Secure Beginnings is a program of psychotherapy for pregnant women with potential problems in the formation of the parent–child relationship including, in the extreme, infant maltreatment. The core of the therapeutic process is to help the mother recognize and think through unbearable thoughts and feelings rather than project expectations of abandonment, hate, helplessness or ridicule into her infant. Case material, from 18 months of treatment, is used to illustrate the Secure Beginnings approach. The case is a mother with psychosis and addiction who lost nine previous children to foster care.  相似文献

Huntington Disease (HD) is a hereditary neurological disease that typically occurs in mid-life. There is no prevention or cure, but persons at risk may request predictive genetic testing to learn whether or not they will develop the disease later in life. Many studies describe why such persons request this knowledge. Few consider how people actually make this novel decision. Drawing on in-depth interviews with sixteen test candidates, this article differentiates three types of stories about—as well as embedded in—the experience of deciding. Each type of story sheds a different light on the meaning and experience of making this decision. Stories of having to know reveal that decision-making may be experienced as a self-evident act while stories of evolving toward it feature an incremental process of weighing up the implications. Stories of taking the decision feature a pivotal point which involves the narrator in seeing anew the opportunity to make a choice. Emphasizing previously neglected temporal, biographical and phenomenological dimensions of decision-making, this typology underscores the relevance of narrative in complex processes of decision-making. It also illustrates the inadequacy of understanding decision-making purely through the lens of rational choice.  相似文献

Insights about the observer's self can prove useful for understanding others who are the subjects of sociological inquiry. This organizational sociologist studied a midwestern lesbian community using participant observation and in depth interviewing. Subsequent problems with preparing the findings led to development of an analytic technique for dealing with the data. This paper re-examines the question of the social “fabric” of urban neighborhoods on the basis of residents' personal networks. Data were collected on the number, relative intimacy, and spatial distribution of social relationships among residents of two ethnically homogenous and two ethnically heterogenous neighborhoods in a medium-sized city in the midwestern United States. The analysis focused on spatial distributions and variables associated with differences in the average number or intimacy of neighborhood network ties. Herbert Gans had predicted that in heterogenous neighborhoods residential proximity would be a less important factor in social network formation than has previously been reported for socially homogeneous residential settings (especially Festinger et al. 1950). The results from this study indicated that the effects of proximity were more, rather than less, reflected in the spatial distribution of social relationships in the ethnically heterogenous neighborhoods. The face-block was identified as an important socio-spatial unit in all four neighborhoods.  相似文献

This paper presents a review of the literature on the meaning, etiology and categorization of acting out symtomatology in children. It is apparent that there is no one cause nor is there one well defined disorder.DSM-III categories as they relate to behavior disorders are discussed and the problems of diagnosis are addressed. Appropriate treatment goals and interventions cannot be developed unless more global aspects of a child's functioning and the environment in which he lives are assessed.  相似文献

The purpose of this study was to learn about mainstream and residential school programs for deaf students in the United States from the perspective of the deaf person, and to present the experiences of clients of educational services in their own words. Data for this paper were collected through open-ended, in-depth interviews with 25 graduates from the National Technical Institute for the Deaf at Rochester Institute of Technology (NTID at RIT). The comments of informants suggest that there are advantages and disadvantages inherent in each educational model. For example, the selection of one model over another may involve 'trading' academic for social opportunity. In particular, it is suggested that both kinds of school experiences play a critical role in the socialization of deaf people and the development of deaf community. It is recommended that further research be conducted to learn more about the perspectives of deaf people on educational services and to explore with them the long term as well as the immediate impact of different kinds of school environments.  相似文献

This exploratory study examined the effectiveness of a foster parent nutrition training program. Conducted in collaboration between Woman, Infants and Children (WIC) and a state foster care licensing agency, the study involved providing an intensive training to 11 self-selected foster parents. The participants completed pre-tests and post-tests related to their level of knowledge and comfort in the area of infant nutritional needs. Paired t-tests detected significant differences before and after the intervention, with participants reporting improved knowledge and understanding. While the sample was small and voluntary, the authors feel these pilot results merit further attention in the areas of practice and research.  相似文献
