Acute Stress Disorder in physical assault victims visiting a Danish emergency ward

The purpose of this article is to assess the prevalence of Acute Stress Disorder (ASD) following violent assault in victims who come to the emergency ward, and compare the effects with degrees of injury. Two hundred and fourteen victims of violence completed a questionnaire 1 to 2 weeks after the assault. Measures included the Harvard Trauma Questionnaire, the Trauma Symptom Checklist and the Crisis Support Scale. RESULTS: Twenty-four percent met the full ASD diagnosis and 21% a subclinical ASD diagnosis. Childhood sexual and physical abuse and shock due to a traumatic event that happened to someone close increased the likelihood of ASD four to ten times. Feeling of security and ability to express feelings reduced the likelihood of ASD by one-quarter, while feeling let down by others and hopelessness increased the likelihood of ASD respectively 1.4 and 2.6 times.     

Intimacy boundaries: between mental health nurses & psychiatric patients

This was the first research study in Canada to explore intimacy boundary violations and sexual misconduct between nurses (both RNs and registered psychiatric nurses) and patients. Using a researcher-generated survey, a total of 923 mental health nurses commented on their sexual attraction to patients, and dating and sexual intercourse patterns with patients. The findings indicated that very few nurses had dated or engaged in sexual intercourse with discharged patients, and the few nurses who had done so tended to be younger men prepared at the registered psychiatric nursing diploma level. A small number of nurses believed it was permissible to have a sexual relationship with a patient while the patient was hospitalized, but none reported having a current relationship. Given the severity of this intimacy boundary violation, nurses need to be educated regarding the serious and dangerous psychiatric effects that can result for patients from engaging in a sexual relationship with nurses. The Code of Ethics of the Canadian Nurses Association and nurses' obligation to follow it needs to be reinforced. Nurses engaging in intimacy boundary violations are vulnerable to patient-initiated lawsuits.     

Logs as adjuncts to therapy for adolescents in a residential psychiatric program

As part of a 3-year ethnography of an adolescent residential psychiatric program, staff, patients, and occasionally family members were observed communicating using written logs. Using participant observation and opportunistic interviewing, and reading the logs, the researcher was able to determine the advantages, disadvantages, and direct value of the logs to patients and staff. Logs, when used judiciously after patient assessment, demonstrated patient responsiveness to treatment and promoted patient-centered care. Writing in logs increased patients' identification of thoughts and feelings, mirrored their developmental state, and demonstrated staff members' skills in communication. They are an invaluable adjunct to therapy for certain adolescent patients.     

The effect of illegitimacy on U.S. general fertility rates and population growth

Abstract Illegitimacy and bridal pregnancy has attracted the attention of scholars concerned with a variety of plausible causes and consequences ofbirths conceived out of wedlock. Sociologists have expressed concern with the impact of such births on maternal and child health, family stability and the economic status offamilies. Although demographers have described trends in various populations, and have occasionally suggested that births conceived out of wedlock influence general fertility rates and cohort fertility, measures of the demographic impact of illegitimate or premaritally conceived births have not been assessed. While it seems obvious that a change in births conceived out of wedlock will affect change in a general fertility rate, this finding would not automatically allow the conclusion that cohort fertility would change because a change in the rate of births conceived out of wedlock occurred.

Patients identified the behaviors of caring, health teaching, and role modeling as helpful to their recovery. 4. Patients have two primary resentments--a lack of privacy and continuity of care.
