When Form Fails to Follow Function

Abstract

Benjamin and Gollan'S (2003) new primer on conducting child custody evaluations, published by the American Psychological Association, presents the authors' flawed evaluation protocol with little discussion of the risks and benefits of the practices they advocate. The authors fail to note where their model varies significantly from the custody evaluation guidelines required by law in some states, and promulgated by various professional organizations. The most troubling practices include not interviewing and assessing pre-adolescent children, destroying videotapes of interviews and observation sessions, organizing the evaluation around parental allegations, use of a suggestive allegation checklist, and limiting consideration of information from collateral witnesses to declarations submitted under penalty of perjury.

Exposure Assessment: Then, Now, and Quantum Leaps in the Future

Health risk assessments have become so widely accepted in the United States that their conclusions are a major factor in many environmental decisions. Although the risk assessment paradigm is 10 years old, the basic risk assessment process has been used by certain regulatory agencies for nearly 40 years. Each of the four components of the paradigm has undergone significant refinements, particularly during the last 5 years. A recent step in the development of the exposure assessment component can be found in the 1992 EPA Guidelines for Exposure Assessment. Rather than assuming worst-case or hypothetical maximum exposures, these guidelines are designed to lead to an accurate characterization, making use of a number of scientific advances. Many exposure parameters have become better defined, and more sensitive techniques now exist for measuring concentrations of contaminants in the environnment. Statistical procedures for characterizing variability, using Monte Carlo or similar approaches, eliminate the need to select point estimates for all individual exposure parameters. These probabilistic models can more accurately characterize the full range of exposures that may potentially be encountered by a given population at a particular site, reducing the need to select highly conservative values to account for this form of uncertainty in the exposure estimate. Lastly, our awareness of the uncertainties in the exposure assessment as well as our knowledge as to how best to characterize them will almost certainly provide evaluations that will be more credible and, therein, more useful to risk managers. If these refinements are incorporated into future exposure assessments, it is likely that our resources will be devoted to problems that, when resolved, will yield the largest improvement in public health.

Placing Clinical Social Workers “Between a Legal Rock and a Practice Hard Place”: Reactions to Madden and Parody

The debate about the veracity of incest memories uncovered in therapy is often impassioned and acrimonious. Recent statements by various professional organizations, advising that corroboration of recovered memories is the only method for determining their accuracy, have not decreased the intensity of this debate. This response to Madden and Parody's paper discusses the importance of corroboration of recovered memories of sexual abuse and details the clinical, scientific, and legal foundations for this approach. A point of confusion in Madden and Parody's paper and an important aspect of this response is the difference between clients who uncover completely new memories of childhood sexual abuse and clients who enter therapy with a history of sexual abuse. Those clients with new memories are the focus of this debate and the standard of care proposed in this paper.

学前教育应当为儿童提供什么样的经验以及如何组织这些经验,将直接影响儿童的成长与发展。随着托幼一体化成为我国学前教育发展的趋势,寻求建设一体化课程体系的有效路径是我国亟须解决的问题。新西兰已建立起比较完善的托育在内的托幼一体化课程标准,从培养目标、内容、实施到评价等一系列经验,给世界各国提供了典型借鉴。目前,我国托幼一体化课程建设处于探索阶段,相关研究比较有限。本文通过解析新西兰托幼一体化课程纲要的逻辑起点和内容要素,以期为破解我国托幼一体化课程体系的建构提供思考方向。     

Indications for,and timing of,planned caesarean section: A systematic analysis of clinical guidelines

BackgroundThere has been a worldwide rise in planned caesarean sections over recent decades, with significant variations in practice between hospitals and countries. Guidelines are known to influence clinical decision-making and, potentially, unwarranted clinical variation. The aim of this study was to review guidelines for recommendations in relation to the timing and indications for planned caesarean section as well as recommendations around the process of decision-making.MethodA systematic search of national and international English-language guidelines published between 2008 and 2018 was undertaken. Guidelines were reviewed, assessed in terms of quality and extracted independently by two reviewers.FindingsIn total, 49 guidelines of varying quality were included. There was consistency between the guidelines in potential indications for caesarean section, although guidelines vary in terms of the level of detail. There was substantial variation in timing of birth, for example recommended timing of caesarean section for women with uncomplicated placenta praevia is between 36 and 39 weeks depending on the guideline. Only 11 guidelines provided detailed guidance on shared decision-making. In general, national-level guidelines from Australia, and overseas, received higher quality ratings than regional guidelines.ConclusionThe majority of guidelines, regardless of their quality, provide very limited information to guide shared decision-making or the timing of planned caesarean section, two of the most vital aspects of guidance. National guidelines were generally of better quality than regional ones, suggesting these should be used as a template where possible and emphasis placed on improving national guidelines and minimising intra-country, regional, variability of guidelines.     

Listen to the Elders: Design Guidelines for Affordable Multifamily Housing for the Elderly Based on Their Experiences

There is an increasing demand for affordable multifamily housing for a rapidly growing population of ethnic elders. Using the lived experiences of Korean American elders currently residing in affordable housing in the Midwest as one such example, this study seeks to develop design guidelines to meet the specific living needs and desires of this aging population. Verified against well-established building standards and accepted guidelines, these design recommendations lay the groundwork for multifamily housing that will assist elders to age in place in safer, healthier and happier environments.     

Web based resource for Statistical Consultants in the Pharmaceutical Industry

The intention of this article is to highlight sources of web-based reference material and software that will aid consulting statisticians when designing clinical trials. The article includes websites that provide links to explanation of statistical concepts for non-statisticians, regulatory guidelines, and free statistical study design software.
