Background

The increasing prevalence and adverse outcomes associated with opioid analgesia use in women of reproductive age have become a significant public health issue internationally, with use during pregnancy potentially affecting maternal and infant health outcomes.

Objective

This study aims to provide national estimates of chronic pain, pain severity and analgesia use in Australian women of reproductive age by pregnancy status.

Method

Data were obtained from the Australian Bureau of Statistics 2011–12 National Health Survey (n = 20,426). Weighting was applied to sample data to obtain population estimates. For this study data were analysed for pregnant (n = 166, N = 192,617) and non-pregnant women (n = 4710, N = 5,256,154) of reproductive age (15–49 years).

Results

Chronic or reoccurring pain was reported in 5.1% of pregnant women and 9.7% of non-pregnant women, and 0.7% and 2.6% of pregnant and non-pregnant women reported recent opioid analgesia use respectively. Moderate-to-very severe pain was more common in pregnant than non-pregnant women taking opioid analgesics, and no pain and very mild-to-mild pain in non-pregnant women.

Conclusion

Approximately 1 in 20 pregnant Australian women have chronic or reoccurring pain. Opioid analgesia was used by around 1% of Australian pregnant women during a two-week period, with use associated with moderate-to-very severe pain. Given that the safety of many analgesic medications in pregnancy remains unknown, pregnant women and health professionals require accurate, up-to-date information on the risks and benefits of analgesic use during pregnancy. Further evidence on the decision-making processes of pregnant women with pain should assist health professionals maximise outcomes for mothers and infants.  相似文献

Racial socialization was examined as a protective factor that might buffer African American youth from the negative effects of perceived racial discrimination. Two types of racial socialization were examined: messages about race pride and preparation for bias. One hundred twenty-eight eighth-grade African American students participated in the study. As anticipated, both types of socialization moderated the relationship between discrimination and self-esteem. The negative relationship between perceived discrimination and self-esteem was mitigated for youth who reported more messages about race pride and a moderate amount of preparation for bias from their parents. In contrast, low race pride socialization and both high and low preparation for bias were associated with a negative relationship between perceived discrimination and self-esteem.  相似文献

Two‐way bilingual immersion programs are noteworthy within the context of US public education because, in them, the children of immigrants are essential assets. Language‐minority and English‐speaking pupils are grouped together, starting in kindergarten or earlier and extending at least through grade five, with a goal of bilingual proficiency and biliteracy for all. This paper explores two‐way immersion educators’ and parents’ responses to California Proposition 227, which severely restricted bilingual education. Given this obstacle, what has motivated the maintenance and further initiation of two‐way bilingual immersion? And to what ends? Answers hinge on varied institutional climates at the school district level and parent–teacher activism towards the goal of truly multicultural education. Proposition 227 was a disastrous move, but dedicated educators, parents and communities have been proactive in creating two‐way bilingual programs to help maintain bilingual education in a way that benefits all students.  相似文献

Informed by critical race theory (CRT), we examine how African-American and white college students, at a predominantly white, structurally diverse, Southern US university, understand their cross-racial experiences. Black–white interactions are understood within the context of the so-called ‘post-racial’ environment, against the backdrop of high-profile cases of racial injustice, and within the added context of the historical legacy of slavery and Jim Crow segregation in the rural Southern United States. Our study suggests that many students, regardless of race, recognized the persistence of racial segregation, especially in nightlife and campus Greek letter organizations (GLOs). African-American students were the most vocal and troubled by this division. Unexpectedly, however, students appeared to take for granted that in the American South, racism is persistent and indestructible. Building on Bell’s (1991) notion of racial realism and Bonilla-Silva’s (2013) notion of naturalization, we expand the view that racism is inherent or related to individual preference, to place and time, with a construct we term southern assumptions. Southern assumptions are the mechanisms in which participants connect collective historical racism in the south to the race problems of today.  相似文献

Access to resources and social relationships are important for teacher development. Unfortunately, within higher education contingent faculty often are under resourced and poorly integrated into their department’s social network. This shortfall could be addressed by Open Educational Resources (OERs), which are websites that freely distribute pedagogical resources and provide a platform for educators to form collegial relationships. The current research focuses on the OERs that have formed around American sociology to assess the characteristics of the faculty who use them, the pedagogical resources they acquire from them, and how often their users form collegial relationships online. Analysis of an online survey of 275 sociology OER users finds that contingent faculty are acquiring resources from the sites. While only a small percentage of faculty are forming collegial relationships via these sites, they are doing so without occupational status, gender, or racial differences.  相似文献

As cities expand, conservation of beneficial insects is essential to maintaining robust urban ecosystem services such as pollination. Urban warming alters insect physiology, fitness, and abundance, but the effect of urban warming on pollinator communities has not been investigated. We sampled bees at 18 sites encompassing an urban warming mosaic within Raleigh, NC, USA. We quantified habitat variables at all sites by measuring air temperature, percent impervious surface (on local and landscape scales), floral density, and floral diversity. We tested the hypothesis that urban bee community structure depends on temperature. We also conducted model selection to determine whether temperature was among the most important predictors of urban bee community structure. Finally, we asked whether bee responses to temperature or impervious surface depended on bee functional traits. Bee abundance declined by about 41% per °C urban warming, and temperature was among the best predictors of bee abundance and community composition. Local impervious surface and floral density were also important predictors of bee abundance, although only large bees appeared to benefit from high floral density. Bee species richness increased with floral density regardless of bee size, and bee responses to urban habitat variables were independent of other life-history traits. Although we document benefits of high floral density, simply adding flowers to otherwise hot, impervious sites is unlikely to restore the entire urban pollinator community since floral resources benefit large bees more than small bees.  相似文献

Social work is a demanding profession as practitioners routinely face difficult situations that affect their well-being. The National Association of Social Workers strongly supports self-care practice as an approved mechanism to offset these challenges, yet practitioners report not learning techniques necessary to perform self-care. In this study, a systematic review compliant with Preferred Reporting Items for Systematic Reviews and Meta-Analyses standards was conducted to identify evidence-based interventions used to improve student self-care practice in social work education. In the four studies meeting inclusion criteria, mindfulness practice was the only empirically evaluated self-care strategy reported—and with mixed results. Mindfulness activities enable social workers to sustain their well-being and is critical to modeling and providing effective service delivery to clients. Social workers are called to respond to disasters to provide assessment, crisis counseling and trauma treatment as part of the recovery effort. While research has identified the types of interventions appropriate in assisting trauma survivors, little empirical research exists to help address the distress responses that social workers experience when exposed to the trauma material of disaster survivors and their families. Case examples can inform social work practice using evidence informed interventions and recording the outcomes. This article presents three case histories of social workers responding to disasters, the course of their secondary traumatic stress responses and their struggle to overcome the psychological adversity they faced.  相似文献

ABSTRACT

This qualitative study of 77 individuals in 40 couples (same-sex and heterosexual), who had adopted publically, privately, or internationally, examined parents' engagement with their child's birth family via technology (e.g., texting, e-mail, social media) through the lens of the Couple and Family Technology frame-work (Hertlein & Blumer, 2014 Hertlein, K. M., & Blumer, M. L. C. (2014). The couple and family technology framework. New York, NY: Routledge. [Google Scholar]). Parents used three approaches to contact: active, passive, and no contact. Regardless of approach, some parents described concerns about boundaries. Couples were generally in agreement in their perspectives on engaging with birth family via technology. Practitioners must be know-ledgeable about management of relationships via technology and help adoptive families set healthy boundaries.  相似文献
