From Despair to Integrity: Using Narrative Therapy for Older Individuals in Erikson’s Last Stage of Identity Development

Adults aged 65 and over are a growing population in the United States today. This population is underrepresented in the mental health literature despite the high rates of depression and suicide. Additionally, the newest generation of older individuals is more likely to seek therapy than past generations, furthering the need for mental health professionals to be prepared for treating older individuals. Erikson in Childhood and society, Norton, New York, (1950) describes this time period as being critical in terms of the final identity crisis, integrity versus despair. Integrity is marked by a positive evaluation of the individual’s entire life, less anxiety about death, and a feeling of gaining wisdom. Individuals who do not resolve this crisis can manifest despair in a number of ways, including depression, anger, and regret. This model proposes utilizing Narrative therapy (White in Maps of narrative practice, Norton, New York, 2007) to understand how elderly individuals evaluate their lives in reference to their environment. The model utilizes externalization, unique outcomes, and re-membering conversations to unlock subjugated stories and promote integrity.     

The Queer Utility of Narrative Case Studies for Clinical Social Work Research and Practice

The author uses a queer critical lens to analyze the marginalization of case study research in the evidence-based practice model of clinical social work. He argues that narrative case studies are necessary for the production of context-dependent knowledge about social work practice and asserts that the method offers particular utility in conducting queer inquiry. The method is explored in relation to its capacity to describe non-normative phenomena and the impact of normative cultural pressures on queer subjects. An abbreviated case study by the author is included to demonstrate these capacities.     

A Case for the Case Study: How and Why They Matter

In this special issue we have asked the contributors to make a case for the case study. The guest editors, Jeffrey Longhofer, Jerry Floersch and Eric Hartmann, intergrate ideas from across the disciplines to explore the complexties of case study methods and theory. In education, Gary Thomas explores the importance of ethnographic case studies in understanding the relationships among schools, teachers, and students. Lance Dodes and Josh Dodes use the case study to articulate a psychoanalytic approach to addiction. In policy and generalist practice, Nancy Cartwright and Jeremy Hardie elaborate a model for a case-by-case approach to prediction and the swampy ground prediction serves up to practitioners. Christian Salas and Oliver Turnbull persuasively write about the role of the case study in neuro-psychoanalysis and illustrate it with a case vignette. In political science, Sanford Schram argues for a bottom up and ethnographic approach to studying policy implementation by describing a case of a home ownership program in Philadelphia. Eric Hartman queers the case study by articulating its role in deconstructing normative explanations of sexuality. In applied psychology, Daniel Fishman describes a comprehensive applied psychology perspective on the paradigmatic case study. Richard Miller and Miriam Jaffe offer us important ways of thinking about writing the case study and the use of multi-media. Each contributor brings a unique perspective to the use of the case study in their field, yet they share practical and philosophical assumptions.     

Staff Member Reactions to Same-Gender,Resident-to-Resident Sexual Behavior Within Long-Term Care Facilities

The current study assesses ageism and heterosexism relating to older adult sexual activity within long-term care facilities. To assess caregiver reactions, 153 residential care facility staff members read one of three vignettes. Each vignette described a scenario in which a staff member walks in on two residents (male/female, male/male, or female/female) engaging in sexual activity. Although no main effects were discovered for vignette type, exploratory analyses revealed that the facility where participants were employed was significantly related to their ratings of approval. Furthermore, an interaction effect between vignette and facility types was also discovered for caregivers’ approval of sexual activity among residents. Additionally, a strong overall approval rating of older adult sexuality was reported by staff members. The results of this study warrant that further research is necessary regarding older adults’ perception of caregiver bias, as well as further investigation of caregivers’ perceptions of older adults’ sexual activity.     

Rethinking Legitimacy and Illegitimacy in Violent Political Conflict

Analytic and conceptual ambiguity associated with the study of legitimacy have motivated debates over the utility of this construct in the analysis of violent political conflict. Using these debates as a point of departure, I review several challenges that have limited the study of legitimacy in this area and discuss two features of legitimation that have gone widely overlooked: (i) the asymmetric causes and effects of legitimacy and illegitimacy, and (ii) the relational dynamics of legitimation. I argue that turning explicit analytic attention to the asymmetry and relationality of legitimacy and illegitimacy allows us to overcome the most intractable challenges associated with the analysis of legitimation in violent conflict.     

Returnees' Perspectives on Their Re‐migration Processes

The return of refugees and migrants back to their country of origin is an important topic on the agenda of Western European governments, as return is considered as the most “durable solution” for the “refugee problem”, and as an instrument with which to tackle “illegal” migration. However, these migration policies generally lack a clear evidence base, as little studies have focused on returnees' current living situations and on their perspectives on the re‐migration process. In this paper we therefore try to listen to returnees' voices, through in‐depth interviews with four Nepalese migrants both before (in Belgium) and after (in Nepal) their return, and with 16 returnees after their return to Nepal. The interviews show how most returnees start with a disadvantageous “point of departure” to realize a “successful” return: mostly, they do not really depart “voluntarily”, and they only have limited possibilities for preparing their return and setting realistic expectations. But also, back in the “home country”, most returnees judge their current economic, social and political living situation as bad, meeting little of the expectations that they set before they returned. The participants consider the support they received through the NGOs' return programmes as minimal, because they are mostly limited to a small amount of financial support, and thus of little significance in these returnees' efforts to rebuild their lives in their “home” country. If return programmes want to make a difference in returnees' lives, they should have two extensive components in the “home” and the “host” country, incorporating in both components an integral approach, including economic, political, social and psychological aspects. Viewing these findings, it is not surprising that most interviewees eventually evaluate their return as unsuccessful, and many returnees consider re‐emigration, all of which clearly questions the current basis of worldwide migration policies.     

Short Break and Respite Services for Disabled Children in England: Comparing Children's and Parents' Perspectives of Their Impact on Children

Researchers, health and social care workers often seek to understand the perspectives of children; but gathering views directly from children can present difficulties. Parents are often asked to provide accounts of children's feelings or opinions on the assumption that their proxy reports are accurate and unproblematic. This qualitative thematic analysis of open‐question responses from 352 parents and 73 disabled children examines their accounts of the impact of short break services on disabled children. Participants' perspectives differed; children tended to describe immediate outcomes such as enjoying activities and participation; parents acknowledged these, but focused on longer term developmental outcomes for children.     

Risk Factors for Homelessness Among Lesbian, Gay, and Bisexual Youths: A Developmental Milestone Approach

Lesbian, gay, and bisexual (LGB) youths are over-represented in the homeless population. To examine why some LGB youths become homeless, this report compares homeless and non-homeless LGB youths. Of the 156 LGB youths, 48% reported ever being homeless (i.e., running away or being evicted from home). Results indicate that sexual orientation awareness and the initiation of sexual behavior occurred earlier in homeless than in non-homeless LGB youths and predated the first homeless episode. Substance use was more frequent and first occurred at an earlier age in homeless as compared to non-homeless LGB youths; however, substance use occurred subsequent to first homelessness. Childhood sexual abuse was associated with homelessness; and, early sexual orientation development was related to homelessness among youths without a history of sexual abuse. Findings suggest that interventions should help youths cope with their unfolding sexual orientation and work to prevent or address the consequences of sexual abuse.
