Direct payments have brought new opportunities for self-determinationand independent living to disabled people in the UK, featuringprominently in government strategy and the 2006 White Paper,Our Health, Our Care, Our Say. However, ten yearsafter direct payments legislation, take-up remains low and implementationvaries greatly. Rates of take-up in England remain more thandouble those in other parts of the UK, raising questions aboutdevolution and equity. This paper presents data from a nationalstudy to examine some of the mechanisms underlying uneven outcomesfor disabled people in different parts of the United Kingdom,with particular reference to the politics of devolved governance.The analysis focuses on scope for interpretations of policy;resources for information and training; the impact of mandatoryduties and targets; extensions to new user groups; and the roleof support organizations and disability activism. The evidencesuggests that local variations have been produced not only bylocal factors, but also by different opportunitystructures for policy development in England, Scotland, Walesand Northern Ireland. With the introduction of the highly publicized Sure Start Local Programme (SSLP) initiative in 1998, the UK government introduced a community‐based set of provisions targeted at families with children under 4 years who were living in some of the 20% most deprived and disadvantaged geographical areas in England. These programmes were designed to combat the adverse effects of poverty and disadvantage on the long‐term outcomes for children and families. One challenging dimension for programme development has been the facilitation of access to the services on offer in these programmes. Drawing on findings from the implementation module within the government‐commissioned National Evaluation of Sure Start, this paper examines the way in which SSLPs have sought to facilitate access to services for their local communities and to encourage community members to take up and continue using those services. This paper presents a detailed overview of programme activity in respect of access. It identifies a continuum of access and a model of three styles of parental service use. The paper highlights the implications of the SSLP implementation process for the development of subsequent related initiatives such as children’s centres and extended schools. 相似文献

ABSTRACTThis study aimed to explore the experiences of women with mental illness who had also experienced breast cancer diagnosis and treatment, in particular, to identify barriers and facilitators to cancer treatment. Semi-structured interviews were conducted with ten breast cancer survivors who had had contact with secondary mental health services prior to their cancer diagnosis. Thematic analysis was used to identify key themes. Participants described positive experiences of cancer treatment, expressing surprise at both how well they coped with diagnosis and treatment and how well they were treated by services. Many contrasted this with their less positive experience of mental illness and mental health services. The women interviewed were active participants in their cancer treatment and did not report feeling stigmatised because of their mental health experiences. A lack of connection between cancer and mental health services was described, but this was not regarded as necessarily a bad thing. This study begins to counter the stereotypes of the difficult patient or the person unable to understand and cooperate in their own care which are prevalent in the literature. Further work is needed to understand the reasons for disparities in cancer outcomes. 相似文献

ABSTRACTDespite a plurality of paternal forms available to men, certain enactments of fathering remain immobile. In South Africa the ‘father as provider’ discourse, which establishes the legitimate father as one who provides financially for his family, continues to be regarded as the primary mark of a good father. Using photo-elicitation interviews to interrogate the persistence of this discourse, this paper examines how adolescents from two low-resourced South African communities construct fathering. Using a critical intersectional discursive framework, the analysis focuses on how gender, race, class and culture are implicated in the reproduction of the ‘father as provider’ discourse. Participants’ constructions suggest that the father’s duty to provide is intensified within marginalised contexts. Central to participants’ reproductions of the ‘father as provider’ discourse was a neoliberal conception of ‘freedom of choice’. The paper concludes that South Africa’s post-apartheid reliance on discourses around liberal democracy has cast the low-income father’s ability to fulfil the provider role as a conscious choice. While the father is made responsible for his failure to provide, broader structural oppressions are in turn rendered largely invisible by such discourse. 相似文献

Māori people contribute significantly to the New Zealand Defence Force. Māori indigenous knowledge, however, did not feature in military curricula until recently. It was a common expectation that Māori personnel would conform to the dominant Western worldview, which often meant sacrificing indigenous identity. The introduction of the Service bicultural policies has paved the way for significant change in military education practices in the twenty-first century. This paper showcases the Māori cultural learning experiences of 34 members from the New Zealand Defence Force as they reflect on their experiences within the contemporary context of a military education and training environment, and the broader context of a decolonising society. This study argues that the inclusion of indigenous knowledge systems in military curricula improves the learning experiences of all members. Themes relating to individual development and improved organisational morale emerged that were found to be directly or indirectly attributed to the inclusion of Māori cultural training. The study is grounded in a multicultural military organisation, operating within a nation seeking to establish a bicultural co-equal partnership between Māori, the tangata whenua, original inhabitants, and non-Māori, the kaiwhakanoho whenua, or settler population, of New Zealand. 相似文献

OBJECTIVE AND PARTICIPANTS: The authors studied the prevalence of partner violence, by type, among Mexican American college women aged 18 to 35 years (N = 149; response rate = 85%). RESULTS: Twelve percent of women who reported a dating partner in the past year were physically or sexually assaulted, 12.1% were stalked, and 9.1% scored as psychologically abused. Among those experiencing partner violence, almost half experienced stalking and 89% reported psychological abuse. Few women (25%) who experienced physical violence believed violence was a problem in their relationship. CONCLUSIONS: Partner violence was prevalent in this population, and participants experienced many forms of violence. Because few women experiencing physical violence report that violence is a problem in their relationship, interventions must address perceptions of violence and its impact on women's mental and physical health in college populations. 相似文献

Domestic and family violence is a significant issue experienced by many children that can have severe detrimental impacts to their health, development, and well‐being. Despite the significance of this issue, it is only recently that children have been included in research that seeks to understand the impacts that domestic and family violence may have on their lives. This paper reports on the findings of a meta‐synthesis, which explored qualitative research about children's experiences of domestic and family violence. Thirty‐two studies, including from the United Kingdom, North America, and Australia were included for review. The meta‐synthesis found that children describe domestic violence as being a complex, isolating, and enduring experience that often results in disruption, losses, and challenges to their significant relationships. Children's common feelings of fear, worry, powerlessness, and sadness were also uncovered, in addition to the strategies they employed to try and facilitate the safety and emotional well‐being of themselves and their family. Children's wants and needs are also highlighted. The findings demonstrate that despite the increasing interest in children's experiences of domestic and family violence, qualitative research remains limited, with many gaps evident. and well-being among 30,023 Europeans aged 50 and above in 12 countries. There is an overall positive correlation between
volunteering and perceived health, life satisfaction, and self-life expectancy and a negative correlation to depression. However,

This paper reports on three complementary research studies that set out to examine the incidence and nature of single sessions in hospital social work practice. The first study was a data‐mining exercise that used statistics recorded by practitioners in nine public hospitals over a 12‐month period to establish the extent of single session work, the clinical areas in which it occurred, and the types of issues it covered. This quantitative information was enhanced with the qualitative findings of the two subsequent studies: the first with hospital social workers; and the second with singlesession clients. We concluded that single session work has not been sufficiently acknowledged in social work education and literature, and that it needs to become a more legitimate part of social work practice and training. At best, it represents social work at its highest level of skill, requiring expertise in quickly building rapport, assessment, and knowledge about sources of practical aid and information. 相似文献

Within a global profession with a stated definition that includes ‘promoting social change and development, social cohesion and the empowerment and liberation of people’ (online), it would be expected that the issue of domestic abuse would be integral to the training and role of all social workers. This article reports on research, which highlighted both a lack of understanding of the role of adult social worker within cases of domestic abuse and also a desire for further training around the issue. However, this article sets out how the current UK (in particular, English) context of social work marginalises the issue of domestic abuse within practice with adults. This marginalisation has been achieved through the construction of domestic abuse as a children and families issue and limited duties, powers and resources within statutory work to support victims/survivors in their own right, rather than as ‘failing’ parents. However, the article argues that the role of social work education should be wider than teaching to the current policy or procedures and instead encourage a wider appreciation of the social, historical and political context. The article concludes with tentative suggestions for how domestic abuse could be considered within the social work curriculum for adult practitioners. This is in acknowledgement that social workers can be well positioned for the detection, investigation and support of those experiencing abuse. 相似文献
