Research concerning people with learning difficulties who have left institutions has typically investigated how well people 'adapt' to life in the community, and has often ignored users' own perceptions of the changes in their quality of life resulting from the move. In this study, eight people with learning difficulties who moved from a hospital and seven people who moved from parental homes to live in staffed homes in the community were interviewed. Choice, privacy, social life and relationships with their parents and staff emerged as important factors in participants' perception of their quality of life. Their relationships with their parents were found to be close, and in some cases their parents were overprotective. Staff were perceived in some cases as too controlling and in others as providing too little support. The findings indicate that people with learning difficulties moving to community residences have aspirations which encompass far more than a wish to adapt to life in the community, and their relationships with family and staff are a central factor in achieving such aspirations.  相似文献

Employees of a large suburban health care center were asked to identify their needs and preferences for child care, and existing fringe benefits that they would be willing to give up in order to obtain employer supported child care services. Results indicate that a majority of the 41 employees in the sample with children under 12 years of age experience problems relating to child care and that those problems affect their work performance. There is no clear preference for type of child care, however, and employees are unwilling to give up existing fringe benefits to obtain child care services. One of the risk factors in foster placement breakdown is the presence of the carers' own children. A project is described through which carers' children express their feelings and conflicts and present a demand for a review of foster care, to be seen as care by families, not only parents. Many children in care have a powerful psychological impact on their carers, and the impact on other children in the household is underestimated and poorly understood. There is a confusion of role between carers' children and children who are fostered—on the one hand, the former are peers, and on the other, considerable demands of patience and understanding are made on them. This understanding, and perhaps also fear of allegations, may make them prematurely adult and distort aspects of their own development. The need for further research into the direct experience of foster care is highlighted and a training model of preparation for carers and their children is described, based on direct work with adolescents.  相似文献

From a sample of low-income households, out-of-pocket medical expenses are found to average about $25 per month. For each household, these expenses vary with annual income, type of insurance for medical care, priority of medical expenses, ethnicity, and number of ill family members. Expenses are reported for households with different socioeconomic characteristics and composition. Multiparticipation in insurance programs is shown. Fourteen percent of the survey participants say they have no insurance, public or private. Thirty-three percent participate in Medicaid.Flora L. Both Japan and (West) Germany were subjected to foreign “Allied”Occupation from 1945. In this fiftieth anniversary year of the ending of World War Two and the commencement of these periods of Occupation, this paper assesses and compares the impact of the latter experience on social policy development in Germany and Japan. In particular, it focusses on social assistance and unemployment insurance, on the grounds that provision for “the able-bodied poor”constitutes a clear guide as to how far each society has progressed or is progressing along the road to “social citizenship”as defined by T. H. Marshall. The import of Allied Occupation was in practice quite different in the two cases, not least because of their very different paths of social policy development beforehand. Developments in the wake of Occupation and upto the present are nonetheless indicative of there being some elements of policy “convergence”, which welfare state regime theory has hitherto failed to allow for in its concentration upon “whole systems”at the expense of more detailed policy “subsystem”review.  相似文献
