Tobacco use continues to be a serious public health issue. Although declining in middle‐ to high‐income countries, smoking rates are often higher in disadvantaged communities and vulnerable groups. Knowledge about tobacco‐related harm also tends to be incomplete. To date, legislation has focused on public domains, but the pressure to protect children in the home and other private spaces is fast becoming a focal point for potential legislation and intervention. Negotiating the boundary between privacy and protection is likely to become a matter of professional concern.  相似文献

BackgroundThis study fills a gap in the literature with a quantitative comparison of the maternity care experiences of women in different geographic locations in Queensland, Australia.MethodData from a large-scale survey were used to compare women's care experiences according to Australian Standard Geographical Classification (major city, inner regional, outer regional, remote and very remote).ResultsCompared to the other groups, women from remote or very remote areas were more likely to be younger, live in an area with poorer economic resources, identify as Aboriginal and/or Torres Strait Islander and give birth in a public facility. They were more likely to travel to another city, town or community for birth. In adjusted analyses women from remote areas were less likely to have interventions such as electronic fetal monitoring, but were more likely to give birth in an upright position and be able to move around during labour. Women from remote areas did not differ significantly from women from major cities in their satisfaction with interpersonal care. Antenatal and postpartum care was lacking for rural women. In adjusted analyses they were much less likely to have booked for maternity care by 18 weeks gestation, to be telephoned or visited by a care provider in the first 10 days after birth. Despite these differences, women from remote areas were more likely to be breastfeeding at 13 weeks and confident in caring for their baby at home.ConclusionsFindings support qualitative assertions that remote and rural women are disadvantaged in their access to antenatal and postnatal care by the need to travel for birth, however, other factors such as age were more likely to be significant barriers to high quality interpersonal care. Improvements to maternity services are needed in order to address inequalities in maternity care particularly in the postnatal period.  相似文献

ABSTRACT

This paper aims to identify housing disadvantages faced by migrants and ethnic minorities; the legal, policy and market forces that shape them; how they have developed over time; how they are manifest nationally and locally; and how they are being responded to locally by those concerned with mitigating them. The paper thereby intends to provide a foundation to inform future research and policy and to engage with local actors to develop ways of overcoming migrant housing disadvantage and challenging discrimination. The paper finds that the interplay of legal changes, which have increasingly differentiated migrants since the 1940s, and shifting housing markets, has driven exclusion of migrants and minorities such that considerable disadvantage is revealed by analysis of census data. However, attention to local specificity provides evidence of positive responses. Examples are presented in relation to access to affordable housing, enactment of homelessness duties and community actions. Methodologically, this paper highlights the importance of simultaneous consideration of migration and ethnicity as markers of difference and exclusion, and the potential of co-production approaches for socially meaningful research concerned with inequalities.  相似文献

A current crisis in education is leaving students less prepared to succeed in the working world than any generation before them. Increasingly complex external, nonacademic pressures have an impact on many of today's students, often causing them to drop out of school. Only 76 percent of Massachusetts high school students graduate, and only 29 percent earn a college degree. National figures are worse. Most educational institutions share a common goal to support students in becoming skilled, productive, successful members of society, but the author argues that this goal is not being met. Despite the constant changes in the world, educational practices have remained static. Most public schools are not adapting to meet the shifting needs of students. Universities are not able to prepare the right mix of prospective employees for the demands of the job market; for example, schools are graduating only 10 percent of the needed engineers. Institutions of higher learning cannot keep up with employers' needs in an evolving global market: strong math, science, and writing abilities; critical thinking skills; and the ability to work in teams. The author draws on exemplary efforts at work in his home state of Massachusetts--whose improvements in student achievement outcomes have been some of the best in the nation--to suggest there is promise in twenty-first century learning. Middle school students involved in a NASA-funded project write proposals, work in teams, and engage in peer review. Older students participate in enhanced, hands-on cooperative school-to-work and after-school programs. Schools are starting to offer expanded day learning, increasing the number of hours they are engaged in formal learning. Yet such programs have not reached significant levels of scale. The author calls for a major shift in education to help today's students be successful in the twenty-first century.  相似文献

Theories of growth have made progress in understanding the mechanisms of growth in economic terms. However, there is less understanding of the political processes that enable or obstruct these mechanisms. This article provides a four‐stage framework to clarify and analyse the connections between politics and growth: (i) discussing the basic conditions essential for growth; (ii) suggesting that whether or not these conditions emerge depends on specific forms of public‐private interaction; (iii) linking these relationships to the incentives facing those in political power and investors; and (iv) considering the factors at country level that may help to push incentives in a pro‐growth direction.  相似文献

This study evaluated the feasibility, safety, and findings from a protocol for exercise-bicycle ergometry in subacute-stroke survivors. Of 117 eligible candidates, 14 could not perform the test and 3 discontinued because of cardiac safety criteria. In the 100 completed tests, peak heart rate was 116 +/- 19.1 beats/min; peak VO(2) was 11.4 +/- 3.7 ml x kg x min(-1), peak METs were 3.3 +/- 0.91, exercise duration was 5.1 +/- 2.84 min, and Borg score was 14 +/- 2.6. Among 71 tests, anaerobic threshold was achieved in 3.0 +/- 1.7 min with a VO(2) of 8.6 +/- 1.7 ml x kg x min(-1). After screening, this protocol is feasible and safe in subacute-stroke survivors with mild to moderate deficits. These stroke survivors have severely limited functional exercise capacity. Research and clinical practice in stroke rehabilitation should incorporate more comprehensive evaluation and treatment of endurance limitations.  相似文献
