This article summarizes the Patient Protection and Affordable Care Act (ACA) provisions that have a direct or indirect impact on the workforce caring for the elder population, explores the challenges to developing the workforce, and critiques the adequacy of the ACA provisions in meeting those challenges. The ACA is the first comprehensive federal legislation to acknowledge gaps in the workforce caring for the elder population. However, its provisions are inadequate given insufficient supply in the number and types of workers necessary both to meet the caregiving demand of the growing elder population and to implement the delivery system reforms instituted by the ACA. One of the challenges is that the workforce is not prepared for the new service delivery models specified in the legislation. They are not trained, supported, or held accountable for effective care coordination and service integration, and they lack the requisite skills, knowledge, and competencies. Moreover, it is likely to remain difficult to recruit and retain competent direct care workers, who represent the largest component of the long-term care workforce, because of the negative industry image, noncompetitive wages and benefits, a challenging work environment, and inadequate education and training. This study explored how learning from mental health lived experience influenced Australian social work students’ practice during their first fieldwork placement. Involvement of mental health consumers in social work education is gaining momentum, yet little is known about how this type of learning informs students’ practice. Ten social work students participated in semistructured interviews and one focus group. Findings suggest that learning from lived experience promoted social work practice that honoured the expertise of mental health consumers and privileged personal recovery. Factors such as organisational culture and supervisor attitudes were found to mediate the students’ attempts to privilege lived experience. 相似文献

Enquiry about childhood sexual abuse (CSA) is not routine practice for mental health practitioners undertaking client assessment. This study audited 100 patient charts to determine rates of enquiry and response by mental practitioners around CSA. Fifty-seven mental health staff also completed a subsequent questionnaire exploring factors that influence practice in regard to sexual abuse. Staff lacked confidence in how to ask and effectively respond to sexual abuse disclosure. Mandatory training focusing on trauma informed care could improve staff confidence and capacity to identify and effectively address not only sexual abuse, but childhood trauma and abuse more generally. 相似文献

As tertiary music educators across the Tasman we argue that music, particularly song, is an effective medium for teaching and learning about non-western music when preparing generalist primary Pre-Service Teachers (PSTs). Using ‘voice’ as a portable and accessible vehicle to transmit cultural understandings, we draw on the Zimbabwean proverb ‘if you can speak you can learn to sing and if you can walk you can learn to dance’ to foster music creativity and enhance literacy development and confidence in our PSTs. Using narrative methodology, we share our teaching and learning experience at Deakin University (Australia) and the University of Auckland (New Zealand) where we include African and Māori music respectively as effective ways to promote cultural understandings. In our experience, the teaching of song goes beyond teaching a tune or something that is ‘fun’. Rather, it is as an effective context for developing knowledge, skills and understandings about multiculturalism and the importance and need to be ‘inclusive of others’. PSTs gained socially, linguistically, cultural and emotionally, to name a few. We encourage other music educators at all education levels to be culturally and linguistically inclusive and to explore non-western music as a positive teaching and learning experience. 相似文献

This paper explores the emotional life of fly‐in fly‐out (FIFO) workers and their families, through an analysis of more than 500 postings made on an online chat forum for mining families. Building on literature on fly‐in fly‐out workers and understandings of emotions as socially constructed, analysis shows how posters to the forum, typically women whose male partners are FIFO workers, construct gendered emotional identities for their partners (sometimes referred to as ‘Mr Miner’), and for themselves, as ‘mining women’, ‘mining widows’ or the ‘mining missus’. Inherent in the creation of gendered emotional subject positions is the process of women undertaking emotion work on and behalf of themselves, their male partners and their children. The findings demonstrate the overarching normative dimensions of women's emotional self‐transformations in the service of their mining partners' careers and the attendant reproduction of everyday patriarchal relations in the private lives of mining families. 相似文献

In 2017, the US Environmental Protection Agency (EPA) was criticized for two controversial directives that restricted the eligibility of academic scientists to serve on the agency's key science advisory boards (SABs). The EPA portrayed these directives as necessary to ensure the integrity of the SAB. Critics portrayed them as a tactic by the agency to advance a more industry-friendly deregulatory agenda. With this backdrop, this research examined board composition and its effect on the perceived legitimacy of risk management recommendations by the SAB. In an experiment, we presented participants with hypothetical EPA SABs composed of different proportions of academic and industry scientists. We then asked participants to rate their satisfaction with, and the legitimacy of, these boards in light of their decisions in scenarios based on actual EPA SAB deliberations. Participants perceived higher levels of satisfaction and legitimacy when SABs made more stringent risk management recommendations. While SABs dominated by industry scientists were perceived to be more strongly motivated to protect business interests, we found no effect of board composition on perceptions of satisfaction and legitimacy. These results are consistent with prior research on decision quality that suggests people use normative outcomes as a heuristic for assessing the quality of deliberations. Moreover, these results suggest that members of the public are supportive of federal SABs regardless of their composition, but only if they take actions that are consistent with normative expectations. 相似文献

Today’s youth face high unemployment and other labor-market challenges; these challenges are often worse for youth with disabilities. This paper explores what employers and employment counselors look for when hiring youth into entry-level jobs and whether they believe youth with disabilities have fewer skills than those without. This study used a qualitative design with a purposive sample of 19 youth employers and employment counselors. Our findings show that youth employers and employment counselors identify the following traits as enhancing young workers’ employability: a well-prepared job applicant; job fit and a good attitude; and soft skills. We find youth with disabilities are perceived to be at a disadvantage. Consistent with a rising body of research, the emphasis on soft skills, and pliability, opens the door for employment discrimination. 相似文献

This paper reports on the qualitative phase of a New Zealand study of young people who had been exposed to “risky” environments from a young age. These young people had experienced traumatic events such as abuse, violence, addictions, mental health issues, and many had been excluded from school. The young people (aged between 13 and 17) were users of multiple services (statutory and nongovernmental services including: child welfare, juvenile justice, remedial education, and mental health services). Qualitative interviews (n?=?109) explored young people's experiences in their families, communities, education, and their perspectives on support provided by services and their own support networks. Three thematic clusters emerged as central motifs in young people's experiences and are the focus of this article: navigating “risky” environments; services and support; and, working to find different pathways. 相似文献
