This article explores how rebalancing efforts can support the needs of individuals aging with a lifelong disability. The National Balancing Indicator project examined the overall long-term supports and services system (LTSS) progress in five indicators within the Sustainability, Coordination and Transparency, and Prevention principles toward a balanced LTSS system for those aging with a lifelong disability. In assessing state efforts to create a balanced participant-directed LTSS system with the National Balancing Indicators, the findings suggest states are better equipping the system to handle a burgeoning population of individuals aging with a lifelong disability, but more progress is still needed. Overall, states need to continue to create a seamless system that allows individuals with lifelong disabilities to transition smoothly through the life course. 相似文献

Americans are living longer, but dying after a prolonged period of management of multiple chronic illnesses and functional disabilities. Despite waves of public and professional activity targeted toward improving care for the dying and supporting the families, gaps in care and challenges in end-of-life care persist. Contentious issues such as the so-called “death panels” or physician payment for discussion of advance directives and care wishes at the end of life; aid in dying; and regarding individuals who actively choose death (case of Brittney Maynard) are continually debated in the public media. Progress toward improvement in the experience of dying remains incremental and change has been slow. With the release of a second Institute of Medicine (2014) report devoted to what it means to die in America in the 21st century, momentum and opportunity for change may increase. If this is to happen, social workers will need to deliver the range of biopsychosocial care that patients and families so desperately need. However, holistic care of the individual will only improve, if the nation also addresses ongoing systemic problems in financing, policy, and service delivery in end-of-life care. 相似文献

It has provided outstanding leadership in the field of career development. ABSTRACT Family members in both separated or divorcing and married families completed a range of questionnaires about the conflict in 3 relationships—couple, parent–child, and sibling—across 2 phases of the study approximately 12 months apart. In addition, the adolescents completed measures of adjustment: self esteem, depression, and anxiety. Adolescents in a subset of the divorcing families were interviewed about the conflict in their families across the 2 phases (107 at Phase 1 and 35 at Phase 2). Analyses showed that conflict was higher in the separated or divorcing families across all 3 relationships, and that the highest levels of conflict occurred for the high-conflict divorcing families. Qualitative data from interviews illustrated the nature of the conflict that was occurring. 相似文献

All health care sectors are currently examining factors that influence delivery of high-quality services for diverse groups with an understanding that minority populations experience barriers to service access that contribute to well-documented ill health and health inequities. With a goal of understanding dynamics that can improve access to care in the home care sector, this qualitative exploratory study examined processes to create inclusive, positive space for diverse lesbians, gay men, bisexuals, transgender, and queer (LGBTQ) people in community-based health and social service agencies. A purposeful sample of eight key informants from agencies in Toronto, Ontario, Canada, which offer programs and services to LGBTQ communities, completed in-depth interviews. Conventional content analysis and a critical lens were used. Themes offered insight into the histories, challenges, and turning points which shaped the development of LGBTQ-positive spaces in these organizations. Community engagement and leadership emerged as relevant, as did strategies to embed LGBTQ voices and visibility within everyday organizational functioning. Given the gap in literature addressing LGBTQ access to home care and the unique dynamics that shape care in the home, implications address the application of these study findings for creating positive space in the home care sector. 相似文献

This research contributes to our understanding of two central and related problems in the study of social movements: tactical innovation and strategic leadership. Focusing on the leadership history (1984–2003) of the founder and director of the first US public school program for gay and lesbian youth, called Project 10 and located in the Los Angeles, California public school system, this case study illustrates the importance of leadership agency on the part of those ‘organizing from within’. Analyses herein indicate the significance of both institutional constraints and life course circumstances in determining leadership choices. This paper maps organizational obstacles and the tactical dilemmas they produced to explain how successful strategic choices get made. The case of Project 10 indicates that institutional constraints can be overcome tactically with organizational elaboration. Additionally, hybridity, assumed in the literature to produce organizational precariousness, is shown here to be a mechanism for stability, facilitating networks and resource acquisition over time. 相似文献
