One of the main challenges facing medication adherence research is to provide a framework that allows patients and practitioners to effectively use the many efficacious treatments that will undoubtedly be developed in the new millennium. This framework, especially for chronic disorders, must be based on requiring patients to make the long-term behavioral changes needed to achieve a high level of commitment to these treatments. The best way to generate this high level of commitment is through a paradigm shift away from medical and patient models toward models that incorporate the social psychological literature focusing on how people's sense of self is constructed, enacted, and sustained over the life course. This review demonstrates the need, value, and appearance of this shift. First, it describes a previous paradigm shift in the medication adherence literature and then argues that a new paradigm shift is necessary. Second, it identifies and describes the models currently used in adherence research and notes three critical shortcomings of these models. Finally, it demonstrates how symbolic interaction's identity theory may be used creatively to overcome these shortcomings and convert efficacious treatments into effective treatments. Developing such an approach would well position the medication adherence research field for excellent contributions to the treatment of chronic disorders in the twenty-first century.  相似文献

Analysis of a nationally representative, longitudinal survey of community-dwelling Americans older than 50 years of age indicated that rating the physical condition of one's dwelling unit as “poor” predicted significantly more rapid cognitive decline in subsequent years. This relationship persisted after controlling for a variety of factors such as wealth, income, education, health, family status, neighborhood safety, depression, and initial cognitive ability. Dissatisfaction with the physical conditions of one's housing may have a direct effect on the rate of cognitive decline in older adults. Addressing housing inadequacy for older adults may thus produce a wider range of societal benefits than previously realized.  相似文献

Abstract

Purpose: Low rates of human papillomavirus (HPV) vaccination among young Asian American and Pacific Islander (AAPI) women need to be addressed, particularly given the high incidence of cervical cancer in this population. The current study aims to investigate predictors of HPV vaccination in young AAPI and non-Latina white (NLW) women. Methods: A secondary data analysis was conducted of a health survey administered to college-aged women (N = 2,270) at a large public university in the Midwest. Andersen's behavioral model of health services utilization guided the study theoretically, and hierarchical logistic regression analysis was conducted to investigate research aim. Results: NLW women had a significantly higher rate of vaccine completion than AAPI women (NLW = 60.7%; AAPI = 38.6%). NLW women also scored higher on all 5 measures of HPV vaccine literacy than AAPI women. Both groups of undergraduate women reported that increasing age, knowledge about HPV, greater use of gynecological services, and knowing someone who had cancer were significant factors related to HPV vaccination completion. In the NLW group, father's income was also found to be a predictor of HPV vaccination completion. Conclusions: Rate and predictors of HPV vaccination completion vary between NLW and AAPI women. Greater promotion of HPV literacy and gynecological service use should be made to young AAPI women to increase their HPV uptake. Further research should examine sociocultural factors that could hinder or promote HPV vaccination in young AAPI women.  相似文献

Abstract

Objective: Content analysis of Internet-based diabetes forum text was used to examine the experiences encountered by students with diabetes transitioning into and through college. Participants: Forum posts (N = 238) regarding attending college with diabetes were collected and analyzed. Methods: Thematic coding was used to identify prominent topics, followed by analysis of theme distribution across college transitional stages. Three students with diabetes were recruited to review results and corroborate findings. Results: Twenty thematic categories were identified. Preparation for college involved efforts to move toward autonomous diabetes management. Transitioning in was marked by adjusting to a college lifestyle, then working to manage issues such as busy schedules and alcohol use as continuing students, and turning attention toward future career options and finances while transitioning out. Conclusions: As they move into and through college, students with diabetes negotiate developmental and diabetes-specific tasks within an environment that presents unique logistical, lifestyle, and psychological challenges.  相似文献

This paper provides a systematic review of education literature focused on identifying school-based approaches for developing students’ intercultural understanding. Studies were assessed using selection criteria and then critically appraised for study quality. A key finding from the review is that developing students’ intercultural understanding beyond cultural awareness requires students and teachers to take a critical approach toward cultural diversity, as well as the opportunity for ongoing intercultural and intergroup contact. Studies reported that only building cultural awareness and knowledge is not enough to promote long-term changes in attitudes. There is a need for more rigorously evaluated longitudinal school-based interventions. Finally, studies consistently call for investment in teachers’ professional and personal intercultural capabilities. The paper concludes by calling for school-based interventions that are informed by best practice approaches at a whole school level in order to effectively develop students’ intercultural attitudes and skills.  相似文献

Case study research in the fields of Industrial Relations and Sociology of Work has always been including the use of multiple methods and perspectives. However, there is little methodological reflection on the specific challenges of combining different forms of data, methods, theories and researchers within a single study. Preparing students with learning disabilities (LDs) to make the transition into the world of work is considered an essential preparation that high schools can provide. However, existing services are limited for career development preparation, and available programs rely on assessments normed for samples of students without LDs. This study examined the predictability of critical career‐related constructs of dysfunctional career thoughts, career maturity, and vocational identity in high school students with LDs, using a sample of 139 such students. Data analyses were performed using multiple regression and t tests. Results indicate that it is possible to predict important career constructs for students with LDs using standardized instruments, which can inform subsequent interventions.  相似文献
