A model is presented to generate a distribution for the probability of an ACR response at six months for a new treatment for rheumatoid arthritis given evidence from a one- or three-month clinical trial. The model is based on published evidence from 11 randomized controlled trials on existing treatments. A hierarchical logistic regression model is used to find the relationship between the proportion of patients achieving ACR20 and ACR50 at one and three months and the proportion at six months. The model is assessed by Bayesian predictive P-values that demonstrate that the model fits the data well. The model can be used to predict the number of patients with an ACR response for proposed six-month clinical trials given data from clinical trials of one or three months duration.  相似文献

Creating a safe place for open classroom engagement on topics of multiculturalism and diversity requires intentionality and honesty. Unless all, professor and students alike, feel safe talking about related issues, the attempt will result in a politically correct exercise in futility. This article focuses on how intentional pedagogical efforts can create a classroom environment conducive to therapeutic self-reflection and result in personal and professional transformation. In both Scotland and England, reducing anti‐social behaviour (ASB) and building a culture of respect are key central government priorities. Accordingly, since 1997 both jurisdictions have seen the introduction of a raft of punitive legislation. Over recent years, however, there have been signs that the official ASB agenda has shifted away from a reliance on ‘enforcement measures’ towards a more balanced approach incorporating measures to address the underlying causes of problem behaviour. With their emphasis on ‘whole family’ approaches and parenting interventions, ASB family projects, pioneered by the Dundee Families Project (DFP), are seen to respond to official concerns about social exclusion and have been promoted as an effective and sustainable response to ASB. In both Scottish and English jurisdictions official endorsement of the DFP model has been marked by government‐funded programmes to ‘roll out’ this concept more widely. Drawing on findings from a recent evaluation of ASB family projects operating in Scotland combined with scrutiny of evidence from a number of English studies, this article explores, in a comparative manner, the differences and similarities in the policy discourses and models of practice employed by Scottish and English projects. More specifically, we critically appraise the role played by sanctions in evoking service user engagement.  相似文献

Burns and Stalker's theory of organic/mechanistic structures (1961, The Management of Innovation. London: Tavistock) has been widely used. However, review of the empirical literature revealed inconsistencies in how the concepts have been operationalized. These inconsistencies may interfere with the ability to consolidate knowledge. This paper reviews the various ways in which researchers have operationalized the concepts, and summarizes the empirical findings derived from these operationalizations. In doing so, it highlights gaps and opportunities for future empirical and methodological work, suggesting the need to further our theoretical conceptualization of the concepts and to draw attention to Burns and Stalker's ( 1961 ) largely neglected corollary of the employee experience. As such, this review provides a road map for future exploration of the wide‐ranging implications associated with organic and mechanistic structures.  相似文献
