has been that increasing the number offacilities and services will automaticallyenhance people's QOL. This paper focuses onboth the content and measurement of leisure andits relationship to quality of life. Itreports the results of a study that examinedthe relative importance of selected place andperson-centred leisure attributes in predictingquality of life. The study tested a set ofobjective and subjective indicators that peopleare most concerned with in their leisure lives. Overall, it was found that the person-centredleisure attribute, leisure satisfaction, wasthe best predictor of quality of life. Place-centred attributes failed to influencequality of life. Further analysis revealedthat people who engage in social activitiesmore frequently and who are more satisfied withthe psychological benefits they derive fromleisure, experience higher levels of perceivedquality of life. The results suggest thatthere is a need to reconcile objectiveknowledge with subjective perceptions ofleisure in order to achieve greaterunderstanding and comprehensive measurement ofthis complex domain and its relationship toquality of life.  相似文献

Inpatient nurses work to engineer an environment in which patients and staff are physically and psychologically safe. A recent research study on an adult psychiatric unit examined the dimensions involved in keeping the unit safe. The four dimensions included ideology, people (staff and patients), space, and time. In this article, we discuss how these dimensions apply to real-world efforts to keep child and adolescent inpatient units safe. We also discuss the challenges nurses face within each of these safety dimensions, including those arising from fluctuating acuity, staffing formulas, surveillance of physical space, and entrenched unit cultures.  相似文献
