The Family Health Program (Programa Saúde da Família) is an initiative of the Brazilian Ministry of Health designed to deliver free primary health care services within communities and households. The program was implemented by municipalities in different periods of time, creating variation in its availability among siblings of different ages. The empirical approach uses this variation to estimate the effect of the program on children’s health in Brazil, in an attempt to control for family and municipality unobserved factors possibly related to the program’s adoption. The results indicate that children for whom the Family Health Program was available in their municipalities during the prenatal period are healthier than children for whom the program was not available during the same period of their lives.  相似文献

Crowdfunding, the practice of asking for money from others using the Internet, is a major private means through which Canadians are funding their health care and education. Crowdfunding has proliferated in Canada during the 2010s and continues to grow, approaching the revenues of Canada's major traditional charities. Proponents describe it as an empowering practice from which anyone can benefit. If its gains are inequitably distributed, however, increasing reliance on this private funding mechanism, especially in core areas of welfare state provision, can further exacerbate inequalities of opportunity and income. This study asks why Canadians turn to health care and education crowdfunding and how equitably funds are raised using this novel method. Based on a mixed methods analysis of 319 campaigns conducted on two prominent crowdfunding platforms between 2012 and 2014, we find that crowdfunding users’ needs frequently correspond to known gaps in the contemporary social safety net, including in the area of cancer care, and that campaigns for older and visible minority Canadians face a disadvantage. We argue that health care and education crowdfunding is a response to the shortcomings of Canadian welfare state provision, but one that reproduces offline inequalities with potentially perilous consequences for democratic life and individual suffering.  相似文献

On the occasion of the 40th anniversary of Angola’s independence, this essay revisits key dimensions of the country’s postcolonial trajectory through the prism of the complex politics of the nationalist struggle and the first decades of self-rule. Its goal is to provide a series of reflections, mostly centred on the MPLA regime, rather than a comprehensive treatment of all political actors in Angolan politics of recent decades. It argues, firstly, that Angola’s trajectory from independence to the end of the country’s long civil war in 2002 was powerfully conditioned by the structural legacies of late colonialism, the associated intra-nationalist politics and the particular circumstances of the end of empire in 1975. To these legacies, the essay adds the (constrained, but real) choices made by Angolan decision-makers in terms of institutional consolidation, the management of the economy and state–society relations. Secondly, the essay outlines the extent to which the MPLA’s 2002 victory against UNITA in the country’s civil war did away with the fragmentation that had characterised Angolan politics since the 1960s. The clashing, indeed mutually exclusionary, nationalist projects that had jostled for control of the Angolan state were replaced by a would-be hegemonic political force with a strong sense of legitimacy and a self-defined project of postwar nation building. The key question for the contemporary study of Angola – and one that the oil-fuelled politics of the national reconstruction era provided plenty of reflection on – is the extent to which the MPLA’s postwar vision can supersede the country’s historical divisions and provide the population with both the material prosperity it yearns for and a shared understanding of belonging.  相似文献

While global polio eradication requires tremendous efforts in countries where wild polioviruses (WPVs) circulate, numerous outbreaks have occurred following WPV importation into previously polio‐free countries. Countries that have interrupted endemic WPV transmission should continue to conduct routine risk assessments and implement mitigation activities to maintain their polio‐free status as long as wild poliovirus circulates anywhere in the world. This article reviews the methods used by World Health Organization (WHO) regional offices to qualitatively assess risk of WPV outbreaks following an importation. We describe the strengths and weaknesses of various risk assessment approaches, and opportunities to harmonize approaches. These qualitative assessments broadly categorize risk as high, medium, or low using available national information related to susceptibility, the ability to rapidly detect WPV, and other population or program factors that influence transmission, which the regions characterize using polio vaccination coverage, surveillance data, and other indicators (e.g., sanitation), respectively. Data quality and adequacy represent a challenge in all regions. WHO regions differ with respect to the methods, processes, cut‐off values, and weighting used, which limits comparisons of risk assessment results among regions. Ongoing evaluation of indicators within regions and further harmonization of methods between regions are needed to effectively plan risk mitigation activities in a setting of finite resources for funding and continued WPV circulation.  相似文献

This article develops a Bayesian belief network model for the prediction of accident consequences in the Tianjin port. The study starts with a statistical analysis of historical accident data of six years from 2008 to 2013. Then a Bayesian belief network is constructed to express the dependencies between the indicator variables and accident consequences. The statistics and expert knowledge are synthesized in the Bayesian belief network model to obtain the probability distribution of the consequences. By a sensitivity analysis, several indicator variables that have influence on the consequences are identified, including navigational area, ship type and time of the day. The results indicate that the consequences are most sensitive to the position where the accidents occurred, followed by time of day and ship length. The results also reflect that the navigational risk of the Tianjin port is at the acceptable level, despite that there is more room of improvement. These results can be used by the Maritime Safety Administration to take effective measures to enhance maritime safety in the Tianjin port.  相似文献

Using Blinder–Oaxaca decomposition and relying on the consistent design of the Displaced Worker Survey since 1996, this study analyses various factors contributing to the rising dislocation of older workers, such as changes in tenure, industry mix, educational attainment, and labor force participation. Although in the past older workers were less prone to displacement compared with prime‐age workers, this paper finds that older workers are now more likely to be displaced, conditional on education, manufacturing industry, and tenure. Declining tenure, a higher incidence of displacement in manufacturing, and a higher labor force participation among older workers largely explain the convergence of displacement rates among older and prime‐age workers.  相似文献

This study utilized a multidimensional measure of social welfare composed of 26 social indicators integrated in nine categories: education, employment and social protection, income, health, housing conditions, subjective wellbeing, social capital, use of technology, and culture and leisure to help understand social welfare in Mexico. We also compared the integrated measure with the Human Development Index. Estimation was performed using the method. Our analysis indicated that the health and housing conditions categories contributed the most to social welfare across the 32 Mexican States. In relation to the indicators, income and trust in other people were associated with welfare. Further, results on the welfare ranking of Mexican states revealed variations between the two indices and the HDI). Specifically, only four states occupied the same position on both indices, ten recorded different positions on moving up or down from their levels of social welfare. This note describes some of the safety and security concerns posed by the transportation of highly enriched uranium (HEU) from a nuclear research reactor located in Bogota (Colombia) to a port on the Colombian Caribbean coast. The operation was made at a moment of extremely high guerilla activity in Colombia, so that the decision-problem belongs in the extension of risk analysis known as threat analysis. By invoking a principle of dispreference for probability based on the concept of stochastic dominance, a qualitative risk comparison was made between the road and air alternatives.  相似文献
