Transition from Home Care to Nursing Home: Unmet Needs in a Home- and Community-Based Program for Older Adults

A major effort is under way nationally to shift long-term care services from institutional to home- and community-based settings. This article employs quantitative and qualitative methods to identify unmet needs of consumers who transition from a statewide home- and community-based service program for older adults to long-term nursing home residence. Administrative data, care manager notes, and focus group discussions identified program service gaps that inadequately accommodated acute health problems, mental health issues, and stressed family caregivers; additional unmet needs highlighted an inadequate workforce, transportation barriers, and limited supportive housing options. National and state-level policy implications are considered.     

A new look at entropy and the life table

The mathematical derivations described in this paper offer a new look at the entropy of the life table, denoted by H. Contrary to previous claims, it is theoretically possible, and has been observed empirically, for life tables to have entropy values greater than unity. A re-expression of H as a weighted average of life expectancy at different ages relative to life expectancy at birth demonstrates clearly the conditions under which reductions in mortality by a fixed amount at all ages can result in even greater gains in life expectancy.     

To Encounter a Child: Maternal Subjectivity and the Aesthetics of Reading

This article offers an outline and preliminary analyses of key aspects of Lisa Baraitser's (2009 Baraitser , L. ( 2009 ). Maternal Encounters: The Ethics of Interruption . London, UK, and New York , NY : Routledge . [Google Scholar]) Maternal Encounters as a situating opening piece in a special feature arising from an intensive seminar on Baraitser's work held in Dublin in September 2010. The first section opens out some of the psychoanalytic implications of Baraitser's theory of maternal subjectivity. The second section considers the importance of temporality and the interruption of the “seamless” thinking subject for the emergence of a maternal subject and considers Baraitser's maternal subject in relation to some key queer and feminist positionings of maternity. The final section assesses the text's contribution to philosophical debates. First, it discusses how Baraitser attends to the everyday in developing an aesthetics of ordinariness. Second, it looks at deconstruction and inhabitation. Finally, it considers cultural phenomenology, objects, and the magic Baraitser locates in things.     

“Here I Am!”—Irreducible Invocation of the Other

New York , NY, and Toronto, Ontario , Canada : Random House . [Google Scholar]) continues to inaugurate new horizons for critical reflection on the psychoanalytic theorizing of human relationships in terms of unconscious-conscious dynamics with its inevitable insertion in different sociocultural constructions of power practices. I follow Benjamin philosophically through post-Hegelian critical social/political theorists, particularly Habermas, and psychoanalytically through her respect for Winnicott's analyses of early developmental struggles for individuality in the face of fear of loss and destructiveness. Philosophical/Psychoanalytic inspiration carries her quest. This article excavates questions weaving throughout the book: Is an ideal of recognizing ourselves and others as “free” individual subjects possible? Could a psychoanalytical dialectics help us to live more responsibly with each other's differences not withstanding ineradicable past trauma? Is there possibility of life together, even in our gender differences, beyond the dialectical spell of erotic domination/submission?     

A Theory of Thinking: A Theory of Desiring

ABSTRACT

This article focuses on aspects of what is broadly known as countertransference in centering on the psychoanalyst Wilfred Bion’s explicit theory of thinking as constituting an implicit theory of desiring. I make a distinction between the discursive terms “thoughts,” “thinking,” “desires,” “desiring,” “wants,” and “craving” and tease out their relationship to one another for clinical practice. I explore how thinking and desiring operate in and around frustration and suggest that the thinking apparatus is not just a mechanism for dealing with thoughts but also is a way of transforming wants into desires. I separate out desires and the desiring apparatus, as Bion does with thoughts and thinking, and argue that thinking is inextricably linked to desiring and thoughts to desires. The interpretative act, moreover, constitutes the analyst’s acting on desire, whereas acts of projective identification are bound up with craving. I demonstrate the potential uses of this theory by discussing it with reference to two published clinical examples.     

Developing Assessment Procedures and Assessing Two Models of Escalation Behavior among Community College Administrators

Escalation behavior occurs when individual decision-makers repeatedly invest time, money, and other resources into a failing project. A conceptual model of escalation behavior based on project, organizational, social and psychological forces was developed, and a 75-item measurement instrument was constructed to assess the various dimensions. The model was tested using data collected from a random sample of North Carolina Community College administrators. A LISREL measurement model analysis provided support for the four escalation forces. Two structural models were tested, leading to support for a mediational model for escalation behavior. The most important contributor to Escalation was the Psychological force.
