Objective : To investigate parents' experiences of surveillance for early talking difficulties. Design : Qualitative study nested within a randomised controlled trial. Setting : Interviews with the parents of 20 children identified as having early difficulties. Results : Most parents were in favour of surveillance of children's language. Parents do not totally welcome surveillance, however, as it also potentially medicalises their children's early lives. The study also revealed that many of the parents felt stigmatised by their children's difficulties. Conclusions : Listening to parents' accounts can increase professionals' understanding of their responses to monitoring of their child's development and referral to specialist services. 相似文献

The main effects of four domains of control (task, decision, resource, physical environment) on job satisfaction and their interactions with three types of job stressors (task demands, role conflict, interpersonal conflict) were examined in a sample of 765 Canadian nurses. Multiple regression analyses were conducted to determine the proportion of variance in job satisfaction that could be accounted for by job stressors, control, and the interaction of stressors and control in that order. The job stressors were analysed separately and as a composite job demands score, as were the four domains of control. The results indicated that the proportion of variance in job satisfaction increased with the addition of control for the composite scores and for task, resource, and physical environment control. However, decision control did not account for additional variance in job satisfaction above that for any of the job stressors. In addition, none of the stressor x control interactions were significant. The results support previous attempts to test the moderating effects of control, suggesting that perceived control is not a moderator of job stress but rather that it has a direct influence. The current approach to health risk assessment of toxic waste sites in the U.S. may lead to considerable expenditure of resources without any meaningful reduction in population exposure. Risk assessment methods used generally ignore background exposures and consider only incremental risk estimates for maximally exposed individuals. Such risk estimates do not address true public health risks to which background exposures also contribute. The purpose of this paper is to recommend a new approach to risk assessment and risk management concerning toxic waste sites. Under this new approach, which we have called public health risk assessment, chemical substances would be classified into a level of concern based on the potential health risks associated with typical national and regional background exposures. Site assessment would then be based on the level of concern for the particular pollutants involved and the potential contribution of site contaminants to typical background human exposures. While various problems can be foreseen with this approach, the key advantage is that resources would be allocated to reduce the most important sources of human exposure, and site remediation decisions could be simplified by focussing on exposure assessment rather than questionable risk extrapolations. 相似文献

Gender differences in rural eighth-grade students' curricular, career, and lifestyle expectations were examined. Girls expect to take more math classes in high school, select careers that require a college education, and plan to go to college more frequently than do boys. Boys and girls do not differ in their interest in various careers, although girls prefer occupations that involve people, whereas boys prefer occupations that involve things. Girls plan to work for as many years of their lives as do boys; however, girls are more likely to expect to work part-time, whereas boys expect to work full-time.
