We prospectively evaluated erectile function (EF) using the Sexual Health Inventory for Men (SHIM) and the erectile hardness score (EHS) as well as urinary statuses using the International Prostate Symptom Score (IPSS) and Overactive Bladder Symptom Score (OABSS) before and 3, 6, and 12 months after a daily treatment with 0.5?mg dutasteride (DUT). Significant improvements were observed in IPSS and OABSS in 98 patients with the DUT treatment, and the effects were similar between 28 patients with potency with baseline SHIM of 8 or greater and 70 severe erectile dysfunction (ED) patients at baseline. In the 28 patients with potency, significant decreases were observed in SHIM and EHS after 3, 6, and 12 months of the DUT treatment, with the severity of ED according to SHIM deteriorating in half of these patients after 12 months of the DUT treatment. Eighteen out of 28 patients (64.3%) with potency at baseline had awareness of the occurrence of ED before the DUT treatment, were younger, and had higher SHIM and EHS just before the DUT treatment than their counterparts. Regular assessments of EF may be needed, especially in younger patients and those with higher levels of EF before the administration of DUT. 相似文献

This article considers computational procedures for the waiting time and queue length distributions in stationary multi-class first-come, first-served single-server queues with deterministic impatience times. There are several classes of customers, which are distinguished by deterministic impatience times (i.e., maximum allowable waiting times). We assume that customers in each class arrive according to an independent Poisson process and a single server serves customers on a first-come, first-served basis. Service times of customers in each class are independent and identically distributed according to a phase-type distribution that may differ for different classes. We first consider the stationary distribution of the virtual waiting time and then derive numerically feasible formulas for the actual waiting time distribution and loss probability. We also analyze the joint queue length distribution and provide an algorithmic procedure for computing the probability mass function of the stationary joint queue length. 相似文献

Background
Older individuals have been shown to present muscle atrophy in conjunction with increased fat fraction in some muscles. The proportion of fat and connective tissue within the skeletal muscle can be estimated from axial B-mode ultrasound images using echo intensity (EI). EI was used to calculate the index of muscle quality. Walking, home-based weight-bearing resistance training, and its combinations are considered simple, easy, and practical exercise interventions for older adults. The purpose of this study was to quantify the effects of walking and walking with home-based resistance training on muscle quality of older individuals.Methods
Thirty-one participants performed walking training only (W-group; 72?±?5?years) and 33 participants performed walking and home-based resistance training (WR-group; 73?±?6?years). This study was a non-randomized controlled trial with no control group. All participants were instructed to walk 2 or 3 sets per week for 10?weeks (one set: 30-min continuous walking). In addition, the WR-group performed home-based weight-bearing resistance training. EI was measured as a muscle quality index using axial B-mode ultrasound images of the rectus femoris and vastus lateralis of the mid-thigh. We further averaged these parameters to obtain the EI of the quadriceps femoris (QF). Participants further performed five functional tests: sit-ups, supine up, sit-to-stand, 5-m maximal walk, and 6-min walk.Results
QF EI was significantly decreased in both groups after training (W-group 69.9?±?7.4?a.u. to 61.7?±?7.0?a.u., WR-group 64.0?±?9.5?a.u. to 51.1?±?10.0?a.u.; P?<?0.05), suggesting improved muscle quality. QF EI was further decreased in the WR-group compared with the W-group. The sit-up test in both groups and the sit-to-stand and 5-m maximal walk tests in the W-group were significantly improved after training.Conclusion
These results suggest that training-induced stimulation is associated with a decrease in EI in some thigh regions. Furthermore, the addition of home-based resistance training to walking would be effective for a greater reduction of EI.34.

Many fields of research need to classify individual systems based on one or more data series, which are obtained by sampling an unknown continuous curve with noise. In other words, the underlying process is an unknown function which the observed variables represent only imperfectly. Although functional logistic regression has many attractive features for this classification problem, this method is applicable only when the number of individuals to be classified (or available to estimate the model) is large compared to the number of curves sampled per individual.To overcome this limitation, we use penalized optimal scoring to construct a new method for the classification of multi-dimensional functional data. The proposed method consists of two stages. First, the series of observed discrete values available for each individual are expressed as a set of continuous curves. Next, the penalized optimal scoring model is estimated on the basis of these curves. A similar penalized optimal scoring method was described in my previous work, but this model is not suitable for the analysis of continuous functions. In this paper we adopt a Gaussian kernel approach to extend the previous model. The high accuracy of the new method is demonstrated on Monte Carlo simulations, and used to predict defaulting firms on the Japanese Stock Exchange. 相似文献

Objectives. Does size of democracy shape citizens' support for the democratic regime? Previous literature demonstrates that a variety of political contexts are associated with the level of system support among citizens, yet none has tested the effect of a fundamental contextual feature of contemporary societies—size of democracy—on system support from a cross‐national perspective. Methods. Survey data for nations of different sizes in Europe, North America, and Oceania are analyzed using a hierarchical generalized linear model. Results. Empirical analysis shows that less populous states and highly decentralized states demonstrate higher system support among citizens, and it offers some evidence that the negative effect of population size is moderated by the level of decentralization. Conclusions. My findings are consistent with the common expectation that smaller size of nation and governments “closer to the people” are preferred. Further, they have substantive implications for contemporary democratic governance. 相似文献

We discuss Bayesian inferential procedures within the family of instrumental variables regression models and focus on two issues: existence conditions for posterior moments of the parameters of interest under a flat prior and the potential of Direct Monte Carlo (DMC) approaches for efficient evaluation of such possibly highly non-elliptical posteriors. We show that, for the general case of m endogenous variables under a flat prior, posterior moments of order r exist for the coefficients reflecting the endogenous regressors’ effect on the dependent variable, if the number of instruments is greater than m +r, even though there is an issue of local non-identification that causes non-elliptical shapes of the posterior. This stresses the need for efficient Monte Carlo integration methods. We introduce an extension of DMC that incorporates an acceptance-rejection sampling step within DMC. This Acceptance-Rejection within Direct Monte Carlo (ARDMC) method has the attractive property that the generated random drawings are independent, which greatly helps the fast convergence of simulation results, and which facilitates the evaluation of the numerical accuracy. The speed of ARDMC can be easily further improved by making use of parallelized computation using multiple core machines or computer clusters. We note that ARDMC is an analogue to the well-known “Metropolis-Hastings within Gibbs” sampling in the sense that one ‘more difficult’ step is used within an ‘easier’ simulation method. We compare the ARDMC approach with the Gibbs sampler using simulated data and two empirical data sets, involving the settler mortality instrument of Acemoglu et al. (2001) and father's education's instrument used by Hoogerheide et al. (2012a). Even without making use of parallelized computation, an efficiency gain is observed both under strong and weak instruments, where the gain can be enormous in the latter case. 相似文献
