Reducing alcohol misuse is a priority for U.S. health officials considering that misuse of alcohol is a leading preventable cause of morbidity and mortality. Consequently, health centers are integrating Screening, Brief Intervention, and Referral to Treatment (SBIRT) for alcohol misuse within usual care. Although SBIRT is well validated among general patient samples, results have not generalized to drinkers with probable alcohol use disorder; moreover, little is known about the efficacy of SBIRT with patients who are of low-income or ethnic or racial minority status. Members of these groups are of particular concern because they are at risk to experience trauma, potentially in concert with alcohol misuse. Therefore, translational approaches to delivering SBIRT particularly with these groups of interest might be needed to meet the Grand Challenge of reducing alcohol misuse. Accordingly, this study combined SBIRT with a model designed to address psychological trauma: T-SBIRT. With a sample of 112 adults, most of whom were African American or Latino/a, authors analyzed multiple indicators of feasibility. Results indicated that T-SBIRT is suitable for and acceptable to patients accessing community-based health services, and T-SBIRT can promote high referral acceptance rates to specialty treatment, particularly among patients with probable alcohol use disorder.  相似文献

This study explores the association between neighbourhood opportunities for Positive Youth Development (PYD) and adolescent depressive symptoms, well-being and suicide risk. A 2-stage random sample of 8500 students(years 9–13) from 91 high schools from throughout New Zealand was collected in 2012. Analyses were restricted to neighbourhoods with more than 10 students resulting in a final sample of 5191 adolescents within 266 neighbourhoods. Multilevel models linked data from neighbourhoods to individual student data to explore the association between neighbourhood opportunities for PYD and student depressive symptoms, well-being and suicide risk. Neighbourhoods with a high proportion of students involved in activities that help others, attend church groups, or participate in sports teams were associated with significantly increased well-being among students living in these neighbourhoods. No neighbourhood-level measures were found to significantly alter rates of depressive symptoms or suicide risk. Findings suggest that providing neighbourhood opportunities for PYD may enhance adolescent well-being.  相似文献

Communicating and engaging with children is a foundational component of child care social work practice, but all too frequently, in the wake of serious incidents, it is the focus of criticism. Drawing on findings from a large‐scale ESRC‐funded research project conducted in the four U.K. nations, this paper explores, through a psychosocial analytic lens, how social workers anticipate, enact and reflect on their encounters with both children and their families. Close analysis of what social workers said about their practice alongside what they were observed to do in practice revealed perceptions, patterns and processes of communication that, first, minimize emotions and the complexity of the professional task and second, overly privilege verbal interaction. Drawing on Sennett's (2012) ideas this paper offers a reconceptualisation of this professional task, from a communicative to a co‐operative one. It affords and creates a space in which social workers can develop more attuned communicative practices that include rituals, gestures and the minimal use of force. The theoretical insights and evidence‐informed practice recommendations arising from this research have conceptual significance for the social work discipline and practical significance for the child care social work profession, across national and international contexts.  相似文献

The aim of this study was to explore how a 1‐year peer‐mentoring relationship contributed to change in young women “in care.” Twenty semistructured, one‐to‐one interviews were conducted with mentors (n = 11) and mentees (n = 9) recruited from two different London local authorities. Participants’ accounts were interpreted through a developmental lens to uncover developmental aspects and locus mechanisms through which transformative change took place. Resilience as a healthy outcome was the result of the dual function the mentoring relationship performed. The mentoring relationship was protective against the risks associated with transitioning to independent living and/or adulthood, and promoted internal assets and competencies whereby the mentees’ ability to resist them was enhanced. Establishing a trustworthy connection with a role model promoted developmental domains within mentees.  相似文献

The article presents the findings of an international literature review conducted to examine the factors that drive inter‐country adoption rates within both sending and receiving countries. The authors then consider the implications of these findings for inter‐country adoption policy reform in Australia. The evidence in the literature highlights a distinction between the factors that drive ICA in sending and receiving countries. Factors that drive the practice in sending countries relate to structural forms such as socio‐economic and political conditions. In contrast, it is the growing demand for a child from infertile couples that drives the practice in receiving countries. The article then considers the challenges of domestic policy reform undertaken to increase numbers of inter‐country adoptions in a context of global decline.  相似文献

Using high-quality data from Norwegian population registers, we examine the relationship between family disruption and children’s educational outcomes. We distinguish between disruptions caused by parental divorce and paternal death and, using a simultaneous equation model, pay particular attention to selection bias in the effect of divorce. We also allow for the possibility that disruption may have different effects at different stages of a child’s educational career. Our results suggest that selection on time-invariant maternal characteristics is important and works to overstate the effects of divorce on a child’s chances of continuing in education. Nevertheless, the experience of marital breakdown during childhood is associated with lower levels of education, and the effect weakens with the child’s age at disruption. The effects of divorce are most pronounced for the transitions during or just beyond the high school level. In models that do not allow for selection, children who experienced a father’s death appear less disadvantaged than children whose parents divorced. After we control for selection, however, differences in the educational qualifications of children from divorced and bereaved families narrow substantially and, at mean ages of divorce, are almost non-existent.  相似文献

Summary.  We consider the application of Markov chain Monte Carlo (MCMC) estimation methods to random-effects models and in particular the family of discrete time survival models. Survival models can be used in many situations in the medical and social sciences and we illustrate their use through two examples that differ in terms of both substantive area and data structure. A multilevel discrete time survival analysis involves expanding the data set so that the model can be cast as a standard multilevel binary response model. For such models it has been shown that MCMC methods have advantages in terms of reducing estimate bias. However, the data expansion results in very large data sets for which MCMC estimation is often slow and can produce chains that exhibit poor mixing. Any way of improving the mixing will result in both speeding up the methods and more confidence in the estimates that are produced. The MCMC methodological literature is full of alternative algorithms designed to improve mixing of chains and we describe three reparameterization techniques that are easy to implement in available software. We consider two examples of multilevel survival analysis: incidence of mastitis in dairy cattle and contraceptive use dynamics in Indonesia. For each application we show where the reparameterization techniques can be used and assess their performance.  相似文献

This study considers the source, nature and direction of ‘crossover’ of occupational stressors and strains in a sample of 74 dual-career couples. It examines patterns and habits of discussion about work between partners and investigates the role of partner communication and job commitment in the crossover process. Contrary to previous research findings which suggest that the direction of crossover is predominantly from men to their female partners, positive relationships were found between women's work stressors and the anxiety and depression reported by their male partners. Only modest evidence of crossover from men to women was found. Work demands were linked to the crossover process for both men and women but, unlike the findings of previous studies, supportive features of the working environment failed to predict crossover between partners. The nature and frequency of marital communication about work was associated with crossover, as was job commitment and satisfaction. The implications of these findings for the psychological health and functioning of dual-career couples are discussed, and recommendations for future research that might further elucidate the crossover process are made.  相似文献

This study examines the development of impoverished pre-school children before and after the implementation of mandatory welfare-to-work initiatives in Canada. Using data from the 1994/95 and 1998/99 cycles of the National Longitudinal Survey of Children and Youth, we explore the relationships that impoverished families’ income source and family income status have with pre-school children's school readiness. Findings indicate that both before and after the implementation of mandatory welfare-to-work initiatives, children in working poor families had higher school readiness scores than their peers whose families receive social assisntance, independent of family environment characteristics that differentiated working poor and social assistance poor families. In addition, both before and after the implementation of mandatory welfare-to-work initiatives, school readiness scores of poor children were lower than scores for non-poor children, with children living in families that had incomes at least 200 percent of the low-income cut-offs being the only group with scores above the expected standard of 100. In sum, our study fails to provide evidence that by the end of the 1990s welfare reforms supported the concurrent policy goal of improving the well-being of Canadian children in poverty.  相似文献
