Hoarding interventions with older adults require significant resources from multiple public agencies, yet recidivism occurs frequently. To improve services through better coordination, some communities have formed multiagency hoarding teams (MAHT), which include aging services. MAHTs requested this mixed methods study to understand the progression of cases through the public sector. Quantitative data collected on 52 cases involving adults ages 60+ identified steps in this process. Qualitative data collected from MAHT members were the basis for case studies illustrating the progression of cases through the public sector. Findings have implications for social workers involved in local service coordination, training, and policy.  相似文献

Multiple testing procedures defined by directed, weighted graphs have recently been proposed as an intuitive visual tool for constructing multiple testing strategies that reflect the often complex contextual relations between hypotheses in clinical trials. Many well‐known sequentially rejective tests, such as (parallel) gatekeeping tests or hierarchical testing procedures are special cases of the graph based tests. We generalize these graph‐based multiple testing procedures to adaptive trial designs with an interim analysis. These designs permit mid‐trial design modifications based on unblinded interim data as well as external information, while providing strong family wise error rate control. To maintain the familywise error rate, it is not required to prespecify the adaption rule in detail. Because the adaptive test does not require knowledge of the multivariate distribution of test statistics, it is applicable in a wide range of scenarios including trials with multiple treatment comparisons, endpoints or subgroups, or combinations thereof. Examples of adaptations are dropping of treatment arms, selection of subpopulations, and sample size reassessment. If, in the interim analysis, it is decided to continue the trial as planned, the adaptive test reduces to the originally planned multiple testing procedure. Only if adaptations are actually implemented, an adjusted test needs to be applied. The procedure is illustrated with a case study and its operating characteristics are investigated by simulations. Flexible designs offer a large amount of flexibility in clinical trials with control of the type I error rate. This allows the combination of trials from different clinical phases of a drug development process. Such combinations require designs where hypotheses are selected and/or added at interim analysis without knowing the selection rule in advance so that both flexibility and multiplicity issues arise. The paper reviews the basic principles and some of the common methods for reaching flexibility while controlling the family-wise error rate in the strong sense. Flexible designs have been criticized because they may lead to different weights for the patients from the different stages when reassessing sample sizes. Analyzing the data in a conventional way avoids such unequal weighting but may inflate the multiple type I error rate. In cases where the conditional type I error rates of the new design (and conventional analysis) are below the conditional type I error rates of the initial design the conventional analysis may, however, be done without inflating the type I error rate. Focusing on a parallel group design with two treatments and a common control, we use this principle to investigate when we can select one treatment, reassess sample sizes and test the corresponding null hypotheses by the conventional level alpha z-test without compromising on the multiple type I error rate.  相似文献

A key aspect of civil society worldwide is the emergence of thousands of nongovernmental organizations that operate on a global scale. The special challenges of organizing and managing these organizations include massive communications problems and the need to accommodate a wide diversity of interests. In this paper, we ask what kinds of organizational structures and management strategies are utilized by globally oriented nongovernmental organizations involved in the development of civil society, and we consider the advantages and disadvantages of alternative structures. From 15 case studies, we find that three principal types of structures are utilized: corporate partnerships, federations, and membership associations. We also find that management challenges are addressed in various creative ways within these structures, and that the federation form appears to be generally effective and avoids some of the risks associated with other forms.  相似文献

Older adults, as active members of the Veterans of Foreign Wars (VFW), describe multiple benefits of participation in the organization to their overall well-being. This qualitative study examined the perspectives of 20 active participants of a VFW Post and its Ladies Auxiliary. Findings indicate that, for the study participants, the VFW serves as a primary source of cross-generational influence, emotional support, and meaningful activity and provides a safe and secure environment for members. This article provides valuable information that social work researchers and practitioners can use to help increase older adults' civic engagement and address service gaps.  相似文献

This qualitative study examined ethical dilemmas faced by female caregivers of frail elders as well as the dominant role of caregivers' spirituality in addressing these dilemmas. Dilemmas are difficult decisions that involve conflicting values, e.g., freedom versus safety. In-depth interviews were conducted with thirteen ethnically diverse caregivers recruited from a home health agency and its parent hospital. Purposive sampling was used to obtain variation among research participants. Focus group interviews of home health staff, key informant caregivers, and interviewees provided guidance for the research design, reflection on findings and development of implications. In order to deal with ethical dilemmas, all caregivers used spirituality as (1) a philosophy of life, e.g., "This is what you do when you're family," (2) an aid to decision-making, e.g., through the use of prayer; and/or, (3) a way to transcend dilemmas, e.g., "no choice is hard." Implications include the importance of caregiver-driven assessment, professional self-reflection, and sustained formal services for caregivers.  相似文献
