Enhancing Palliative Care for Low-Income Elders with Chronic Disease: Feasibility of a Hospice Consultation Model

Challenges exist in assimilating palliative care within community-based services for nursing home eligible low-income elders with complex chronic illness as they approach the end of life (EOL). This study assessed the feasibility of a consultation model, with hospice clinicians working with three Care Wisconsin Partnership Program teams. Consults occurred primarily during team meetings and also informally and on joint patient visits and were primarily with the palliative care nurse addressing physical issues. Fifty-seven percent of consultant recommendations were implemented. Benefits of consultation were identified with focus groups of clinical staff as were opportunities and barriers to the implementation. Models of integration are proposed.     

Activity-based blended learning

As part of the trend towards blended learning, the Open University of Shell International Exploration and Production is shifting from predefined content-based learning to activity-based learning grounded in learners' actual workplace problems. This article acts as a case study by describing key design principles adopted by the organization and also describes how the shift addressed courses that served multinational participants. Data are provided on the learning activities in thirty-seven courses redesigned in 2002 (covering nearly 300 separate learning activities). The article concludes with some comments on work that still needs to be done as well as key implications for practitioners in other organizations.     

Marriage in a British Evangelical Congregation: Practising Postfeminist Partnership?

This article explores understandings and practices of marriage in a congregation from the growing British evangelical Christian movement New Frontiers International (NFI). It investigates how those who subscribe to evangelical religion interact with the gendered ideas and behaviour of the more ‘secular’ society they inhabit. The data for this research are drawn from participant observation and interviews with members of the congregation. These are situated in the context of the official discourse of NFI and of contemporary debates about the move to individualized partnership. It is argued that though these evangelicals claim to shape their marriages according to ‘biblical’ patterns, they in fact reflect the partnership practices of their less religious peers. Building on work by Stacey and McRobbie, patterns of marriage and heterosexual partnership in contemporary Britain are conceptualized as ‘postfeminist’; the article locates within this framework NFI's declared – and undeclared – marriage practices. It finds that while they are somewhat more conservative than their ‘secular’ peers, NFI evangelicals are indeed practising postfeminist partnership. Observations are also offered on the impact of religion on people's ability to live out individualized partnerships.     

An impact evaluation of student teacher training in HIV/AIDS education in Zimbabwe

Zimbabwe introduced a nation-wide program to teach AIDS education in 1994. This paper evaluates changes in student teachers' level of knowledge about transmission, symptoms and prevention of STDs and HIV/AIDS; their attitude towards persons living with AIDS; and their sensitivity to the impact of the epidemic and to discussing and teaching about these issues. There was an increase in knowledge of HIV prevention and in teachers' ability to discuss reproductive health and sexual issues. However, as students were exposed to other HIV material outside the programme, not all of this change is due to the programme. Course attendance needed to be enforced and the curriculum needed to be updated with student participation. Peer educators and participatory techniques are needed to get students to internalise positive attitudes and behaviour. The education materials need to address the lack of female empowerment in making decisions and negotiating for safer sex.     

What Happened on Ruby Ridge: Terrorism or Tyranny?

In the 1992 Ruby Ridge, Idaho, incident during which U.S. Marshal William Degan and Sammy and Vicki Weaver were killed, law enforcement and the Weavers socially constructed each other's roles. We focus on how the framing of what happened on Ruby Ridge changed. Drawing on Gamson's (1968) discussion of law enforcement strategies, we suggest that certain federal agencies were challenged about the justness of their actions. The Weavers, white separatists, and others tried to alter the dominant frame of the federal government from a legitimating one to that of unjust authority. To counter that, investigations by the Senate Subcommittee on Terrorism and the Department of Justice portrayed certain agents as incompetent rather than unjust. Various federal agencies “yielded ground” by recognizing mistakes, making payments to the Weavers and Kevin Harris, and charging one official and temporarily suspending others, but they maintained the legitimacy frame.
