Mejor Sola Que Mal Acompañada: Strengths and Challenges of Mexican-Origin Mothers Parenting Alone

Marriage is often viewed as central to family stability and health, and problem-saturated discourses about single parenting have failed to address the advantages of parenting alone. Heuristic methodology was used to examine the meanings and experiences of Latinas parenting alone. This study reflects the lived experiences of 20 Mexican-origin women who participated in a 1- to 2-hour, in-depth, semistructured interview. Data are presented by three exemplary portraits and five core consistencies embedded in a composite depiction. Overall, although parenting alone was challenging, it was considered beneficial and preferable, especially when the child's father's influence was problematic or detrimental to the family's well-being. Chicana feminism and an intersectionality lens is used to interpret findings. Implications for research and intervention are also discussed.

Change and Inertia in the New York State Medicaid Personal Care Services Program: An Institutional Case Study

This study analyzes how competing logics (belief systems) of stakeholders have influenced patterns of change and inertia in the development of the New York Medicaid Personal Care Services (PCS) program. A case-study methodology was used to collect documents, statistics, and interview data from four key stakeholder groups: state and city officials, PCS agencies, a labor union, and consumer advocates covering the period 1999 to 2005. The New York PCS program is one of the oldest, largest, and most stable programs in the United States. Its early unionization of workers resulted in relatively generous wages and benefits and made New York number one nationally in PCS spending per capita. In spite of wide support from stakeholder groups, the overall number of participants has gradually declined since 1999. A consumer-directed model of personal care developed in 1995 challenged the status quo and has grown steadily. Resistance by public officials, agency providers, and union representatives to the consumer-directed model has resulted in a small program that is often targeted toward individuals labeled “difficult to serve.” Dominant stakeholders in New York have ensured a stable personal care program that has resisted change and led to program inertia.     

Moving Toward Medicare Home Health Coverage for Persons with Alzheimer's Disease

ABSTRACT

Medicare home health care policy does not incorporate research evidence of effective palliative home care interventions for Alzheimer's disease and dementia patients and caregivers. This article examines the dissonance between the needs and burdens of Alzheimer's disease patients and caregivers, research results on medical and palliative care interventions, and medicalized public policy in the Medicare home health benefit. The article asserts existing research establishes a prima facie case exists for the federal government to fund a Medicare Palliative Home Care for Alzheimer's disease demonstration project. The article cites the success of the Medicare Hospice Demonstration project and Hospice Medicare Benefit in reducing costs and improving client quality of life as precedent and a model for Alzheimer's disease. Other research implications are identified.     

Bondage and resistance in the land of lead: antebellum Upper Mississippi River Valley mineral country landscapes

Slaves and indentured servants in the Illinois–Wisconsin border region suffered manifold hazards in mining, building, transport, domestic, and hospitality realms of the lead economy. Their toils underpinned settlers’ survival and reputed hardiness as well as the region’s prosperity and growth. While local white abolitionist fiction emphasized secret, successful resistance, unfree people themselves pursued more public, mobile paths to freedom. Masters’ knowledge of and concerns about the landscape, combined with weak anti-slavery support, often impeded freedom and justice.     

Police Crime: The Criminal Behavior of Sworn Law Enforcement Officers

This essay examines the criminological conceptualizations and research on police crime, that is, the criminal behavior of sworn law enforcement officers. It starts with an overview of the conceptualization of police crime as it relates to organizational and scholarly perspectives of social deviance. Police deviance is often conceptualized to include various misconduct, corruption, and/or crime committed by police organizations and police officers. This essay focuses on police crime resulting in the arrest of a sworn law enforcement officer and is organized within a conceptual framework that incorporates five types of police crime: sex‐related police crime, alcohol‐related police crime, drug‐related police crime, violence‐related police crime, and profit‐motivated police crime.     

Transition from Home Care to Nursing Home: Unmet Needs in a Home- and Community-Based Program for Older Adults

A major effort is under way nationally to shift long-term care services from institutional to home- and community-based settings. This article employs quantitative and qualitative methods to identify unmet needs of consumers who transition from a statewide home- and community-based service program for older adults to long-term nursing home residence. Administrative data, care manager notes, and focus group discussions identified program service gaps that inadequately accommodated acute health problems, mental health issues, and stressed family caregivers; additional unmet needs highlighted an inadequate workforce, transportation barriers, and limited supportive housing options. National and state-level policy implications are considered.
