Capacity assessment is a growing area of practice in geriatrics. In this pilot study we illustrate the potential relationship between clinician variables and capacity ratings. Twelve older adults with mild dementia or schizophrenia were rated by six clinicians from diverse disciplinary backgrounds. Capacity ratings were associated with clinician values regarding patient involvement in medical decisions, clinician-patient mismatches on such values, and emotional reactions to patients. Expanding our understanding of the impact of clinician variables on capacity ratings may enhance the reliability and validity of capacity assessments and help to promote autonomy when appropriate.  相似文献

Observers watched videotaped face‐to‐face mother–infant and stranger–infant interactions of 12 infants at 2, 4, or 6 months of age. Half of the observers saw each mother paired with her own infant and another infant of the same age (mother tapes) and half saw each infant paired with his or her mother and with a stranger (infant tapes). Observers were asked to judge which was the mother–infant dyad in each pair. Observers' accuracy improved as infants aged and was above chance for both mother and infant tapes when infants were 6 months. Differences between mother–infant and stranger–infant dyadic communication patterns also emerged as the infants aged. At 6 months, mother–infant dyads had more symmetrical communication and less asymmetrical communication than stranger–infant dyads.  相似文献

International education is a thriving industry, but relatively little is known about its consequences for the social integration of international scholars in educational institutions or the wider community. This article reviews research on attitudes toward international students as a key component of intercultural relations and a significant marker of social cohesion. An organizational framework for the investigation of these attitudes is proposed with threat mediating the influences of personal and situational factors on attitudinal outcomes. Broader contextual factors are also seen to exert influence on the process of attitude development and maintenance. A model derived from this framework is then tested with student, teacher, and community samples. A "tipping point" in attitudes toward international students as a function of the proportion of international enrolments in educational institutions is also explored. Finally, the applicability of the findings for promoting social cohesion in educational institutions and the wider community is discussed.  相似文献

ABSTRACT

A range of institutional and financial instruments has been used to drive population health outcomes in primary health care in Australia. However, GP sovereignty and the corporatized nature of general practice have generated major challenges. The core of government reform strategy since 1992 has been the creation and financing of Primary HealthCare Organizations (PHCOs), in various forms, to provide an organizational basis to connect GPs to population health performance, and a closer link between the state and GPs. The shift from Divisions of General Practice, the first PHCO, to Medicare Locals (MLs) in 2011 was notable. The latter constructed the object of performance as a raft of broader population health goals, which were framed in terms of accountability to communities through public reporting. Drawing on interviews with Federal government, health professional associations, ML executives and GPs, this paper examines the ways in which such performance instruments were imagined and understood, and areas of contestation. The findings show the different rationalities at play and how different actors seek control of the policy space. They also demonstrate the political precariousness of PHCOs, and the wider difficulty of steering market-based professionals in the achievement of population health objectives.  相似文献
