Domestic violence is everywhere and nowhere. No statutory organization or health service has work with either perpetrators or survivors of domestic violence (usually women and children) as the primary focus of their service, yet all agencies will have very significant numbers among their clients/service users. It is therefore crucial that the policy framework is developed both within and between agencies to address the need, and scope, of intervention in this area and particularly the impact on children. Currently, significant steps have been taken by some agencies in the UK to address this previously neglected issue, though the developments are patchy. This paper draws on a UK‐wide research study which mapped the extent and range of service provision for families where there is domestic violence and also developed a framework of good practice indicators for provision in this area. This article examines one of the indicators of good practice arising from the research—that of policy development—within social service departments and within the multi‐agency arena. Two assumptions which characterize the measurements of quality life (QOL) studies, are questioned by the present report: First, the assumption that QOL is the same thing for all subjects, is usually related to health problems and can easily be measured across subjects. Second, when this assumption is given up by introducing subjective base-rates in an intra-subject (before-after) design, researchers still tend to assume that no response shifts occur on the QOL scales, e.g., that these scales maintain the same meaning and values for subjects over time and interventions. In our study we found that QOL had different subjective interpretations [1--4]. While for some certain issues in their family-life determined their quality of life, for others these were issues at their workplace or of their health conditions. Second, when QOL was tested against a subjective base-line (for each individual according to their choices of domains), about eleven percent of the hypertensives and normotensives showed a clear response shift of scale-calibration over the period of one year. When the scores of these subjects were excluded, the significance of certain previously reported results changed. For example, the significant difference between normotensives and hypertensives concerning the change in their subjective evaluation of QOL over the year and their initial depression became more significant, while similar changes in their evaluation of sexual impairment and control at their work-place became insignificant. These results suggest that response shifts have to be traced and quantified, before one can claim any results (or lack of results) in 'before-after' designs, concerning subjective meaningful issues like quality of life. 相似文献

To be unemployed is often associated with being stigmatised by others. People may blame the unemployed themselves, insist that they could find a job if they tried harder, maintain that too little is demanded from recipients of unemployment benefits and consider differences in standard of living between the unemployed and the employed too small. This article is aimed at studying the prevalence of such negative attitudes and to examine the determinants behind them. In May 2000, questions were added on to a randomly selected subsample in the regular Swedish labour force survey. The results indicate that own unemployment experience and perceived risks of becoming unemployed make people less apt to blame the unemployed. Having family members or close friends with unemployment experience tends to have a similar impact. In contrast, young people more often express stigmatising attitudes. Background
Perinatal audits in the Netherlands showed that stillbirth was nearly always preceded by a period of reduced fetal movements. Patient or caregiver delay was identified as a substandard care factor.Aim
To determine whether the use of a new information brochure for pregnant women on fetal movements results in less patient delay in contacting their maternity caregiver.Methods
A pre- and post-survey cohort study in the Netherlands, including 140 women in maternity care with a singleton pregnancy, expecting their first child. All participating women filled out a baseline questionnaire, Cambridge Worry Scale and pre-test questionnaire at the gestational age of 22–24 weeks. Subsequently, the intervention group received a newly developed information brochure on fetal movements. At a gestational age of 28 weeks, all women received the post-test questionnaire. Multiple regression analyses were used.Findings
Per-protocol analysis showed less patient delay in the intervention group compared to the control group (Odds Ratio 0.43; 95% Confidence Interval 0.17–0.86, p = 0.02). A significant linear relation was observed between reading the information brochure and an increase of knowledge about fetal movements (B = 1.2, 95% Confidence Interval 1.0–1.4, p < 0.001). Maternal concerns did not affect patient delay to report reduced fetal movements.Conclusion
Use of an information brochure regarding fetal movements has the potential to reduce patient delay and increase knowledge about reduced fetal movements. A national survey to determine the effect of an information brochure about reduced fetal movements on patient delay and stillbirth rates is needed. 相似文献100.

Interpersonal sensitivity, defined as the accurate recall of another person’s nonverbal behavior, was measured in two studies. In Study 1, nonverbal recall accuracy (NRA) was based on recall of cues expressed by a man and/or woman being interviewed on videotape. Retest reliability after 2 weeks was satisfactory and the male and female tests demonstrated sufficient convergence. Participants could assess their own recall accuracy at better than chance levels; ability to decode the meanings of visual nonverbal cues was positively related to NRA; discriminant validity of NRA was demonstrated with regard to general cognitive ability; and women had higher NRA than men. In Study 2, NRA was based on recall of a live interaction partner. Women again had higher NRA than men; NRA was significantly better than chance; and higher NRA was associated with more smiling and self-reported positive affect during the interaction. Nonverbal recall accuracy is a promising new definition of interpersonal sensitivity.
