Cognitive-behavioral therapy for obsessive-compulsive disorder: review of treatment techniques

Obsessive-compulsive disorder (OCD) is a chronic, impairing condition with an estimated lifetime prevalence in adults of 2.5%. Controlled treatment trials have demonstrated that cognitive-behavioral therapy (CBT) is an effective intervention for OCD. However, many individuals diagnosed with OCD do not receive appropriate, empirically validated interventions, perhaps due to limited knowledge of CBT among mental health practitioners. This article provides a review of CBT for OCD. Issues related to treatment delivery and assessment are presented and highlighted by an individual example.

1. Organizations should establish workplace violence prevention policies and education programs. 2. Multidisciplinary teams should be established to address potentially violent situations. 3. Law enforcement or other experts should be consulted in stalking cases.     

A randomized trial of emotion-focused therapy for couples in a training clinic

Forty married couples participated in a randomized trial comparing 8 weekly sessions of emotion-focused therapy (EFT) for couples to a group of couples who were placed on an 8-week waiting list. A composite marital satisfaction score was created from scores on the Dyadic Adjustment Scale, Positive Feelings Questionnaire, and Personal Assessment of Intimacy in Relationships scale. Controlling for pretest scores, participants in the treatment group had significantly higher levels of marital satisfaction after 8 weeks than wait-list participants. Supplementary analyses identified variables associated with gains in therapy and with dropping out of the study.     

The impact of non-bottleneck variation in a manufacturing cell

Protective capacity is the 'extra' capacity placed at non-bottleneck resources to absorb random disruptions in planned levels of performance so that the bottleneck resource continues to be effectively utilized. A full factorial experiment with a simulation model was conducted to explore issues associated with the quantity and location of processing variance in a five-station manufacturing cell. The cell's performance was measured using both mean flow time ( MFT ) and bottleneck shiftiness ( SHIFT ) for 3 patterns of variance for the non-bottlenecks at 5 different levels of variation. In order to investigate the importance of the quantity of added capacity on the variation both a low level of protective capacity (10%) and a high level (50%) were considered. The results indicate that having the higher variation work centres close to the bottleneck provides reduced MFT and SHIFT . The performance measures improved at both the low and high setting of protective capacity.     

Differentiation of Self and the Process of Forgiveness: A Clinical Perspective for Couple and Family Therapy

In this article we explore the role of differentiation of self in facilitating forgiveness in the context of couple and family relationships. Differentiation is defined from the Bowen perspective as the ability to connect with others without being excessively emotionally reactive to the ebb and flow inherent in all significant relationships (being able to connect to others yet also being able to self‐regulate). Forgiveness is described as the releasing of an emotional injury via a complex psychological and relational process that is less an act of will than a discovery or possibility through understanding and empathy. Differentiation of self is related to emotional intelligence and empathy. The developmental and relational benefits of such are illustrated and discussed. A rationale for viewing differentiation and forgiveness in a contextual, historical, and relational attachment paradigm is suggested. Relevant clinical cases illustrate the dynamics of differentiation and forgiveness as discovery in the context of an understanding and empathic relational environment.     

Urban tree cover: an ecological perspective

Analysis of urban tree cover is generally limited to inventories of tree structure and composition on public lands. This approach provided valuable information for resource management. However, it does not account for all tree cover within an urban landscape, thus providing insufficient information on ecological patterns and processes. We propose evaluating tree cover for an entire urban area that is based on patch dynamics. Treed patches are classified by their origin, structure, and management intensity. A patch approach enables ecologists to evaluate ecological patterns and processes for the entire urban landscape and to examine how social patterns influence these ecological patterns and processes.     

Land of forgetfulness: dementia care as spiritual formation

The personal circumstances of dementia caregiving are sometimes experienced as so severely demanding as to be overwhelming. While sustenance within, and recovery from these experiences are increasingly addressed in helpful ways from psychological, emotional, and practical life planning frames of reference, we are still learning how to speak with one another about the spiritual dimensions of this peculiar journey. From a spiritual perspective, the caregiver initially, and frequently perpetually, finds her/himself in a state of spiritual exhaustion. Oddly enough, there are ample resources at hand to address this existential state with wisdom and grace, allowing for the possibility that the caregiver might move from spiritual weariness to enhanced spiritual formation.
