Evaluations of health programs are typically conducted from a rational bureaucratic framework in which client change and client outcome are seen as logical outcomes of program activity. This paper proposes a theoretical framework for examining organizational consequences of program implementation. Factors in the internal environment (e.g., the hospital) and the external environment (e.g., the community) which contribute to the program's success or failure, are considered. Observational and structured interview data are used to compare two state funded perinatal projects, one an organizational "success" and the other a costly failure. Attributes of external environments which may be important to successful implementation include demand for services, provider competition, access to information, social beliefs, and professional commitment. Important features of the internal environment include host organization commitment and conflict, and structural attributes: complexity, formalization, centralization, and coordination. Financial, social and ecological cost and benefit criteria were used to examine the sustainability of land-use options on an upland Scottish estate. The costs and benefits of alternative land uses were examined using a psychometric modelling technique based on decision-conferencing, using an expert group of people. Three decision models based respectively on financial, social and ecological criteria were developed, compared and then integrated. The management options that emerged from the integrated model as best compromises appeared to be reasonable and feasible within the framework of facts and projections available to the group. The effects of different future scenarios, such as changes in fiscal policy, were also examined using the models. Summary When illness is socially defined, the task of therapy is to enable the patient to move from a sick role to one of maximal social functioning. In a day hospital setting, methods are explored to involve a patient with his family both directly and indirectly. Thus the patient is supported both in the treatment milieu and when he is at home. Not only the patient, but the family, is vulnerable at this time and each must be helped to make parallel and complementary shifts in interpersonal behavior. SUMMARY. This paper reports results from a study of playgroups in three areas of England. It describes the extent and nature of parent involvement in the study playgroups, and how far this involvement included fathers as well as mothers; examines the relationship between involvement and various socio-economic characteristics of the parents, to see whether certain groups were more or less likely to be involved in their children's playgroups; and considers attitudes to parent involvement among both mothers and playgroup leaders, and playgroup leaders' experience of involvement, in particular whether they experienced any difficulties in getting or maintaining involvement. 相似文献

This study examined how attachment quality, measured during the preschool period using the Waters & Deane Q-Sor. (1985). affects preschoolers' collaborative problem-solving interactions. We compared collaborative styles of 19 securely- and 18 insecurely-attached three and four year olds during a grocery planning task with mother and then a strange female adult who was also the mother of a preschool child. Results indicated that securely-attached preschoolers were more likely to show task-relevant and metacognitive behavior in both contexts and performed better with the stranger than did their insecure peers. The collaborative style of mothers of secure children was more in synchrony with secure children's level of participation in the task than with that of insecure children who were less focused on goal-directed task activities. Strangers were generally less structuring than mothers particularly with insecure children. Interestingly, the security status of the stranger's own child did not influence her collaborative style with an unfamiliar preschooler. A theoretical understanding of the gendered contours of structural, everyday and symbolic violence suggests that young addicted women are particularly vulnerable to the infectious diseases caused by injection drug use-especially hepatitis C. Participant-observation fieldwork among heroin and speed addicts in San Francisco's Haight Ashbury neighborhood reveals that extreme levels of violence against women are normalized in the common sense of street-youth drug culture. Physical, sexual and emotional violence, as well as the pragmatics of income generation, including drug and resource sharing in the moral economy of street addicts, oblige most young homeless women to enter into relationships with older men. These relationships are usually abusive and economically parasitical to the women. Sexual objectification and a patriarchal romantic discourse of love and moral worth leads to the misrecognition of gender power inequities by both the men and women who are embroiled in them, as well as by many of the public services and research projects designed to help or control substance abusers. Despite deep epistemological, theoretical and logistical gulfs between quantitative and qualitative methods, applied public health research and the interventions they inform can benefit from the insights provided by a theoretical and cross-methodological focus on how social power contexts shape the spread of infectious disease and promote disproportional levels of social suffering in vulnerable populations. 相似文献

This review gathers together and synthesises research relating to young people's experiences of social support during their transition from state care. A systematic approach was used to identify relevant studies published since 2001 and forty-seven were found which met the inclusion criteria. Relevant key themes were identified and consolidated under five overall thematic headings: The influence of past experiences on social support in the present, Supportive relationships during the transition from care, Relationships with birth families, The crucial role of practical support and The lived experience of leaving care. This paper attempts to discuss the Chinese cultural implications of the recently proposed document: ‘Global Qualifying Standards for Social Work Education’ by the IASSW and IFSW. It seems that this document still embraces an ideology of western social work imperialism in perceiving the development of social work in other countries.
