Abstract

The adequacy of provisions for young people leaving care and in aftercare in the Republic of Ireland has been the subject of recent policy attention. A landmark report, the Ryan Report (2009), into historic abuse in state institutions recommended strengthening provisions in this area. However, the legislative basis for aftercare remains relatively weak and services for young people leaving care remain ad hoc and regionally variable. This article outlines the current context of leaving and aftercare provision in the Republic of Ireland and traces some of the recent policy debates and recommendations in this area. A genealogical analysis of leaving care and aftercare provision highlights that this issue has historically only emerged as a concern in the context in which young people leaving the care system are perceived as a “threat” to social order. It is argued that the failure to adequately reform leaving and aftercare provision is reflective of wider social inequality and of a context in which young people in care are largely invisible from view.  相似文献

This paper focuses on the introduction and development of midwifery education and training in Sydney during the last decades of the 19th century. The aim of the training, it is argued, was to displace the lay midwives by trained midwifery nurses who would work under medical control. The lay midwives were one of the largest occupational groups among women and two-thirds of births in NSW were being delivered by them in the late 19th century. It was a period of professionalisation of medicine and medical men laid claim to midwifery as a legitimate sphere of their practice and saw it as the gateway for establishing a family practice. The lay midwife stood in the way of their claim. The training programs were established purportedly to control maternal mortality. From the beginning in 1887 medical men were in control of midwifery nurse training. In addition to training at the Benevolent Society Asylum, three more women's hospitals were established in the 1890s in Sydney making it possible to train a stream of midwifery nurses. The midwifery nurses were charged exorbitant fees for their training; the fees contributed substantially towards running the new hospitals that delivered birth services to the poor and destitute women mostly in their homes. The midwifery nurses worked hard in miserable conditions under the guise of clinical experience required for training. When a critical mass of poorly trained midwifery nurses were in the offing, a Bill was introduced into the Parliament in 1895, restricting registration to midwifery nurses and this would have eliminated the lay midwife if passed. It took more than two decades to get a Registration Bill passed in the NSW Parliament.  相似文献

PURPOSE: We need solid estimates of maternal mortality rates (MMR) to monitor the impact of maternal care programs. Cambodian health authorities and WHO report the MMR in Cambodia at 450 per 100,000 live births. The figure is drawn from surveys where information is obtained by interviewing respondents about the survival of all their adult sisters (sisterhood method). The estimate is statistically imprecise, 95% confidence intervals ranging from 260 to 620/100,000. The MMR estimate is also uncertain due to under-reporting; where 80-90% of women deliver at home maternal fatalities may go undetected especially where mortality is highest, in remote rural areas. The aim of this study was to attain more reliable MMR estimates by using survey methods other than the sisterhood method prior to an intervention targeting obstetric rural emergencies. PROCEDURES: The study was carried out in rural Northwestern Cambodia where access to health services is poor and poverty, endemic diseases, and land mines are endemic. Two survey methods were applied in two separate sectors: a community-based survey gathering data from public sources and a household survey gathering data direct from primary sources. FINDINGS: There was no statistically significant difference between the two survey results for maternal deaths, both types of survey reported mortality rates around the public figure. The household survey reported a significantly higher perinatal mortality rate as compared to the community-based survey, 8.6% versus 5.0%. Also the household survey gave qualitative data important for a better understanding of the many problems faced by mothers giving birth in the remote villages. There are detection failures in both surveys; the failure rate may be as high as 30-40%. PRINCIPLE CONCLUSION: Both survey methods are inaccurate, therefore inappropriate for evaluation of short-term changes of mortality rates. Surveys based on primary informants yield qualitative information about mothers' hardships important for the design of future maternal care interventions.  相似文献

The post-Victorian anthropologist Baldwin Spencer was the first to investigate the central and northern aboriginal tribes of Australia. His ethnographic works in this area have greatly in-fluenced related disciplines and studies in fields such as kinship, totem worship, and primitive reli-gions. In the field of classics and anthropology, Spencer’s academic heritage has received wide-spread respect and recognition, and has made sub-sequent academic discussion possible. In order to present Spencer’s personal experiences and aca-demic ideas clearly and comprehensively, it is nec-essary to return to the post-Victorian context, and comb Spencer’ s life history and academic history. Taking important clues from various times an e-vents in his life, the paper introduces three peri-ods:Spencer’s early training in the discipline and his epistemic background, his medium-term eth-nographic investigations and works, and the later investigations of Tierra del Fuego. Textual study, based on Spencer’s life history and academic histo-ry, is very useful to understand his ethnographic investigations. Sir Walter Baldwin Spencer ( 1860 -1929 ) was born on 23 June, 1860 in Lancashire, Eng-land. Spencer was educated at Old Trafford School and at the Manchester School of Art. His interest in art and sketching was lifelong, and would reveal itself in his competence as a scientific draftsman and illustrator ( D. J. Mulvaney,1990 ) . Entering Owens College ( Victoria University of Manchester) in 1879, Spencer intended to study medicine. In-spired by Milnes Marshall, a disciple of Darwin disciple, he became a committed evolutionary biol-ogist, soon abandoning conventional religion. He entered the University of Oxford in 1881 to study
science under Professor H. N. Moseley, who com-bined an enthusiasm for evolutionary biology with ethnological interests. Spencer grasped Oxford ’ s diverse opportunities, which included lectures by Ruskin and E. B. Tylor. In 1887, Spencer ar-rived at Melbourne University . With his colleague Professor David Masson, Spencer helped to trans-form university standards and they co-operated as entrepreneurs of Australian science. Spencer was recruited as zoologist and photog-rapher in the 1894 Horn scientific exploration ex-pedition to central Australia. His anthropological interest was rekindled when he met F. J. Gillen, the Alice Springs postmaster, during the expedi-tion. In 1896 Spencer joined Gillen for the most intensive fieldwork then attempted in Australia. The Native Tribes of Central Australia ( 1899 ) , which resulted, was to influence contemporary theories on social evolution and interpretations of the origins of art and ceremonial practices. Spencer and Gillen drove a buggy from Oodnadatta to Borroloola in 1901-02 . Their research resulted in The Northern Tribes of Central Australia ( 1904 );Haddon had written that the names of Spencer and Gillen are familiar to every ethnologist in the world, and probably no books on ethnology have been so wide-ly noticed and criticized as have The Native Tribes of Central Australia and The Northern Tribes of Central Australia (A. C. Haddon ,1902). To assist the Government of the Common-wealth, Spencer was appointed Special Commis-sioner for Aboriginals in the Northern Territory, and also their Chief Protector. He also led three other scientists, including J. A. Gilruth, on the 1911 Preliminary Scientific Expedition. Native Tribes of the Northern Territory of Australia (1914)
described his ethnographic observations and the ex-tensive collections made on the expedition. At the government ’ s request, Spencer visited Alice Springs and Hermannsburg in 1923 . He published The Arunta: A Study of a Stone Age People ( 2 vols, 1927 ) to respond to the criticisms derived from Carl Stretlow and defend his work. A popular rewrite of previous books followed—Wanderings in Wild Australia ( 2 vols, 1928 )—this time under his sole authorship. Spencer retired as emeritus professor in 1919 . His nerves and his judgments were impaired from the strain of continuous overwork, the virtual disin-tegration of his marriage, and he was finally hospi-talized in 1921 . His health improved and within two years he resumed anthropological activities and rebuilt his art collection. He sailed to Tierra del Fuego together with Jean Hamilton to undertake anthropological studies early in 1929. Spencer, with an unrivalled record of anthropological field-work in Australia, undertook this journey to fill a gap in our knowledge, and compare very different and remote races of mankind (A. C. H. ,1931). However, his gallant attempt was prematurely frus-trated by his death from angina pectoris, at which point he had been only two months in the field. His notes were organized and published as Spencer’s Last Journey (1931). His achievements were recognized. Elected as a fellow of the Royal Society in 1900 , he was ap-pointed C. M. G. in 1904 and K. C. M. G. in 1916 . Manchester University conferred him with an honorary doctorate of science, while Melbourne a-warded him a doctorate of letters. Exeter College, Oxford, elected him to an honorary fellowship in 1907 , and stained glass in its hall commemorates his contribution. James Frazer’s letter to Spencer in 1899 was prophetic: books like mine, merely speculative, will be superseded sooner or later ( the sooner the better for the sake of truth) by bet-ter induction based on fuller knowledge;books like yours, containing records of observations, will nev-er be superseded ( John M. Cooper, 1932 ) . Therefore, the sense and value of reading and un-derstanding Spencer and his books today goes with-out saying.  相似文献

BackgroundTraditional and gestational surrogate mothers assist infertile couples by carrying their children. In 2005, a meta-analysis on surrogacy was conducted but no study had examined empathy and maternal–foetal attachment of surrogate mothers. Assessments of surrogate mothers show no sign of psychopathology, but one study showed differences on several MMPI-2 scales compared to a normative sample: surrogate mothers identified with stereotypically masculine traits such as assertiveness and competition. They had a higher self-esteem and lower levels of anxiety and depression.Research objectiveTo determine if there is a difference in empathy and maternal–foetal attachment of surrogate mothers compared to a comparison group of mothers.MethodsThree groups of European traditional and gestational surrogate mothers (n = 10), Anglo-Saxon traditional and gestational surrogate mothers (n = 34) and a European normative sample of mothers (n = 32) completed four published psychometric instruments: the Interpersonal Reactivity Index (empathy index), the Hospital Anxiety and Depressions Scale and the MC20, a social desirability scale. Pregnant surrogate mothers filled the Maternal Antenatal Attachment Scale (n = 11). Statistical non-parametric analyses of variance were conducted.FindingsDepending on cultural background, surrogate mothers present differences in terms of empathy, anxiety and depression, social desirability and quality of attachment to the foetus compared to a normative sample.ConclusionsEnvironment plays a role for traditional and gestational surrogacy. Surrogate mothers of both groups are less anxious and depressed than normative samples. Maternal–foetal attachment is strong with a slightly lower quality of attachment. Surrogate mother's empathy indexes are similar to normative samples, sometimes higher.  相似文献

Abstract

Despite continued interest and growth in special care units in the United States, there is no uniform agreement on characteristics for these units, specific outcomes, or satisfaction with them. This study examined family satisfaction with Dementia Special Care Units (SCUs) using a modified standardized instrument and follow-up interviews. The study included five SCUs in one state, three participating in a dementia SCU demonstration project. Family outcomes were evaluated using surveys and interviews. The only significant difference between facilities in the demonstration project and the comparison facilities was one variable noting family with residents in the demonstration project had more positive perceptions regarding resident safety. The study illustrates a need for policy makers to more clearly identify and regulate special aspects of SCUs, taking family perceptions into consideration.  相似文献
