Where strength training has been used in conjunction with functional-task training in older people, not only have there been improvements in leg strength but also improved function has been measured (e.g., Skelton & McLaughlin, 1996). Many studies use participants from care homes rather than community dwellers. We investigated changes in leg power, balance, and functional mobility in community-dwelling sedentary men and women over 70 years of age (n = 6 for training group [TR]; n = 10 for control group [CN]). Progressive training took place over 24 weeks using seated and nonseated exercise. For TR, leg power increased 40%, from 108 +/- 40 to 141 +/- 53 W (p < .01); dynamic balance increased 48%, from 22.3 +/- 7.9 to 33.1 +/- 6.1 cm (p < .01; functional reach); and functional mobility increased 12%, from 7.46 +/- 1.32 to 6.54 +/- 1.41 s (p < .05; timed walk). CN showed no significant change. In conclusion, a community-based exercise program led to large improvements in leg-extensor power, dynamic balance, and functional mobility. 相似文献

Characteristics of effective providers of marital and family therapy in rural mental health settings
Characteristics of effective providers of marital and family therapy (MFT) in rural mental health settings were investigated. The survey sample included 74 members of the National Association for Rural Mental Health who, through a modified Delphi process, created a composite picture of effective MFT providers. The findings yielded six major rank-ordered characteristics of effective MFT providers in rural mental health settings, with effective skills in MFT ranked first. Rural community understanding, appreciation and participation ranked second. Specific competencies contributing to each major characteristic were also identified and are discussed. This paper adopts a global perspective to review the current situation with respect to the systematic study of civil society. It locates the analysis in a geohistorical framework, arguing that, as a distinct area of scholarship, contemporary civil society research is a two-track post-Cold War phenomenon exhibiting major shortcomings. A new type of dedicated research financing and process of allocation could redress today's research limitations in ways that will beneficially cross-fertilize parallel approaches and epistemological traditions while increasing the spread of centers dedicated to such knowledge generation. Semi‐structured one‐on‐one interviews with 102 gay and bisexual men were conducted to examine the reasoning processes men use to exempt themselves from practicing safe sex. Qualitative analysis of the interviews revealed the following recurring themes: (a) Many men who were in couple relationships avoided condom use for reasons involving intimacy or trust, or because both partners were HIV‐negative; (b) unsafe sex sometimes occurred inadvertently or involuntarily; (c) negative moods and self‐images were associated with unsafe sex; (d) by “intuiting” that their partner was HIV‐negative, participants exempted themselves from the need for safe sex; and (e) when the boundary between safe and unsafe was unclear, participants used a combination of unofficial and official guidelines to determine what is safe. 相似文献

The joint models for longitudinal data and time-to-event data have recently received numerous attention in clinical and epidemiologic studies. Our interest is in modeling the relationship between event time outcomes and internal time-dependent covariates. In practice, the longitudinal responses often show non linear and fluctuated curves. Therefore, the main aim of this paper is to use penalized splines with a truncated polynomial basis to parameterize the non linear longitudinal process. Then, the linear mixed-effects model is applied to subject-specific curves and to control the smoothing. The association between the dropout process and longitudinal outcomes is modeled through a proportional hazard model. Two types of baseline risk functions are considered, namely a Gompertz distribution and a piecewise constant model. The resulting models are referred to as penalized spline joint models; an extension of the standard joint models. The expectation conditional maximization (ECM) algorithm is applied to estimate the parameters in the proposed models. To validate the proposed algorithm, extensive simulation studies were implemented followed by a case study. In summary, the penalized spline joint models provide a new approach for joint models that have improved the existing standard joint models. 相似文献

Although social constructionists have taken the process by which people learn to present social problems for granted, these skills are acquired in the course of socialization. Through high school debate, adolescents acquire techniques for making arguments, using evidence, and presenting claims to multiple audiences. In the process, they learn that taking a public position does not necessarily require a deeply held commitment. This model resonates with the institutional structure of politics and law in the United States in which adversaries battle each other in a rule-governed "game,'a contest ultimately evaluated by judges assumed to be impartial. To understand the process by which adolescents gain the skills to construct social problems, I conducted an ethnographic study of debate teams at two high schools. The ability to take both sides of an argument, express ideas with which one does not personally agree, and present powerful, if questionable, evidence, constrained by time, teaches teenagers how to engage in social problem discourse and provides a training ground for moral entrepreneurs. 相似文献
