This article reviews scientific and other literature during the 1990s that links migration and mobility with the spread of sexually transmitted diseases (STDs), including HIV/AIDS. The focus is on key population groups linked to the spread of HIV and STDs in West and Central Africa: migrant laborers, truck drivers, itinerant traders, commercial sex workers (CSWs), and refugees. Countries with high emigration and immigration tend to have high levels of HIV infection, with the exception of Senegal. The main destination of immigrants are Senegal, Nigeria, and Cote d'Ivoire in West Africa and Cameroon, Congo, Gabon, and Congo in Central Africa. The risk of infection and the spread of HIV is variable among migrants. There is little in the literature that substantiates hypotheses about the strong association between migration and HIV-positive status. Information is needed on the duration, frequency of return visits, living conditions, sexual activities with multiple partners, and information before departure, along the routes, at final destination, and at the time of returns. Action-based research in five West African countries (Burkina Faso, Cote d'Ivoire, Mali, Niger, and Senegal) should produce results in late 1998. Comparable studies in Central Africa are unknown. Regional studies should be complemented by local studies. Prevention would benefit from studies on the relative size of these five population groups by geographic location. 相似文献

Immigration policies and their management in a country like Canada have long been an interesting and instructive study for other countries. 1) With borders naturally protected by great distance from almost all migrant routes; 2) with a long, undefended border with the US and a further 3000 kilometers to its border on the south; 3) with a parliamentary system capable of comparatively rapid legislative and administrative responses to problems; and 4) with a relatively small legal, and even smaller illegal, population Canada had historically "experimented" with novel, often quite creative, immigration policies and programs to both encourage and control the increases in its population. This paper summarizes what Canada did and is doing in response to am important item of public policy--the entry and presence of illegal migrants. Canada has experimented with 1) discretionary amnesty for long-term illegals with a capacity to be successfully integrated into Canadian life, 2) tighter border controls with the extended use of the visitor's visa, and 3) employer sanctions. Prevention continues to gain importance among the strategies open for governmental development of social policy. This paper offers two exploratory conceptual discussions related to prevention in social policy. The first is a representation of the policy making environment with special reference to the informational requirements for the development of policy. The second is a discussion of social pathologies, broadly defined as patterns of behavior with important negative repercussions on individuals whom we refer to as victims, and of possible strategies toward the development of (at least partial) preventive measures. The two discussions are interrelated as the shape of the second is dictated by the desiderata outlined in the first. The paper is divided into five parts:
- Introduction: the conundrum of prevention.
- The Development of a paradigm of the policy context.
- Dependent variables: social pathologies.
- Independent variables: a focus on structural determinants.
- Conclusion: information requirements for preventive action.

This paper is an attempt to raise some general objections to the whole concept of making decisions on teleological principles. Its scope is finally the whole concept of how we rationally decide, on a teleological basis, to do any act at all. However the arguments developed here arise from the recent discussions about one particular species of teleological decision-making - utilitarianism. Therefore, while I hope that the paper has consequences that are important far beyond this one ethical theory, my objections are phrased in terms of direct arguments about utilitarianism. The recent lively debates on this topic make it easy to ask some interesting questions about this sort of goal-oriented theory. However, if my objections are correct, the scope of these questions is much wider than any one utilitarian theory (or cluster of theories) because my objections are about an assumption that is common to all teleological theories and not just to classical utilitarianism. 相似文献

This paper uses data from the Fragile Families and Child Wellbeing Study to test the hypotheses that (1) similar to other
positive pre- and post-natal outcomes, Mexican immigrant mothers are more likely to breastfeed, and to breastfeed longer,
than white or Mexican-American mothers; and (2) acculturation accounts for the ethnic/nativity differential in breastfeeding
initiation and duration. The results support both hypotheses. Mexican immigrants to the U.S. are much more likely than whites
to breastfeed, and to breastfeed longer. Mexican-American mothers, after controlling for background characteristics, have
similar initiation and duration to whites. Using expanded acculturation measures developed for this paper, acculturation accounts
for some of the difference between whites and Mexican immigrants in breastfeeding initiation, and much of the difference for
breastfeeding duration. The results suggest that low levels of acculturation operate to protect Mexican immigrants from choosing
to formula-feed, which gives their babies many health advantages, and may be associated with better health outcomes across
the life course. The results also suggest that successive generations of Mexican immigrants may abandon breastfeeding, which

Federal housing policy in the US across the postwar period supported the construction of new houses more than public provision
or renovation of older structures as a means of ensuring a sufficient supply of quality dwellings. Understanding trends in
new housing in particular is thus crucial to understanding the housing regime. Following Myers (Housing demography: Linking demographic structure and housing markets. University of Wisconsin Press, 1990; Housing Studies, 14, 473–490,1999), this paper conceptualizes historical change in the housing stock within a demographic framework as the movement
of cohorts of households through cohorts of housing stock. Recent evidence suggests that a new cohort of houses arose in the
1980s and 1990s (larger with more amenities than past vintages), and that buyers of those new houses were increasingly affluent.
In this paper, I link the succession to a new cohort of houses to household cohort succession and examine the increasing affluence
of new house buyers by age and cohort, focusing especially on the entry of the Baby Boom generation exactly when the new cohort
of houses arrived. I use US Census microdata for 1960–2000 to develop a cohort longitudinal dataset, and analyze historical
change in stratification in new house ownership. I find significant shifts between cohorts in income inequality among new
house buyers, with implications for the capacity of the housing regime to meet the future needs of an increasingly diverse
population.

There is a need to investigate novel interventions that promote worksite physical activity and wellness. OBJECTIVE: The authors' purpose in this study was to evaluate the effectiveness of a 12-week walking program supplemented with a pedometer, computer educational program, and weekly e-mails. METHODS: College faculty and staff participated in a one-group pre-posttest study to determine whether the 12-week walking intervention had an effect on body mass index (BMI), blood pressure, blood glucose, and cholesterol. The authors also determined participant-perceived wellness effects. RESULTS: The authors observed differences between baseline and follow-up in BMI (p = .024), blood glucose (p = .06), and total cholesterol (p = .09). The program had a moderate effect on fitness, mood, health awareness, nutrition, and health. CONCLUSIONS: It is incumbent that experts develop innovative worksite physical activity and wellness programs. A pedometer-monitored walking program is one way that a worksite health initiative can improve the health and wellness of its employees and simultaneously reduce health-care costs. 相似文献
