Interviewing Elderly Residents in Assisted Living

The interpretation of interview data requires an understanding of its context. In this study, the context was Assisted Living (AL). Twenty-one interviews were conducted with elderly residents of 4 Midwestern Assisted Living Facilities (ALFs). Institutional fear and loneliness associated with ALF living framed residents’ interpretation of and responses to the interview questions. Residents feared transfer to a nursing home, and they experienced the loneliness of being cut off from their homes and put into the care of busy staff members. Their framing of and responses to questions (both qualitative and quantitative) reflected this context, and included the invocation of impairments, the reluctance to criticize staff, and the refusal to complain about the ALF or their present situation.

Preadoption Genetic Testing: Social workers' decision-making process

Abstract

Genetic testing requested by prospective adoptive parents and/or an adoption agency prior to adoption, known as pre-adoption genetic testing, is postulated to help identify the best placement for the child. However, limited data exist about factors impacting social workers’ decision making about genetic testing and their perceptions about positive and negative implications of testing. A survey and semi-structured interview based on a pre-adoption genetic testing scenario of an inherited condition explored social workers’ perspectives and decision-making process (N?=?129). The majority reported limited education in genetics, but favored genetic testing. Results highlight the value of an interdisciplinary team in genetic testing decision making.

Voluntary work in the Norwegian long-term care sector: complementing or substituting formal services?

ABSTRACT

Across Europe, governments call for increased involvement of volunteers to shoulder some of the welfare burden. Nevertheless, there is little research into what kind of work and how much volunteers currently contribute in the long-term care services and whether this has the potential to substitute formal services. Drawing on findings from a survey of employees in Norwegian nursing homes and home care districts, we examine the nature and volume of voluntary, unpaid work in the long-term care services in Norway. Our data suggest that volunteers to a very limited degree carry out work that has traditionally been considered the formal system’s domain: personal care and practical help. Nearly all the voluntary, unpaid contributions in our data takes place within cultural, social and other activities aimed at promoting mental stimulation and well-being, indicating a classic specialisation of tasks between volunteers and professionals. However, there has been an expansion of the formal care system to include activities aimed at promoting well-being in recent decades. This may indicate that there is a certain level of task sharing between voluntary and formal care. Thus, social workers need to consider voluntary service provision when assessing the needs of clients.     

POPULATION DENSITY, ELITES, AND THE DISTRIBUTION OF INFRASTRUCTURAL RESOURCES IN THAILAND

Literature on population density and infrastructure has generally used an ecological/economic model to identify the determinants of the distribution of infrastructural resources. While this approach has been fruitful, a political-economic approach suggests the importance of an alternative set of determinants of resource distribution: the needs of elites. This paper operationalizes both political-economic and ecological/economic explanations and compares their efficacy in predicting infrastructural development in the provinces of Thailand. Overall, our data suggest that the distribution of infrastructural resources is primarily a function of two ecological-economic variables—gross provincial product and density, and the political-economic factors of presence of governmental and non-governmental elites in the province.     

Comparison of response adaptive randomization features in multiarm clinical trials with control

We investigate multiple features of response adaptive randomization (RAR) in the context of a multiple arm randomized trial with control, where the primary goal is the identification of the best arm for use in a broader patient population. We maintain constant control allocation and vary the length of time until RAR is started, interim frequency, the underlying quantity used to calculate the randomization probabilities, and a threshold resulting in temporary arm dropping. We evaluate the designs on five metrics measuring benefit to the internal trial population, the future external population, and statistical estimation. Our results indicate these features have minimal interaction within the space explored, with preference for earlier activation of RAR, more frequent interim analyses, randomizing in proportion to the probability each arm is the best, and aggressive thresholding for temporarily dropping arms. The results illustrate useful principles for maximizing the benefit of RAR in practice.
