Looming threats to the intimate bond in hospice care? Economic and organizational pressures in the case study of a hospice

Hospice organizations are assailed by stiff competition, ever-rising costs, limited funding, and policy changes. Do such pressures stifle the high quality of care these organizations strive to provide? As a case-in-point, we draw from the mid-1990s accounts of caregivers at a nonprofit hospice in a Midwestern city in the United States. We maintain that economic pressures drive organizational restructuring, which then weakens working conditions and, thereby, weakens the staff-client relationship. We discuss effects upon worker behaviors, the worker-client relationship, and client care. This ethnographic case study signals the need to closely examine the threats that current economic and organizational pressures in the United States may pose to the quality of hospice care.     

Identifying race and ethnicity in the 1979 National Longitudinal Survey of Youth

The 1979 National Longitudinal Survey of Youth is among the few surveys to provide multiple reports on respondents’ race and ethnicity. Respondents were initially classified as Hispanic, black, or “other” on the basis of data collected during 1978 screener interviews. Respondents subsequently self-reported their “origin or descent” in 1979, and their race and Hispanic origin in 2002; the latter questions conform to the federal standards adopted in 1997 and used in the 2000 census. We use these data to (a) assess the size and nature of the multiracial population, (b) measure the degree of consistency among these alternative race-related variables, and (c) devise a number of alternative race/ethnicity taxonomies and determine which does the best job of explaining variation in log-wages. A key finding is that the explanatory power of race and ethnicity variables improves considerably when we cross-classify respondents by race and Hispanic origin. Little information is lost when multiracial respondents are assigned to one of their reported race categories because they make up only 1.3% of the sample.

Subclinical Psychopathic Traits and Romantic Attachment in Treatment-Seeking Couples

Subclinical psychopathic personality traits have been associated with multiple indicators of couple distress and romantic attachment insecurities but essentially in community samples. This study examined dyadic associations between primary and secondary psychopathy and attachment anxiety and avoidance, assessed with self-reported measures, in 183 French-Canadian cohabiting partners seeking couple therapy. Two Actor-Partner Interdependence Models were tested: Model 1 considered psychopathic traits as independent variables and Model 2 treated attachment insecurities as independent variables. Analyses revealed that in both partners, secondary psychopathy was associated with attachment anxiety and avoidance (Models 1 and 2), while primary psychopathy was not only associated with attachment anxiety (Model 2; actor associations). Partner effects between primary and secondary psychopathy in men and women's attachment anxiety were also found in both models. These findings complement those reported in community samples and have clinical implications for treatment-seeking couples.     

Virtual journey coupled with face‐to‐face exchange: enhancing the cultural sensitivity and competence of graduate students

This study attempted to enhance cultural sensitivity for graduate students at an American and a South African university, using a six‐week online List Serv, email buddy exchange, and two‐week face‐to‐face experience. After the course was over, results of the intercultural developmental inventory, using t‐tests for related samples, showed that there was a significant difference (alpha equals .04) in the universalism and cognitive frame shifting clusters as well as the overall developmental cultural level of sensitivity. Positive trends were noted in the areas of cultural self‐identity enhancement, and a decrease in inflation in the area of perceived intercultural sensitivity was noted.

The author also discusses analytic serials cataloging, MARC fields, and field types with a list ranked by importance, classification information, and a sample procedure.     

Attachment Insecurities and Women's Sexual Function and Satisfaction: The Mediating Roles of Sexual Self-Esteem,Sexual Anxiety,and Sexual Assertiveness

We examined the potential role of three mediators—sexual self-esteem, sexual anxiety, and sexual assertiveness—of the association between romantic attachment insecurities (anxiety and avoidance) and two aspects of women's sexual functioning: sexual function and sexual satisfaction. A sample of 556 women aged 18 to 30 agreed to complete an online series of validated questionnaires assessing attachment insecurities and several aspects of sexual functioning. Lower sexual self-esteem and higher sexual anxiety mediated the associations between attachment anxiety and lower sexual function and satisfaction. Lower sexual self-esteem and higher sexual anxiety also partially mediated the links between attachment-related avoidance and the two sexual functioning variables. Sexual assertiveness, however, did not mediate these associations. A significant interaction between attachment anxiety and avoidance was also found to predict sexual satisfaction, with women high in avoidance and low in anxiety being the least satisfied. Results are discussed in terms of theoretical and clinical implications.
