The study objective was to explore knowledge, attitudes, and behavior about advance directives and how cultural values influence these beliefs. Three focus groups with 23 Korean American older adults were conducted. Advance directives were seen as helpful for ensuring that preferences for unwanted end-of-life treatment are honored and for relieving the decision-making burden on family members. However, some viewed completing advance directives as contrary to focusing on living. Culturally competent education about advance directives for Korean American older adults is necessary to help them make informed decisions about end-of-life care and informing family and health care providers of these preferences.  相似文献

Contact between parents and children in care is a contested area. Parental contact is recognized to be important, yet may present protective issues; in the kinship care environment, it brings the particular challenges of complex family relationships. Seeking the parents' perspective in a child protection context is difficult and therefore under‐researched. This paper describes a nested study within an Australian research project on family contact in kinship care in which the perspectives of 18 mothers and 2 fathers were sought via in‐depth interviews. Mothers and fathers described strong feelings of disempowerment in the context both of their family and the child protection system. The relationship between parent and caregiver emerged as a significant issue. All of the parents wished to remain in contact with their children in a meaningful way, whether or not they were likely to resume their children's care; however, contact arrangements presented many difficulties for them. Mothers articulated the need for services that are more empowering and respectful, rather than oriented towards them as failed parents. In order to build appropriate models of support and intervention, we argue for a more inclusive conceptual frame for family life that gives greater recognition to the role of non‐custodial parents in the lives of their children.  相似文献

Statutory child protection has a long and contested history in domestic violence intervention where children are involved. The inadequacy of the response has been raised by researchers, women and their advocates for more than 20 years. In fact, Linda Gordon, in her classic analysis of the files of charitable agencies from 1880 to 1960, showed that violent men were ignored, women whose own lives were in danger were judged as inadequate when they failed to protect their children and ‘child rescue’ was frequently seen as the solution to protecting vulnerable children. The same issues continue to be raised today. The repetitive pattern points to the structured nature of the problem which goes beyond the intentions and practice of individual workers. A policy response and a cultural change is needed, which tackles not only the ‘age old’ but also some of the newer permutations of intervention in this area. This paper will explore aspects of this structured pattern and then draw from international research and projects which are grappling with positive changes to child protection intervention in this area.  相似文献

Partnerships among service providers are an important aspect of human service delivery, including in the early childhood and family service sector. There is extensive international literature on factors contributing to partnerships – also termed service coordination, collaboration or integration – but little evidence of partnership outcomes exists where partnerships are a funded and mandatory component of large‐scale programmes. This paper reports findings from an evaluation of the Australian Government's Communities for Children (CfC) programme. Under CfC, partnerships were mandated and funded, and the evaluation findings show that the programme resulted in an increased number of agencies working together to support families with young children (0–5 years) and that working relationships between agencies improved. The effectiveness of these partnerships depended on funding for partnership activities and on organisational and practical factors.  相似文献

Problem

Post-traumatic stress disorder and post-traumatic stress symptoms following birth occur amongst a small proportion of women but can lead to poor maternal mental health, impairment in mother-infant bonding and relationship stress. This integrative review aims to examine the associated risk factors and women's own experiences of postnatal post-traumatic stress in order to better understand this phenomenon.

Method

Fifty three articles were included and critically reviewed using the relevant Critical Appraisal Skills Program checklists or Strengthening the Reporting of Observational studies in Epidemiology assessment tool.

Findings

Risk factors for postnatal post-traumatic stress symptoms and disorder include factors arising before pregnancy, during the antenatal period, in labour and birth and in the postnatal period. Potential protective factors against postnatal post-traumatic stress have been identified in a few studies. The development of postnatal post-traumatic stress can lead to negative outcomes for women, infants and families.

Discussion

Risk factors for post-traumatic stress symptoms and disorder are potentially identifiable pre-pregnancy and during the antenatal, intrapartum and postnatal periods. Potential protective factors have been identified however they are presently under researched. Predictive models for postnatal post-traumatic stress disorder development have been proposed, however further investigation is required to test such models in a variety of settings.

Conclusions

Postnatal post-traumatic stress symptoms and disorder have been shown to negatively impact the lives of childbearing women. Further investigation into methods and models for identifying women at risk of developing postnatal post-traumatic stress following childbirth is required in order to improve outcomes for this population of women.  相似文献

Within this article, we convey ideas about stereotypes and ethnic supremacy that many university students tend to hold about Jamaica and the challenges of disabusing travellers of these notions and to achieve educational goals related to equity, diversity and inclusivity. We explore the concept of the tourism imaginary and key ideas in critical pedagogy, critical literacy and critical race theory that guided our course development, and we explain how we used these concepts as a framework for building intercultural competence within our student population. By addressing the complexities of Jamaica’s transnational cultural identity and using tourism to deconstruct the idea of experiencing an ‘authentic’ Jamaica, this two-part immersion course works to shift student consciousness from racial and ethnic superiority to a place of intercultural competence.  相似文献

Background

Aboriginal women and families are under-represented in Australian research on pregnancy and childbirth. The Aboriginal Families Study aimed to investigate the views and experiences of a representative sample of women giving birth to an Aboriginal baby in South Australia between July 2011 and June 2013, using methods designed to respect Aboriginal culture and communities.

Methods

A team of 12 Aboriginal researchers facilitated community engagement and recruitment of Aboriginal and non-Aboriginal mothers of Aboriginal infants in urban, regional and remote areas of South Australia over a two-year period.

Results

A total of 344 women took part, around a quarter of all Aboriginal women giving birth in South Australia in the study period (39% urban, 35% regional and 25% from remote areas). Participants were representative in relation to maternal age (mean age of 25 years, range = 15–43 years). Over half of women (56%) first heard about the study via a member of the fieldwork team making contact with them through community connections. Other major sources of recruitment were: Aboriginal health services/programs (20%) and public maternity hospitals (16%). Almost all of the women (95%) recruited via community networks of the fieldwork team completed the questionnaire. In contrast, 51% of women recruited via public hospitals completed the questionnaire (odds ratio = 0.1, 95% confidence interval 0.0–0.1, p < 0.001).

Conclusions

Aboriginal researchers’ community knowledge and leadership is critical to the conduct of successful Aboriginal health research. High levels of participation in research by ‘harder to reach’ populations are achievable when researchers take time to build relationships and work in partnership with communities.  相似文献

Contact with family for children in care is identified as a right under the United Nations Convention on the Rights of the Child. However, such contact often presents challenges because of the protective concerns that have led to care arrangements being made. The Family Links: Kinship Care and Family Contact research study explored the nature and extent of family contact in kinship care, with a particular focus on the circumstances that create positive contact and foster family relationships. Findings included evidence of a large proportion of parental contact that was negatively affecting children's well‐being, and was sometimes unsafe. By contrast, the frequent contact that children were enjoying with their siblings and wider family was reported to be mostly positive and supportive. Kinship carers described a range of services needed to facilitate more positive parental contact and to enable children to keep contact with significant family members.  相似文献

Although the importance of contact between children in care and their parents, when safe, is accepted, there is limited research about supervised face‐to‐face contact. There is no literature that has systematically critiqued how supervised contact can be best delivered. The aim of this systematic review was to evaluate the evidence for interventions aimed at improving the quality of contact visits between parents and their children who are in out‐of‐home care. Twelve studies were included in this review. Each study was graded and assigned scores according to the presence or absence of each of seven criteria. The studies demonstrated key similarities in the types of interventions provided, although delivery varied across group, individual and educational interventions. Parents reported improved capacity to manage their emotions and parents' satisfaction with the programmes was high. Although there was a lack of large scale, methodologically rigorous studies with long‐term follow‐up, some promising findings were identified: the literature indicates individual family support and group programmes have the potential to improve parent–child relationships and the quality of contact visits. This review suggests that future studies build on current evidence by addressing their methodological limitations and evaluating interventions that can be tailored to meet the needs of individual families.  相似文献
