Concepts from family systems theory are used to facilitate understanding of the needs of families coping with the spinal cord injury of an adolescent. Five assumptions derived from systemic principles are discussed as related to assessment and family functioning. Use of an autobiographical interviewing approach in the psychosocial assessment process allows the clinician efficient access to rich diagnostic material in a way that is comfortable for the client. This approach invites rather than discourages storytelling and is consistent with recent interest in narrative perspectives on assessment and treatment. Heinz Kohut has contributed a new understanding of altered states as fixations on aspects of early developmental forms of selfobjects. Fixations can occur as a result of inadequate empathic relatedness necessary for the internalization of anxiety relieving functions. Fixations on aspects of archaic forms of the idealized selfobject may take the form of ecstatic, trance-like religious feelings. These altered states provide needed sensations that substitute for missing psychic structure. Understanding the self sustaining function that these altered states provide is crucial to the treatment. The author shares moment to moment steps in the treatment of a highly successful professional who is addicted to heightened sensation states. There are substantial numbers of high school dropouts among middle class, high achieving and majority youth. Yet, little research has been undertaken to identify the psychosocial and family characteristics, educational needs, and program requirements of middle class dropout youth. This paper reports the educational history, individual psychosocial characteristics and family backgrounds of 102 middle class dropout youth who participated in an educational treatment program within a university. Data from a questionnaire and a family assessment instrument indicate that middle class dropout youth have a variety of family, social, emotional and learning disorders. Implications for clinical interventions are presented along with a case history and treatment plan of one of the youth in the study. 相似文献

Significant numbers of adolescents in foster placement plan to live independently following discharge. Mentoring is increasingly being used as a support service to assist older foster youths to make the transition to adult living. A survey of 29 child welfare programs indicate that a variety of mentoring models are in use. The main models are transitional Life Skills Mentors, Cultural-Empowerment Mentors, and Corporate-Business Mentors. Mentoring connects foster wards with a cross-section of community citizens who provide a bridge to higher education and employment, and serve as a resource for transitional problem-solving. Information is needed on the impacts of mentor-mentee matches and efforts to sustain mentor-mentee relationships.Based on a paper presented at the Second National Child Welfare Conference, Division of Child Welfare Research, Arlington, Virginia, 1994. The purpose of this study was to describe policy and practice with respect to the assessment of intimate partner violence in a sample of child welfare agencies located throughout the United States and to examine the relationship of contextual characteristics and assessment practices. Telephone interviews were conducted with key informants from child welfare agencies. A snowball interviewing strategy was used to identify the best informant in each agency. Almost all of the participating agencies conducted some assessment of intimate partner violence, with most reporting that the majority of screening or assessment occurred during investigation of referrals. However, only 43.1% reported that all of the families referred to the child welfare system were assessed for intimate partner violence, and 52.8% indicated they had a written policy pertaining to screening and assessment of the problem. There was little relationship between county or agency characteristics and assessment practices. Additional research is needed to determine factors that influence assessment practices and to identify strategies to support and extend efforts to identify intimate partner violence and provide appropriate services for families in the child welfare system. 相似文献

Between 400,000 and 800,000 infants are born in the United States each year to women who smoked cigarettes during their pregnancy. Whereas the physical health consequences to infants of prenatal exposure to smoking are well established, the early neurobehavioral consequences are less well understood. This study investigated the neurobehavioral consequences of prenatal exposure to smoking using an epidemiologically derived sample of 454 infants who were drawn from a larger sample of 1,292 infants whose families were recruited at birth. Results indicated that, on average, infants who were exposed to higher levels of prenatal smoking exhibited less positive affect and greater irritability. Moreover, among male infants, elevated levels of prenatal exposure to smoking were associated with lower levels of approach, gross motor movement, reactivity, and attention. There was no evidence that the effects of prenatal exposure to smoking on infant neurobehavioral functioning were mediated by physical growth parameters (infant weight and head circumference) that are also known to be affected by prenatal exposure to smoking. 相似文献
