There have been few longitudinal studies investigating the immigrant health and changes in their health with longer residency in the host country. Additionally, the pathways and mechanisms by which transition of health over time occurs are poorly understood, limiting the ability to implement policies that will result in improved health for all, including immigrants. We assessed differences in health outcomes among foreign-born people from English speaking countries and non-English speaking countries relative to native-born Australians over a 10-year period using a large representative longitudinal dataset. We also explored English language proficiency, socio-economic factors and health behaviour factors as possible mechanisms through which health outcomes change over time post-migration. Conventional multilevel mixed and hybrid regression models were used to evaluate health outcomes in 9558 native-born and 3067 foreign-born people from the Household, Income and Labour Dynamics in Australia survey. There were clear differences in physical health, mental health and self-assessed health between foreign-born subgroups in comparison with native-born Australians. Foreign-born people from English speaking countries typically had a health advantage relative to native-born people, and foreign-born people from non-English speaking countries had a health disadvantage with respect to native-born people for all health outcomes. There was no evidence that these differences changed by duration of residence except for self-assessed health amongst foreign-born people from non-English speaking countries when duration of residence exceeded 20 years. English language proficiency mediated the relationship between duration of residence and health for foreign-born people from non-English speaking countries.  相似文献

In order to analyse the salience of race/ethnicity, sexual orientation, and adoptive status for lesbian adoptive parents, this study examined the challenges and strengths described by 20 White lesbian couples (40 women) who were adopting racial/ethnic minority children from within the United States. Data from two time points (pre‐ and post‐adoptive placement) were analysed qualitatively. Results indicated that the majority of women voiced concerns about raising racial/ethnic minority children in a ‘White privileged’ society. Many women reported facing a lack of understanding from others and were aware of multiple layers of stigma. However, having faced discrimination themselves as sexual minorities, many also perceived themselves as possessing unique strengths (e.g., experience coping with stigma). Many women expressed feeling that by drawing on support from their communities as well as their own awareness of and experience with diversity issues, they could prepare their children for the challenges they might face.  相似文献

The Investigating Choice Experiments Capability Measure (ICECAP) is a new preference-based measure of the extent to which a person is able to achieve attributes or capabilities related to the quality of life. Conceptually, it differs from health-related quality of life (HRQoL) as the focus is upon the ability or capacity to achieve as distinct from the current experience of the attributes. The objective of this study was to explore the empirical relationships between capability as assessed by the ICECAP for Adults (ICECAP-A) and HRQoL as assessed by the Assessment of Quality of Life (AQoL)-8D and the five-level EuroQol Five Dimensions questionnaire (EQ-5D). To compare these measures, the study employed self-reported survey data from the healthy public and from seven disease areas in five countries. Results indicate that, despite their conceptual origins, the ICECAP-A is strongly associated with the AQoL-8D and that the clear distinction between capabilities and HRQoL found in other studies is attributable to the use of the EQ-5D in the comparison and the weaker association between the EQ-5D and ICECAP-A. The suggestion that ICECAP-A should be included in evaluation studies along with a HRQoL instrument is more persuasive when the instrument is the EQ-5D. The case for its inclusion with other HRQoL instruments requires further research and evaluation.  相似文献

As I reviewed the available case information on Allan, the concept of career development shaped my impressions of the client. “Career development is, for most people, a lifelong process of getting ready to choose, choosing, and typically, continuing to make choices from among the many occupations available in our society. Each person undertaking this process is influenced by a great number of factors, including family, personal values and aptitudes, and societal context” (Brown, Brooks, & Associates, 1990, p. xvii). As this statement suggests, career development is a holistic approach to counseling individuals. Each “factor” represents a piece of the puzzle that guides the individual through the process. Life-style issues (i.e., family, leisure, sexual orientation, personal values, societal context), as well as interests, work values, and skills are tightly interwoven within an individual's major or career choice.  相似文献

This paper argues that the popular American TV series The West Wing provides, within popular culture, a sympathetic and realistic view of the work of the political ‘spin doctor’. This enterprise is sociolinguistically interesting because of the extensive range of metalinguistic and metacommunicative reflection which it involves. The West Wing centres its storytelling within the world of the spin doctor, thus making such characters central to the narrative: further, it constructs them as likeable, virtuous characters who are also clever. Their cleverness is a matter of sociolinguistic performance expressed in dialogue which displays their professional writing craft skills and their spontaneous witty badinage.  相似文献
