In this paper, we present the process of developing and evaluating an instrument designed to measure the extent to which a complex community system has changed as a result of a community initiative, and for purposes of this research, doing this within the content area of developing local trauma-informed child welfare systems in specific communities in Michigan. The instrument was designed for the Southwest Michigan Children’s Trauma Assessment Center’s (CTAC) Substance Abuse and Mental Health Services Administration (SAMHSA)-funded initiative to bring a trauma-informed perspective to professionals working with children in child welfare. The Trauma Informed System Change Instrument was developed with the input of experts in trauma-informed system change. Two parts of the instrument were analyzed separately using confirmatory factor analysis. A two factor model was fit for Community Characteristics and a three factor model for Individual Characteristics. Although adequate factorial validity was obtained for the instrument, specific items on the instrument that were problematic in fitting the model were identified, and suggestions for revising the instrument for improved functionality are offered, as are other potential uses of the instrument.  相似文献

The transition from a placement in care to an independent life can be a problematic phase for young people. In Sweden, special care‐leaving services are almost non‐existent. What then happens to young people when they leave a placement in out‐of‐home care? This paper draws on the results of a study in which 16 young care leavers between the ages of 18 and 22 years were interviewed. Telephone interviews were also performed with the young care leavers' parents, social workers, foster carers and institutional staff. The aim of the study was to investigate how young care leavers perceive the transition from care to an independent life. The Swedish welfare model, the prolonged transition to adulthood and the family‐oriented welfare discourse have been used as analytical perspectives. The results show that young care leavers have a pronounced need for social, emotional, practical and financial support. Whilst such support is occasionally provided by foster carers and residential staff, it is seldom given by social services or biological parents. This group is at risk of facing severe problems in the transitional phase from care to independent life, a fact that is not acknowledged by the Swedish welfare system.  相似文献

Abstract

The sixteenth issue of our index of interviews appearing in current little magazines received for the Sukov Collection of Little Magazines (Department of Special Collections, Memorial Library, University of Wisconsin–Madison) is a double one, covering 2000 and 2001. The index includes 1059 entries, with the usual mix of well-known and new writers in a wide range of journal titles. The index follows an initial list of magazine titles currently received by the Sukov Collection, now arranged by country of publication, which are not included in the thirty-seventh edition of the International Directory of Little Magazines and Small Presses (Dustbooks). There is a need for greater understanding of setting-specific influences on physical activity to complement the predominant research paradigm of individual-centered influences on physical activity. In this study, the authors used a cross-sectional multilevel analysis to examine a range of neighborhood-level characteristics and the extent to which they were associated with variation in self-reported physical activity among older adults. The sample consisted of 582 community-dwelling residents age 65 years and older (M = 73.99 years, SD = 6.25) recruited from 56 neighborhoods in Portland, OR. Information collected from participants and neighborhood data from objective sources formed a two-level data structure. These hierarchical data (i.e., individuals nested within neighborhoods) were subjected to multilevel structural-equation-modeling analyses. Results showed that neighborhood social cohesion, in conjunction with other neighborhood-level factors, was significantly associated with increased levels of neighborhood physical activity. Overall, neighborhood-level variables jointly accounted for a substantial variation in neighborhood physical activity when controlling for individual-level variables.  相似文献

Inhibition of emotional expression has been associated with the incidence and progression of breast cancer and other chronic illnesses. The important health-related factor, however, may be ambivalence about the expression of emotions rather than repression itself. This cross-sectional analysis of baseline data from 159,557 participants in the Women's Health Initiative examined the influence of expression of negative emotion and ambivalence about expression of emotion on psychosocial factors and health-related quality of life measured by the Medical Outcomes Study Short-form 36 (SF-36). Overall, observed correlations were modest but in the expected direction; that is, greater ambivalence about negative emotional expression was associated with worse general health and poorer psychosocial risk profile. Ambivalence about expressing negative emotion was more highly correlated with psychosocial factors and health-related quality of life than emotional expression. In general, our analysis supports prior studies suggesting that ambivalence may be more important to consider in studies of health-related outcomes than expression.  相似文献

Joan Acker can be considered the godmother of gendered organizations. In this paper, we reflect on the impact that Joan Acker's work has on our thinking and our careers as gender scholars in management and organization studies in Europe. First, we tell our personal stories of close encounters with Joan Acker. Second, we highlight what we consider to be two key contributions of Joan Acker. The first are the interrelated gendered processes regarding structure, culture, interaction and identity from her work in the early 1990s, the second the notion of inequality regimes from her later work on the intersections of gender, class and race. We then discuss how Acker's work has been influential in our research on gendered organizations, and in our teaching when we use it in our explanations of the functioning of gender in organizations to students, and in our work as advisors and consultants for organizations interested in equality, diversity and inclusion. Finally, we elaborate on new directions building on Acker's work, especially in current theorizing on gender and diversity in organizational change.  相似文献

This paper considers the experiences of older self-funders in England in the context of policies promoting choice and control. Self-funders are people who are not state-funded; they pay for social care from their own resources. Choice and control have been operationalized through personal budgets, based on the assumption that managing resources enhances ability to access appropriate care and support. This paper uses data from 40 qualitative interviews with self-funders and their relatives, and 19 with professionals. It explores the impact of the financial and social capital that self-funders are assumed to have and asks how older self-funders experience choice and control. The study found that older self-funders drew on personal experiences, family, and friends for information; were reluctant to spend their wealth on care due to competing priorities; and felt they had more control over the timing of decisions than people who were state-funded. Personal wealth appears to be perceived differently to funds “gifted” to people through cash for care schemes.  相似文献

ABSTRACT

This study examines the extent to which the use of part-time work and parental leave is accepted at German workplaces. Is there evidence for a weakening of ideal worker and ideal parent norms? Interviews were conducted with 95 employees in different status positions in hospitals, police stations and industrial companies. The results indicate that even though ideal worker norms and ideal parent norms still prevail, especially in higher-status positions and typical male professions, they have shifted partially for some employees. Partial change can be observed for mothers in high-status positions (part-time) and for men up to mid-level positions (parental leave). There is greater acceptance of fathers’ parental leave, mainly due to the reform of the German parental leave legislation. Part-time for women in upper-level positions has become more accepted due to work-life balance policies at the company level. Norm changes, however, have stalled at the halfway mark. Fathers are still expected to prioritize their career and to schedule parental leave according to their organization’s business needs. Part-time working mothers in high-status positions are expected to deliver performance similar to that of full-time workers. Moreover, norm changes are hindered by economic constraints, mainly staff shortages.  相似文献

Background

Decision-making in midwifery, including a claim for shared decision-making between midwives and women, is of major significance for the health of mother and child. Midwives have little information about how to share decision-making responsibilities with women, especially when complications arise during birth.

Aim

To increase understanding of decision-making in complex home-like birth settings by exploring midwives’ and women’s perspectives and to develop a dynamic model integrating participatory processes for making shared decisions.

Methods

The study, based on grounded theory methodology, analysed 20 interviews of midwives and 20 women who had experienced complications in home-like births.

Findings

The central phenomenon that arose from the data was “defining/redefining decision as a joint commitment to healthy childbirth”. The sub-indicators that make up this phenomenon were safety, responsibility, mutual and personal commitments. These sub-indicators were also identified to influence temporal conditions of decision-making and to apply different strategies for shared decision-making. Women adopted strategies such as delegating a decision, making the midwife’s decision her own, challenging a decision or taking a decision driven by the dynamics of childbirth. Midwives employed strategies such as remaining indecisive, approving a woman’s decision, making an informed decision or taking the necessary decision.

Discussion and conclusion

To respond to recommendations for shared responsibility for care, midwives need to strengthen their shared decision-making skills. The visual model of decision-making in childbirth derived from the data provides a framework for transferring clinical reasoning into practice.  相似文献
