Elderly People's Perceptions of a Telehealthcare System: Relative Advantage,Compatibility, Complexity and Observability

The use of telehealthcare systems to promote independent living for elderly people is growing. The results presented in this article, derived from an initial user lab test of a telecare system—GiraffPlus—indicate that the crucial factor for adoption of telehealthcare systems is not usability but the system's ability to support autonomy in everyday life. Eleven users tested the usability and reported what they perceived as possible benefits of having such a system at home. To support autonomy, customization is crucial for the system to be perceived as meaningful for the individual. Our analysis confirms previous research.

The author examines a particular form of addictive exploitation of sensory experience to extend Meltzer’s concept of claustrum states. She elaborates how bodily experience—both the patient’s and the analyst’s—is key to emergence from these states. Following the distinction between holding and containing made by Bion and Cartwright, the author traces the reciprocity of meaning making through the analyst’s seeing/sensing of beauty in the patient, noted by the analyst’s erotic response to a coming-alive self, allowing for aesthetic reciprocity and transformation.     

Casino gaining offense inmates: What are these men like?

The characteristics of 28 men convicted for casino gaming offenses in Nevada were determined. Twenty-seven of these inmates cheated using slot or video poker machines. Most of the subjects used slugs. The other cheated at cards. Compared to other inmates, a disproportionate number of inmates were white. There were no black inmates incarcerated for this crime. Eighteen (57%) of the gaming offenders were over the age of 40 when convicted, in significant comparison to 27% of the general population inmates. They had significantly more aliases than the general population inmates. None of the gaming offenders had a history of violent felony convictions. They had, however, a history of great social, marital, occupational, and financial instability in addition to a criminal life style.     

Explained Variation and Predictive Accuracy in General Parametric Statistical Models: The Role of Model Misspecification

When studying a regression model measures of explained variation are used to assess the degree to which the covariates determine the outcome of interest. Measures of predictive accuracy are used to assess the accuracy of the predictions based on the covariates and the regression model. We give a detailed and general introduction to the two measures and the estimation procedures. The framework we set up allows for a study of the effect of misspecification on the quantities estimated. We also introduce a generalization to survival analysis.     

Health care in the 21st Century

Our primary agenda for the 21st Century is the reinvention of America. We must reinvent democracy, capitalism, entrepreneurism, and community. Indeed, we must recreate all of our major social institutions. This includes health care. A design for a new society requires a new design for health care. In fact, health care enjoys a special privilege in this regard. It is the gateway to total societal redesign. Health is the common denominator in any society. If you loose your health you cannot work, you cannot play, you cannot study; and, if you lose it sufficiently, you cannot even pray. Health and well-being create the foundation for all other constructive human endeavors. Therefore, the design of healthy communities is the necessary first step in the redesign of total human habitats. This massive redesign effort will take a century. However, it will be launched in the next few years.     

Spirituality and the physician executive

The "s" word can now be spoken without flinching in health care organizations. Spirituality is becoming a common topic in management conferences around the world. Many U.S. corporations are recognizing the role of spirituality in creating a new humanistic capitalism that manages beyond the bottom line. Spirituality refers to a broad set of principles that transcend all religions. It is the relationship between yourself and something larger, such as the good of your patient or the welfare of the community. Spirituality means being in right relationship to all that is and understanding the mutual interdependence of all living beings. Physician executives should be primary proponents of spirituality in their organizations by: Modeling the power of spirituality in their own lives; integrating spiritual methodologies into clinical practice; fostering an integrative approach to patient care; encouraging the organization to tithe its profits for unmet community health needs; supporting collaborative efforts to improve the health of the community; and creating healing environments.     

Leading well is a matter of resources: Leader vigour and peer support augments the relationship between transformational leadership and burnout

ABSTRACT

Although studies suggest that transformational leaders play an important role in employee health and well-being, the relationship between transformational leadership and employee burnout remains unclear. One reason may be that moderators may play an important role. Building on conservation of resources theory, we examined if leaders’ perceptions of internal and external resources in terms of vigour and peer support augmented the relationship between transformational leadership and employee burnout in a sample of municipality workers and their leaders in Sweden (N?=?217). Multilevel analyses over two time points revealed that both vigour and peer support enhance this relationship, such that when leaders experience high levels of vigour or peer support, the negative relationship between transformational leadership behaviours and employee burnout was strengthened. Our findings suggest that both personal and contextual resources may help leaders to better engage in transformational leadership, which is important in order to protect employees from burning out.
