In addition to introducing markets and market‐like structures into public services, New Labour wished to promote the involvement of users and the public in decision‐making in other ways than as individual consumers. One way was to involve the public in the governance of organizations. This could be done by removing public services from state control, and transferring them to mutual ownership; or by increasing public involvement in the governance of public bodies. NHS foundation trusts (FTs) were presented as mutuals. Our study shows FTs are not mutuals, as they continue to be owned by the state. Moreover, staff of FTs were generally not engaging with the new governance structures. In general, there was mixed experience of the new structures enabling governors to increase accountability of the hospitals to the public. On the other hand, having a membership did enhance the legitimacy of FTs, as opposed to other NHS organizations. The findings of the study are of current interest as the coalition government is continuing with the policy of FTs, and also encouraging mutuals and other forms of public involvement.  相似文献

This article compares policy and practices for engaging older people in community life in Sweden and Australia. Barriers and support for active engagement through paid work, social activism, volunteering and aged services are compared. Both countries face issues of ageing populations, services for rural areas and people with small needs. Issues for Sweden were the absence of age discrimination legislation, availability of funds and lack of recognition of the growing levels of volunteering. Issues for Australia concerned the new managerialist approach to services, with associated complexities of access and limited social activism.  相似文献

Legislators at the state and national levels are addressing renewed concerns over the adequacy of hospital nurse staffing to provide quality care and ensure patient safety. At the same time, the well‐known nursing shortage remains an ongoing problem. To address these issues, we reexamine the nurse scheduling problem and consider how recent health care legislation impacts nursing workforce management decisions. Specifically, we develop a scheduling model and perform computational experiments to evaluate how mandatory nurse‐to‐patient ratios and other policies impact schedule cost and schedule desirability (from the nurses' perspective). Our primary findings include the following: (i) nurse wage costs can be highly nonlinear with respect to changes in mandatory nurse‐to‐patient ratios of the type being considered by legislators; (ii) the number of undesirable shifts can be substantially reduced without incurring additional wage cost; (iii) more desirable scheduling policies, such as assigning fewer weekends to each nurse, have only a small impact on wage cost; and (iv) complex policy statements involving both single‐period and multiperiod service levels can sometimes be relaxed while still obtaining good schedules that satisfy the nurse‐to‐patient ratio requirements. The findings in this article suggest that new directions for future nurse scheduling models, as it is likely that nurse‐to‐patient ratios and nursing shortages will remain a challenge for health care organizations for some time.  相似文献

Cumulative harm is a major child protection concern, with significant consequences for child well‐being and development often into adult life. This systematic literature review examines how the construct of cumulative harm is understood and operationalized within current Australian child protection legislation, policy, and practice and situates this within an international context. Scholarly articles and grey literature were qualitatively analysed to explore two main research questions: How is cumulative harm to children identified, assessed, and ultimately incorporated into child protection and legal structures? And what are the most effective responses to cumulative harm identified for child protection practice? What was found was that although the construct of cumulative harm is increasingly incorporated into child protection practice and legislation, in practice, this remains crisis‐driven. Although the literature emphasized prevention and early intervention as responses to reducing the cumulative impact of adverse childhood experiences, there is negligible research on what constitutes an effective response to cases involving cumulative harm. They are cases which are less visible within the child protection paradigm; attention to the impact of risk of maltreatment and its influence on long‐term developmental outcomes is less well addressed creating uncertainty and ambiguity about effective responses for child protection.  相似文献

The development of child care policy has been characterized by debates organized along various intersecting constructs such as universal vs. selective services and prevention vs. treatment. Family centres, as a site of service delivery, represent a point of intersection of many of these arguments. Their role is contested between protagonists of all positions within the child care policy debates: whether they should be professionally managed, led by service users, provide a service to referred families, or offer a universal resource for local communities. This paper argues that a needs-led approach to family centre development may supersede the sterile dichotomies of many of these debates. A case study is presented of an evaluation research project, designed to inform a needs-led family centre policy for an urban unitary social services department. The methodology utilizes an approach combining quantitative analyses of geographical distributions of family need with qualitative presentation of the perspectives of actual and potential centre users, and agency stakeholders. The outcome is a family centre model which, by focusing on need, seeks to transcend the limited oppositions of the child care debates.  相似文献
