This study examines the identity cues that family caregivers and healthcare personnel use with seniors living with dementia and living in nursing homes. The identity cues represent biographical knowledge used to stimulate the dementia sufferer, trigger signals and incite interaction. Our grounded approach hinges on three objectives: to identify and categorize identity cues; to document their uses; and to gain a better understanding of their effectiveness. We interviewed nine family caregivers and 12 healthcare workers. Qualitative data indicates that the participants use identity cues that evoke seniors' sociological, relational and individual characteristics. These identity cues play a central role in communication and constitute important information that the family caregivers can share with healthcare personnel. They sustain memory, facilitate care and reinforce seniors' self-value. These results help to define identity, foster a greater role for family caregivers, and constitute a sound basis for the implementation of personalized interventions.  相似文献

This note represents a portion of the research that has been conducted using the Ohio State University's large kidney transplant database. Our latest results into understanding the impact of covariates on renal graft success including the impact of drug therapy are discussed.

The major result here is that using both the Cox model and Breiman's Random Forest Data Mining techniques has helped to unravel the mystery of the “induct” immunosuppressant covariate slipping in and out of the list of important variables. We also make the interesting observation that the Random Forest method seems to mimic the clinician's (R.P.) understanding of the importance of variables.  相似文献

The main point in this article is to conceptualise how demands connected to children's life conditions influence both children and caregivers. To pursue this aim I advocate an extension of Vygotsky's cultural-historical theory of children's learning and development. Vygotsky pursued a wholeness approach to children's development with his concept of “the child's social situation of development” as the child's dialectic experiential and motivational relation with his or her surrounding. This conception I extend with the concepts of institutional practice and activity setting. The conditions for children's activities are the institutional practice and its activity settings. But a child's activities in these settings also has to be seen from the child's perspective, that is, his or her motive orientation. To focus on the child's motive within an activity setting—requires the researcher to focus on the child's social situation of development to discern how the dialectic between the child's orientation within an activity setting and the demands from the setting and other persons influence the child's activities within the child's zone of proximal development.  相似文献

A central dilemma in developmental psychology has been to combine general concepts with research of the individual child in all her complexity in everyday life activities. Psychologists such as Riegel, Bronfenbrenner, Burman, Morss, Hedegaard, and Walkerdine have criticized research approaches that study child development from a functional view. Sociologists and anthropologists, such as Corsaro, James, Jenks, Prout, and Qvotrup have instead argued for childhood studies as the alternative to developmental psychology. None of these approaches is alone sufficient; instead, it is important to formulate a theoretical approach of child and youth development that combines general psychological concepts with research of children and youth in concrete settings, such as home or school. The aim of this article is to argue that this will be possible by building on Vygotsky's cultural–historical theories of the zone of proximal development and developmental crises. A theory of children's development should include more directly than it has in the past the practice in children's everyday institutions and the conditions the society give children for development and at the same time attempt to grasp the child's perspective. A theory of children's development has to be anchored in societal values, that is, what different institutions value as a good life. Examples from my research on children in Danish kindergartens and immigrant children in Danish schools are used to exemplify the arguments.  相似文献

Primary nurses can use their theoretical, pragmatic and creative efforts by being involved in groups such as the one mentioned in this article. For those unfamiliar with group work, course and seminars are available through colleges, universities and inservice departments of major hospitals. Even with this knowledge base, adequate supervision by a clinical expert is essential in providing this type of care to patients. Taking on a leadership role, the nurse becomes accountable for presenting effective interpersonal techniques with the hope that the patient would adapt and integrate such skills when he returns to the outside community.  相似文献

ABSTARCT

Social isolation and high levels of psychosocial stress are common among parents of children who are hospitalized for cancer treatment. Many hospital-based programs offer support groups for parents, however there is little published guidance or systematic evidence on their effectiveness. This study describes the development and program evaluation of a 16-week parent support group offered to parents (N = 10) of children hospitalized for pediatric cancer or bone-marrow transplant. A summary of the pilot evaluation is provided and successes, barriers, and areas for future improvement are discussed.  相似文献

This paper studies the large deviations behaviour of the kernel estimator of a probability density f, by considering the case when the kernel takes negative values. It establishes large and moderate deviations principles for the kernel estimators of the partial derivatives of f. The estimators of the derivatives exhibit a quadratic behaviour for both the large and the moderate deviations scales, whereas for the density estimator there is a classical gap between the large deviations and the moderate deviations asymptotics.  相似文献

We apply the stochastic approximation method to construct a large class of recursive kernel estimators of a probability density, including the one introduced by Hall and Patil [1994. On the efficiency of on-line density estimators. IEEE Trans. Inform. Theory 40, 1504–1512]. We study the properties of these estimators and compare them with Rosenblatt's nonrecursive estimator. It turns out that, for pointwise estimation, it is preferable to use the nonrecursive Rosenblatt's kernel estimator rather than any recursive estimator. A contrario, for estimation by confidence intervals, it is better to use a recursive estimator rather than Rosenblatt's estimator.  相似文献

The aim of this paper is to explore mortality in Quebec during the nineteenth century from a demographic perspective. During the nineteenth century, there was excess urban mortality in various countries; in order to identify such mortality differentials, we compared mortality indicators for the province of Quebec and then for the urban areas of Montreal and Quebec City. Using data from various studies, we produced life tables and compared life expectancies. We show that at different times during the nineteenth century, spatial variations in mortality levels across the province of Quebec and its urban areas were significant. According to the data we analyzed, mortality is undoubtedly higher in urban areas even though a convergence in trends took place towards the end of the century, resulting in an overall reduction in mortality. Also, exploring life expectancies within a cohort approach at times of fast-changing mortality patterns has proved to be instructive. Life expectancy estimates based on a cross-sectional approach were systematically lower than those resulting from a cohort-specific one. Trends diverged to a greater extent beginning with the 1870 cohort, reflecting the improvements made from that point on to World War II. Since current mortality levels are substantially determined by the cumulative effects of past behaviour specific to each generation, it is quite obvious that mortality analysis will reveal its true meaning only with the help of cohort life tables.  相似文献
