Background

Low breastfeeding duration rates reflect the pain and distress experienced by many women who discontinue breastfeeding in the early weeks and months of life. This paper explores modern key historical events that have significantly influenced Australian breastfeeding education and practice.

Method

Relevant literature reviewed from 1970 to 2010 identified key events that appear to have contributed to the decrease in Australian breastfeeding rates and the increase in women experiencing breastfeeding complications, particularly nipple pain and trauma.

Findings and discussion

The rise in institutionalisation and medical intervention in labour and birth has also medicalised midwifery practice. Technocratic intrusion and institutionalised care is contributing to the separation of the mother and newborn at birth. Delayed mother–baby initiation of breastfeeding and interruption of the duration of the first, and subsequent breastfeeds, negatively affects the innate ability of the mother and newborn to establish and sustain breastfeeding. The ‘pathologising’ of breastfeeding that involve midwives teaching women complicated and unnatural breastfeeding techniques interfere with instinctive sensory and mammalian behaviours and further contributes to the high complication rates.

Conclusion

Midwives are encouraged to reflect on their role as ‘experts’ in the breastfeeding process and give confidence to women so that they utilise their instinctive ability to breastfeed by self-determined techniques that encourage mammalian skills for newborn sustenance and survival.  相似文献

Drawing on the UK research project, ‘Farmers’ understandings of GM crops within local communities’, this paper considers the application of the concepts of communities of practice and networks of practice in the agricultural context. A brief review of theories about communities of practice and networks of practice is given and some of our findings are discussed in the context of those theories.Farmers were found to be a particular type of network of practice, characterised by a weak organisational framework but with a relatively stable network of other communities of practice (or networks of practice) they interact with, which we have called a ‘web of influencers on practice’. Together, farmers’ network of practice and their web of influencers on practice represent the whole environment in which learning may occur, and so provide insights into their social learning system. Most farmers have to work at the boundary of their network of practice and their web of influencers, which creates a significant load on their knowledge management. This is in contrast to other networks of practice where only some members take on this boundary brokering role. The paper concludes that these theories (on networks and communities of practice) provide a useful lens through which to view farmers and their practice, highlighting important points for policy. However, in such contexts these theories need to be extended to include the role of a broader ‘web of influencers on practice’.  相似文献

The article reviews empirical studies on trends in attitudes toward lesbians and gay men and compares data from a 1985 survey of 297 students living in a college residence hall with data collected in 2001 when 152 students living in the same residence hall responded to a similar questionnaire. College students in 2001 expressed more accepting attitudes toward lesbians, reported more contact with lesbians, and assessed their parents' attitudes as more positive when compared with the 1985 sample. Consistent trends in the importance of these variables to the development of attitudes toward lesbians were shown, with gender role attitudes, parental attitudes, and exposure to education and media about gays and lesbians remaining the most important predictors. This suggests that strategies that discourage gender role stereotypes and provide educational and media experiences could increase acceptance of sexual minorities. Although gender was significant in 1985, there were no effects of gender in the 2001 sample. Continued study of trends in attitudes toward all sexual minorities and attention to the relative importance of gender and gender roles are recommended.  相似文献

In the multilingual centres of Northern Europe's major cities, new varieties of the host languages are emerging. While some analyse these ‘multiethnolects’ as youth styles, we take a variationist approach to an emerging ‘Multicultural London English’ (MLE), asking: (1) what features characterise MLE; (2) at what age(s) are they acquired; (3) is MLE vernacularised; and (4) when did MLE emerge, and what factors enabled this? We argue that innovations in the diphthongs and the quotative system are generated from the specific sociolinguistics of inner‐city London, where at least half the population is undergoing group second‐language acquisition and where high linguistic diversity leads to a heterogeneous feature pool to select from. We look for incrementation ( Labov 2001 ) in the acquisition of the features, but find this only for two ‘global’ changes, BE LIKE and goose ‐fronting, for which adolescents show the highest usage. Community‐internal factors explain the age‐related variation in the remaining features.  相似文献

Abstract

Psychosocial safety climate (PSC) refers to a specific organizational climate for the psychological health of workers. It is largely determined by management and at low levels is proposed as a latent pathogen for psychosocial risk factors and psychological strain. Using an extended Job Demands-Control-Support framework, we predicted the (24 month) cross-level effects of PSC on psychological strain via work conditions. We used a novel design whereby data from two unrelated samples of nurses working in remote areas were used across time (N=202, Time 1; N=163, Time 2), matched at the work unit level (N= 48). Using hierarchical linear modelling we found that unit PSC assessed by nurses predicted work conditions (workload, control, supervisor support) and psychological strain in different nurses in the same work unit 24 months later. There was evidence that the between-group relationship between unit PSC and psychological strain was mediated via Time 2 work conditions (workload, job control) as well as Time 1 emotional demands. The results support a multilevel work stress model with PSC as a plausible primary cause, or “cause of the causes”, of work-related strain. The study adds to the literature that identifies organizational contextual factors as origins of the work stress process.  相似文献
