In 1996, the eight-million member Kaiser Permanente HMO adopted a vision statement that said by 2005 it would expand its services to include home- and community-based services for its members with disabilities. It funded a 3-year, 32-site demonstration that showed that it was feasible to link HMO services with existing home-and community-based (HCB) services and that members appreciated the improved coordination and access. This private-sector project showed that devolution can produce innovative and feasible models of care, but it also showed that without federal financial and regulatory support, such models are unlikely to take hold if they are focused on "unprofitable" populations, for example, those who are chronically ill, poor, and/or disabled.  相似文献

This article presents data from an evaluation of the Pennsylvania Student Assistance Program (SAP). Focusing on both program process and effectiveness, the evaluation was conducted to determine the overall efficacy of SAPs in Pennsylvania and, more specifically, how SAP is currently being implemented. Five data collection strategies were employed: statewide surveys of SAP team members and county administrators, focus groups, site visits, and the Pennsylvania Department of Education SAP Database. A total of 1204 individual team members from 154 school buildings completed the team member survey. Fifty-three county administrators completed the county administrator survey. Focus groups were comprised of SAP coordinators, school board personnel and community agency staff. Site visits were conducted at five schools. The findings of the evaluation indicate that SAP in Pennsylvania is being implemented as designed. Recommended is the development of benchmarks and indicators that focus on the best SAP practices and the extent to which various indicators of the effectiveness of SAP are occurring at appropriate levels.  相似文献

The well-being of youths who age out of the out-of-home care system in the U.S. has long been of great interest to child welfare practitioners and policymakers. In spite of this interest, however, very little is known about how these youths fare when they must make the transition to independence. The Foster Youth Transitions to Adulthood Study is tracking the experiences of 141 young adults who left care in Wisconsin in 1995 and 1996. This article describes these youths and their experiences in the first 12 to 18 months after leaving care. The findings suggest that the transition to independence is a difficult time for youth leaving the out-of-home care system.  相似文献

Psychological abuse among battered women has been relatively understudied. However, battered women's reports in the existing qualitative and quantitative research suggest that the effects of psychological abuse can be even more damaging than the effects of physical abuse. The current study attempted to clarify the relationship between psychological abuse and posttraumatic stress disorder (PTSD) within a sample of battered women by statistically controlling for the effects of physical abuse. This study also explored the affective experiences of shame and guilt as important variables in the development of PTSD in battered women. This investigation replicated previous work suggesting that battered women are very much at risk for a diagnosis of PTSD and suggests that clinicians and researchers may need to focus on psychological abuse as a predictor of PTSD symptomatology. The current findings encourage attention to shame reactions in battered women and suggest new directions in the study of PTSD for other traumatized populations.  相似文献

This paper examines the extent to which the structure of Russian organizations is similar to that observed among organizations in the West. Data were obtained in 1995 from a sample of organizations in the greater Moscow area by means of a structured interview schedule administered to the chief executive officer within each organization. The interview schedule contained measures developed by the Aston Group which have been used widely in previous research. The structure of Russian organizations was evaluated in terms of a causal model based upon consistent findings of previous research in this area. The results suggest that the structure of Russian organizations differs in important ways from the type of structure observed in most Western organizations. The major differences are that size, specialization and formalization are positively associated with centralization in Russian organizations. It is suggested that the historical tradition of centralized rule in Russia and the criminal environment of current Russian organizations are, in part, responsible for this difference.  相似文献

In recent decades unprecedented urbanization has constituted a huge challenge to urban infrastructure development and management in most developing countries. Meanwhile, the appropriateness of conventional urban planning approaches to the improvement of urban environmental conditions in the face of deteriorating urban environment have often been questioned. It became inevitable to search for new approaches particularly in the context of low-income urban communities. International organizations such as the United Nations Development Programme (UNDP) and the United Nations Centre for Human Settlements (UNCHS) have subsequently initiated strategies to evolve a participatory approach to the development and management of the urban environment. One of such strategies is the Sustainable City Programme (SCP) which aims to provide municipal authorities and city stakeholders in the public, private and community sectors with an enhanced capacity for environmental planning and management. The programme is being implemented in several cities in parts of the developing world, including Ibadan – the Sustainable Ibadan Project (SIP) – in Nigeria. This paper examines the partnership approach to urban environmental infrastructure improvement, development and management as represented by the SIP. The relevance of the participatory partnership approach of the SIP for cities in parts of developing regions is examined. The paper implicitly stresses that bottom-up participatory stakeholder partnership is a strategy capable of enlisting the financial, material resources and expertise of various sectors of the urban community towards the improvement of urban environmental infrastructure. The paper presents preliminary findings of a study. Although traditional appraisal techniques were not employed, nonetheless, the paper presents a discussion of the processes and dynamics of the SIP on the basis of which some useful lessons may be drawn.  相似文献
