Although symbolic interactionist studies are often thought to abandon motivational explanation in favor of a focus on social process, the interactionist tradition has usefully employed two deterministic themes. One is inherent in the appreciation of the actor's inescapable sense of interactional significance for behavior that will be socially witnessed. A second deterministic theme appreciates external constraints on the individual's perception and generation of social acts. The former shapes all work in symbolic interactionism. The latter has been developed most consistently in the work of Howard S. Becker. Without addressing the matter as such, Becker has worked with a series of ideas about constraint on individual motivation, ideas that are harmonious with the interactionist's understanding of personal creativity in social action. His writings invite a reconsideration of the supposed antagonism between the appreciation of interactive process and the understanding of motivational constraint. Becker's work on motivation is also notable for outlining a theory of increasing constraints through the life cycle, constraints which, if appreciated by the actor, carry paradoxically liberating implications. The distinctive interplay of personal creativity and collective constraint in his thought is succinctly captured by the metaphor of jazz in social interaction. 相似文献

In a longitudinal study, the substance use/abuse histories were obtained on a community sample of 640, (mean age of 24.9 years) randomly selected from the 8,000 Philadelphia black subjects who had been studied comprehensively from birth to seven years of age in the National Collaborative Perinatal Project (NCPP). Among other conclusions, it is postulated from some of the many significant associations found between early life variables and substance use/abuse in early adulthood, that an infant is at risk who has the following combination of characteristics and family situation: outgoing, responsive, assertive or impulsive, or willful, and who is in an unfavorable family environment, with a mother who is generally negative toward the infant, has a larger number of small children and has had more fetal deaths, in a generally poor social environment. 相似文献

The physician can play an important role in managing high-risk nursing home residents without restraints and working with interdisciplinary care teams in comprehensive fall evaluations. A reduction or elimination of physical restraints can be measured for a facility over time, and it represents a relevant quality indicator of physician and facility interactions during the process of care. We explore the relationship between economic volatility and human capital formation within a context of migration. We show
that increased variability may raise the average educational level of the population even after netting out expected migration.
In particular, we demonstrate that this is the case when individuals' abilities and the economy-wide shock are uniformly distributed.

Background and purpose. The number of octogenarians requiring surgery increases constantly. Data regarding perioperative morbidity and mortality in octogenarians is limited. Our aim was to assess surgery-related complications in octogenarians, undergoing urological surgery.Patients and methods. We prospectively collected data from octogenarians and a control group of younger patients undergoing an elective urological surgery in our institution. Recorded data included: age, gender, American Society of Anesthesiologists (ASA) score, co-morbidities, number of medications, operation extent, anaesthesia type, surgery duration and perioperative morbidity and mortality.Results. Forty-seven octogenarians and 80 patients with a median age of 59 years (range 19–75) enrolled prospectively. Gender ratio, surgeries extent and median operative time were similar among groups. General anaesthesia was more prevalent in the control group. ASA classification and duration of hospitalization were significantly higher in octogenarians. The rate of intra-operative complications was significantly higher in the octogenarians group 6.38% versus 3.75% (p = 0.007), there was no significant difference in immediate post-operative and post-discharge complications among groups. One octogenarian patient died 2 days post-surgery, no death occurred in the control group.Conclusions. Octogenarians have higher rate of intra-operative morbidity, leading to longer hospital stay. More experienced surgeons and anaesthetists should be involved in the operation; and careful surgical technique, tapered anaesthesia and higher level of post-operative monitoring should be applied for patients in this age. 相似文献

This paper examines how the relationship between parents' educational achievement (a marker of their socio‐economic status) and children's early developmental outcomes has evolved in Australia since the early 1980s. The specific focus of this paper is whether the gradient in children's early developmental outcomes by parents' education has changed since the 1980s. A comparative analysis of two surveys is undertaken that follows Australian cohorts of children through their early years – the Australian Temperament Project (following children born in Victoria in the early 1980s) and the Longitudinal Study of Australian Children (following a representative sample of children born in Australia in 1999). The analysis shows that the relationship between parental education and children's early developmental outcomes does not in general appear to have changed greatly over the years. The gradient associated with behaviour difficulties, persistence in behaviour difficulties over time, and in reading skills has either remained the same or strengthened somewhat, while the gradient associated with social skills has weakened. The paper concludes with a discussion of issues that might explain these trends. 相似文献
