Many authors have suggested that administrative health data files could provide the solution to the well-known research problem of obtaining data about volunteer subjects. However, many authors have failed to achieve satisfactory results because the use of these data files leads to special difficulties. Thus, unless the proper methodology is applied, use of health data files does not produce valuable results. In this paper we propose a conceptual framework which can be used to examine the suitability of health data files. More specifically, five aspects are discussed: (1) research team knowledge and skills; (2) identification and documentation of an accessible file; (3) file adequacy to answer the research question; (4) data quality; and (5) data processing. This conceptual framework should help health policy planners and researchers to better identify the problems they will meet and to find adequate solutions to these problems before embarking on the venture of using public data files. 相似文献

Recent changes in life expectancy among race and sex groups in New York City were evaluated by analyzing the relative effects of different causes of death in 1983 and 1992, a period in which life expectancy at birth declined by 1.1 years among white males, remained unchanged among black males, and increased 1.2 years among white and black females. Heart disease was found to be the leading cause of death making positive contributions to changes in life expectancy regardless of race or sex, and HIV/AIDS was the leading negative contributor. Overall, deaths from infectious diseases and external causes are becoming more important compared to degenerative conditions in explaining trends in life expectancy in New York City. Past improvements in survival due to reductions in infant deaths are being reversed due to an increase in deaths from preventable causes such as violence and AIDS. Future gains in longevity may require a greater emphasis on policies and programs emphasizing conflict resolution and HIV prevention. 相似文献

A decade has passed since family group conferences were initially introduced into the UK by Family Rights Group. Ten years on, this paper examines the extent to which family group conferences have developed and become embedded into current social work practice. Despite the initial interest by social work practitioners and the picture often painted of a growing radical movement, the degree to which family group conferencing has become part of mainstream practice has until now remained fairly anecdotal. A number of difficulties have been identified with implementing the model, including fitting it into an existing system and the challenge it poses to professionals to hand over power. Two surveys, the first undertaken in 1999 and the second in 2001, describe the current use of the model in the UK by Councils with Social Services Responsibilities (Councils). The surveys reveal the areas of practice within which family group conferences are being used, the size and capacity of projects and why some Councils have adopted the model whilst others remain hesitant. It concludes by considering why family group conferences remain on the margins of practice. 相似文献

This article critically assesses the marketization of income security, and identifies links between equality, self‐reliance and welfare reform. Marketization in emergent economies is distinguished by a strong separation between the use of insurance for the mainstream economy and relief for the poor. The impact of this model on the shaping of working lives and on market rigidity is discussed through a review of implications in the areas of subsistence, integrity interests and employment transitions. The broad faith in insurance solutions is argued to derive from a highly abstract approach to welfare reform and to result in a lack of attention to uneven and unstable markets, and to self‐government as a motive to work. Evidence of this emerges from a comparison of insurance in its more ideal form (in Chile) with modified models (Brazil and Korea). In the last two cases a developmental orientation has aided in the provision of broad‐based security. Other factors that appear to enhance the importance of direct assistance are also discussed, including aspects of state administration and labour services that limit work opportunity and individual autonomy in uneven economies. The segregated dual approach to income security is argued to be broadly deficient, but not because insurance is inherently wrong. Countries as diverse as Barbados and Denmark show that more cohesive economies are a better foundation for integrating insurance with general welfare and for income security and individual enterprise broadly conceived. 相似文献

We explored whether emotion understanding promotes positive social functioning in childhood using the ability emotional intelligence (AEI) framework, which defines emotion understanding more broadly than is common in developmental science. The prospective study included children ages 9–11 years who completed a measure of AEI at the start of the school year, and whose playground interactions were observed for one full year. Findings showed that, among girls, low AEI was associated with higher levels of direct aggressive behaviour in the playground; boys and girls high or low in AEI were more likely than their peers to watch others during playground social interactions. Further, higher AEI was associated with indirect aggression in school, suggesting higher AEI during childhood may be associated with the developmental transition from direct to indirect forms of aggression. The implications of the findings for school practice in relation to the teaching of emotion understanding are discussed. 相似文献

The capacity to manage emotional reactions effectively, frequently in complex care settings, is central to the role of a social worker. Nonetheless, there is evidence that social work students frequently find their placements emotionally demanding and stressful. It is proposed that emotional intelligence may help students manage their emotional reactions more effectively during placements and their subsequent career. To date, however, little systematic research has explored whether emotional intelligence and associated competencies can be enhanced during social work training and the implications for wellbeing. This paper presents a mixed-methods two-stage study which aimed to increase emotional competencies in social work students during the first year of training. More specifically, it assesses the impact of a workshop designed to enhance emotional competencies and an emotional writing task on levels of emotional intelligence, reflective ability and empathy which were assessed via questionnaire and reflective logs pre- and post-intervention (Times 1 and 2). Levels of reflective ability and empathy increased significantly between Times 1 and 2 and psychological distress decreased. Content analysis of reflective logs found evidence that reflective ability, empathy and emotional intelligence were enhanced following the interventions. The implications of the findings for the development of the curriculum are considered. 相似文献

Neo‐liberalism represents a significant and enduring shift in the politics shaping social policy. Although frequently ascribed a hegemonic, all‐powerful status that focuses our attention on the coherence found in neo‐liberal policies, this article builds on scholarly work highlighting variegation in the neo‐liberal project across different policy areas, national settings and time periods. Specifically, it employs Peck's and Tickell's (2002) view that neo‐liberalism has gone through multiple phases in response to both external and internal crises as an entry point for studying neo‐liberalism's impact on public support for the welfare state. Drawing upon New Zealand and British attitudinal data, the article argues that public reactions to an early period of retrenchment (‘roll‐back’ neo‐liberalism) differ from those reported in the ‘roll‐out’ or embedding phase of neo‐liberalism implemented by Third Way Labour Governments in both countries. Indeed, continuing public support in many policy areas arguably contributed to the internal crisis that provoked an adaptation to the neo‐liberal project. The article further explores public support for the welfare state following the external crisis provoked by the financial meltdown of 2008–09 asking whether New Zealand and British attitudes showed signs of resisting austerity measures or whether they, instead, indicated a third, ‘roll‐over’ period of neo‐liberalism where the public accepted not only a neo‐liberal economic agenda but also the need for further retrenchment of the welfare state. Conclusions about the politics of social policy at the level of public opinion offer both good and bad news for welfare state advocates. 相似文献
