Adaptive designs for multi-armed clinical trials have become increasingly popular recently because of their potential to shorten development times and to increase patient response. However, developing response-adaptive designs that offer patient-benefit while ensuring the resulting trial provides a statistically rigorous and unbiased comparison of the different treatments included is highly challenging. In this paper, the theory of Multi-Armed Bandit Problems is used to define near optimal adaptive designs in the context of a clinical trial with a normally distributed endpoint with known variance. We report the operating characteristics (type I error, power, bias) and patient-benefit of these approaches and alternative designs using simulation studies based on an ongoing trial. These results are then compared to those recently published in the context of Bernoulli endpoints. Many limitations and advantages are similar in both cases but there are also important differences, specially with respect to type I error control. This paper proposes a simulation-based testing procedure to correct for the observed type I error inflation that bandit-based and adaptive rules can induce. 相似文献

ABSTRACTReverse mentoring is a means to address the social work Grand Challenge of social isolation. Among older adults, reverse mentoring can improve social connection by increasing the digital competence of older adults so they can use technology for social benefit, and by facilitating intergenerational connections with young adult mentors. In this paper, reverse mentoring is examined within an intergenerational program that serves older adults and utilizes the native technological knowledge and skills of young adults who mentor older adult participants. Qualitative data were collected through young adult mentor logs of each session, and through open-ended questions on the post-surveys collected from older adults and young adult mentors. Qualitative analysis revealed three themes related to social connection: (1) an increased sense of self-efficacy for older adults as they build confidence in technological use, and for young adults as they develop leadership skills through mentoring, (2) the breaking down of age-related stereotypes, and (3) intergenerational engagement and connection. The findings demonstrate that reverse mentoring can be used in various settings to decrease the social isolation of older adults by developing intergenerational connections and increasing older adult usage of technology. 相似文献

The notion that employment is a precondition for personal wellbeing and social inclusion is almost sacrosanct in Australian culture. Yet, the working and living conditions of Australia's low paid workers tells a different story. While labour market participation is a cornerstone of the Federal Government's Social Inclusion Agenda, it is recognized that low pay and poor job quality often derail this key objective. Indeed, the difficulty of ‘making work pay’ at the bottom end of the labour market is a social inclusion challenge that has yet to receive the policy or public attention that it warrants. This article considers what the Fair Work Act means for the social inclusion of disadvantaged workers, in the context of wider‐ranging strategies to address low pay. It concludes with some considerations of the challenges that a carbon‐constrained economy is likely to present for labour force participation as a remedy for social exclusion. 相似文献

One of the biggest challenges currently facing European society is the dramatically high level of youth unemployment. Commonly, political solutions and strategies can be found in those countries that have been able to keep youth unemployment low in spite of the financial and economic crises. Austria is such a case. On the basis of European Union Member State data, the article gives a multifactorial explanation of youth unemployment and asks whether these factors can explain relatively low youth unemployment in Austria. With the country's “youth safety net” presented in detail, it is shown that active labour market policy reduces youth unemployment in Austria. The article also points out the limitations of cross‐country comparisons of youth unemployment rates and proposes the use of a greater number of indicators. Finally, the article argues for economic policies to stimulate demand, which have to be based on a political and social commitment to full employment. 相似文献

This study examined relationships between social networks and immigration stress among first‐generation Chinese immigrants. Using data from a larger study of health behavior among first‐generation Mandarin/English‐speaking immigrants residing in the Los Angeles metropolitan area (N = 1,183), this study found that Chinese immigrants living closer to immediate family and maintaining larger social networks experienced lower immigration stress. Unexpectedly, immigrants with larger family sizes and who participated in voluntary associations (e.g., religious, alumni, and nationality associations) reported increased immigration stress. The findings suggest that practitioners need to be cautious of a possible downside in designing interventions to expand social networks among immigrant clients. The study is especially important in the context of a rapidly increasing immigrant population from Mainland China to the USA. Key Practitioner Message: ● Working with immigration families should incorporate assessment of their social network; ● Interventions designed to facilitate supportive social networks should differentiate different social network ties; ● Different social network ties may affect the stress level of immigrants differently. 相似文献

En collaboration avec des experts judiciaires, ils ont conceptualisé le harcelement sexuel et en ont fait une question de droits de la personne. Notre recherche, fondée sur un sondage effectue en 1992 auprès de 1 990 femmes canadiennes, réVéle que le harcèlement sexuel se produit aussi dans les lieux publics. Nous présentons des données inédites sur le sujet en exa-minant le type, la fréquence et la gravité du harcèlement, les carac-téristiques des femmes le plus susceptibles d'être harcetées, les implications pouvant servir à bâtir une théone sur le harcèlement dans les lieux publics, les réactions des femmes a l'égard du harcèlement ainsi que ses conséquences sur les plans émotionnel et psychologique. Researchers have expended much effort studying sexual harassment in the workplace and universities. Together with legal experts, they have conceptualized sexual harassment as a human-rights issue: a form of sexual discrimination that typically occurs in relationships of unequal power. Our research, based on a 1992 survey o{ 1,990 Canadian women, suggests sexual harassment is also prevalent in public places such as streets, transit systems and malls. We supply rare data about sexual harassment in public places by examining the types, frequency, and severity of harassment; the characteristics of women most likely to be harassed; the theoretical implications of public harassment; women's responses to harassment; and its emotional and psychological effects. 相似文献
